Complete all sections – incomplete forms will not be considered.
This form should be completed where the named Customer has chosen an SSE Energy Supply Ltd branded Supply (Southern Electric, Scottish Hydro, SWALEC) and the proposed
new supply is required to be registered by SSE, metered and energise it (make live) for the first time. It is the customers’ responsibility to ensure that all site works are completed
by the Distribution Network Operator (DNO) that is installing the supply and there appointed electrical contractor connecting the customer’s electrical installation to it. In this
electricity industry process, a Supplier’s (such as SSE) responsibility, is only to register the supply, appoint the necessary Agents (such as specified below) and arrange for a
D0142 flow (electronic industry message) to the Meter Operator to install the meter and a D0134 flow to the (DNO) that is installing the supply to energise it (make live).

Declaration
I/we agree to be charged for electricity in accordance with the Terms and Conditions and Pricing Arrangements under the SSE Energy Supply Limited: “Scheme for determining
the Terms and Conditions of a Deemed Contract for Supply of Electricity” taking into account the voltage of supply and the above information (unless there is a contract in
place between the Parties for the above supply that supersedes it).
I hereby confirm I am the authorised signatory for the customer detailed above ………………………………………………………………… (signature)
I request SSE Energy Supply Ltd to start the registration process and make arrangements for a new electricity meter to be installed at the premises above. I accept and understand
that I will be liable for any cost of unsuccessful attempts by the Meter Operator to install their metering equipment where the customer or their appointed agent(s) fail to provide
access or the site is not ready. I accept and understand that the Meter Operator is responsible for the installation or removal of meters and carries out this work on behalf of SSE. I

NOTES – For use in completing this NSF05 form (See also notes by hovering the “cursor” over the blue
on the form).
(1) Core MPAN: Please state the new Core MPAN (Meter Point Administration Number) the Distribution Network Operator (DNO) who has provided the supply must
issue to you. This is a 13 digit number, which will be unique to the supply you are having installed. It is your DNO’s responsibility to issue this number. Please refer to
your DNO if you do not know the MPAN.
(2) Site Access: please provide the site contact name and number of the person who will meet and provide access for the metering engineer. Most engineers require
your electrician or a customer representative at site when they fit the meter.
(3) Distribution Network Operator: Please provide details of the Distribution Network Operator (DNO) carrying out your service installation and DNO Project Manager’s
name and contact telephone number. Please also provide the Job Reference Number issued in case we have need to contact the DNO. The DNO will provide a quotation
for the new supply (that needs to be accepted by the customer and paid for), in many cases with engineering drawings that need to be passed on to the customer’s
electrical contractor, a Connection Agreement to be signed by the customer and the supply national registration “MPAN” number.
(4) Electrical Contractor: Please provide electrical contractor’s name and number in case there is a need to ask any technical questions.
(5a) Supplier / Agents: Please indicate which brand of SSE you wish to supply your site; Southern Electric, Scottish Hydro Electric or SWALEC If you do not have a
preference request “SSE to appoint”.
(5b) Half Hourly (HH) metered supplies (and non Half Hourly (NHH) Automatic Meter Reading (AMR) where the customer has a direct contract), the Meter Operator must
be appointed and have a signed contract in place before registration starts. For all other NHH installations request “SSE to appoint”.
For Half Hour metered supplies a Data Aggregator and Data Collector will have to be appointed. Non Half Hour metered supplies a Data Aggregator, Data Collector
and Data Retriever will need to be appointed. If you have a direct contract with any of these “Agents” please state their details, otherwise request “SSE to appoint”.
(6) Meter Type & Capability: There are 2 types of metering – Whole Current (WC) used for supplies up to 69kVA (3 phase, 100amp / phase) and Current Transformer (CT)
used for supplies of 70kVA or over. If you are unable to provide this information please refer to the Distribution Network Operator (DNO) that provided the supply. N.B.
Whole current metering cannot be installed on CT supplies and vice-versa.
(7) For Current Transformer (CT) connections: You must confirm that the cut out panel is in place, the CTs are installed and wired to the CT chamber Test Block, the
customers cables “tails” from the Customer’s main switch to the CT Chamber (including “Lugs” crimped onto the end each of the tails) are available for connection and
the DNO cut out fuse holders have been left in the sealed CT cabinet (See CT panel checklist).
N B – It is the joint responsibility of the customer’s electrical contractor working with the Distribution Network Operator’s staff arranging the supply to agree the
equipment and work provided by each party.
(8) CT Ratio: In order to ensure that the correctly programmed meter is installed, please ask your Distribution contact to provide you with details of the appropriate ratio
(e.g. 100/5, 500/5). N.B. If this ratio is incorrect, the customer’s billing will be incorrect.
(9) Available Capacity: This is measured in Kilo Volt Amps (kVA) and is the capacity proposed by your electrical contractor / consultant sufficient to cover your electricity
supply maximum demand requirements and should have been incorporated in the design of the new connection by the Distribution Network Operator and shown in the
Connection Agreement between the Customer and the Distribution Network Operator. If you do not know this or if you are unsure please contact your contract
electrician or the DNO that is providing the supply. N.B. This has a bearing at what your metering is set at and how it is subsequently quoted and billed. Please do not
guess or confuse this with your supply’s amperage (Amps).
(10) Meter installation date (and energising / making live the supply): Generally we require 5 weeks’ notice to arrange a new connection. This includes 5 working days
for a successful registration (without which the metering / switching on of the supply cannot go ahead) and 10 working days to arrange for the metering and switching
on the supply). N.B. Making a supply live without supplier registration and metering is theft.
N.B. If you require an appointment earlier than 10 working days, we will contact the Meter Operator to see if it is possible to do the job earlier. There is no guarantee of
an earlier appointment and we may still have to accept the 10 working days. If an earlier date is available we will pass on the Meter Operator Early Appointment Charge
to you by adding a miscellaneous charge to your first bill. If an urgent / out of Hours job is required the charge will be higher and will be stated by the meter operator.
This charge will normally be required by cheque to be given to the Meter Installer when he calls to carry out the job. The Meter installer will not complete the job
without payment.
In this electricity industry process, a Supplier’s (i.e. SSE) responsibility is only to register the supply, appoint Agents and arrange for a D0142 flow (electronic industry
message) to be sent to the Meter Operator to install the meter and to send a D0134 flow to the DNO installing the supply to energise (make live). All physical work on
site (apart from installing the meter itself) is the responsibility of the DNO or the electrical contractor.
(11) Electrical Contractor Work: A WIRING COMPLETION CERTIFICATE is required to confirm that your electrical contractor has completed and tested the customers’
electrical equipment and internal wiring to a point that it is safe to connect.
(12) Distribution Work (DNO work): Please ensure all distribution work has been completed and the service cable is energised BEFORE advising us to book any meter
job. The engineer will not fit a meter to an incomplete supply due to safety issues. If the supply is going to be CT metered please ensure that the cabinet is in situ or has
been arranged to be in situ before the metering job goes ahead. If a metering engineer calls and the work cannot be completed due to no fault of their own (e.g. site not
ready) we will be charged for this abortive visit and will pass this charge on to the customer.
(13) Meter Position / Location: on order to assist with future meter reading and billing queries, it would be useful to know the intended Meter position and location. E.g.
Utility cupboard, ground floor in main factory.
(14) Service Plans: Please choose a Quarterly or Monthly service plan, taking into account the Available Capacity of the supply and the energy usage.
HH or NHH. If the available capacity is less than 200kVA we will set this up as Non Half Hourly metered unless specifically requested to set up as Half Hourly metered. If
over 200kVA we will set the supply up as “Half Hourly” metered. This involves setting up a telecommunications link which downloads half hourly data every night. You
must enter into a separate half hourly meter operator agreement for this service.
(15) Supply Type: There are 3 types of supply available: “Single Phase”, “Three Phase” or occasionally mainly in rural areas “Split Phase”. If you do not know the Supply
Type that has been installed, please refer to the quotation or the Connection Agreement issued by the DNO that provided the new supply or the engineers that
completed the install.
(16) Voltage: There are 3 voltage levels as follows: Low Voltage (LV) - under 1000 volts, High Voltage (HV) - 1,000 volts and above but less than 22,000 volts and Extra
High Voltage (EHV) - 22,000 volts and above. Also supplies can be Substation (Sub) or Network (Net) fed. Please advise only if you know how the supply is fed. It is
important to provide the correct supply voltage as this is one of the factors determining your electricity charges. If you do not know the supply voltage, please refer to
the quotation or Connection Agreement issued by the DNO that provided the new supply or the engineers that completed the install.
(17) Supplies under SSE Energy Supply Limited: “Scheme for determining the Terms and Conditions of a Deemed Contract for Supply of Electricity” are only available for
supplies below 22,000 volts.
(18) Emailed pictures of the meter position should be provided for more complicated whole current (WC) and all CT metering installations.
