Certain sales and field service support positions will normally qualify for a fleet vehicle, while other positions where an employee travels more than 10,000 miles per year on business may be assigned a Company vehicle subject to a determination of need by the operating management and subject to Human Resources approval. The Fleet Department may also operate pool vehicles in locations where business conditions warrant, or provide vehicles for maintenance, security or similar Company requirements as approved by the President and VP of Finance and Administration. BASIC RESPONSIBILITIES AND REQUIREMENTS It is the responsibility of the Fleet Department to recommend appropriate vehicle types to meet the business needs as established by operating management. The Fleet Department is authorized to assign, replace, change, transfer, transport or relocate any existing vehicle with the intent to optimize overall utilization and minimize cost consistent with best industry practices for safety and reliability. Drivers (and an authorized spouse) must provide a copy of a current valid driver's license, and agree to allow the Company to perform a Motor Vehicle Record (MVR) check prior to vehicle assignment and on a regular basis thereafter, normally annually. Authorized drivers must have and maintain an acceptable driving record, free of any of the following serious violations within the previous three-year period: Driving while intoxicated or under the influence of alcohol or illegal drugs. Drivers who are convicted of a violation of Driving Under Influence (DUI) or Driving While Intoxicated (DWI) by alcohol or a controlled substance will lose their company vehicle driving privileges for a period of three years. Between the issuance of a citation to a driver for impaired driving and a conviction for same, use of company vehicle will be suspended until the driver is cleared of the charge. Hit and run or leaving the scene of an accident Murder or assault with a motor vehicle Reckless driving which resulted in bodily injury or property damage Negligent homicide Any other serious incident, which resulted in suspension or revocation of driving privileges Use of false motor vehicle documents such as license or registration If a serious violation does occur, the driver must notify both their manager and Fleet department immediately. Agfa fleet vehicle privileges may be revoked: If an accident results in a fleet driver’s conviction for any of the above-referenced violations When two "At Fault" accidents determined by Accident Committee occur within a three-year period. The suspension for any reason of a driver’s driving privileges, license or permit in any state. Due to any failure (by driver or authorized spouse) to report in writing to the operating manager and the Fleet Department notice of suspended or revoked driving privileges, whether temporarily or permanently suspended by the licensing authority. Upon failure to perform routine preventive maintenance thereby risking personal safety and Company cost. Upon failure to promptly resolve unpaid traffic or parking violations. Upon misuse or abuse of fuel cards, maintenance system authorizations, or similar Company fleet tools. Drivers are responsible at all times for the legal and authorized operation of the fleet vehicle. Drivers are expected to drive defensively, are responsible for keeping the vehicle in a safe and clean condition at all times and are expected to arrange for the proper licensing and maintenance of the vehicle as required. Drivers are expected to obey all traffic controls, laws and instructions from traffic enforcement personnel. 533575286 2 Revised: 03/20/2013 Cargo, loads, tools, brochures, etc. should be stowed securely in appropriate trunk locations and loads must not exceed the vehicle GVW or tire rating. Do not carry non-essential weight as this impairs fuel economy. Drivers must report to ARI and their Manager any accident or damage involving a Company vehicle within 24 hours. Agfa will review all accidents and incidents involving damage to Company vehicles. Determination utilizing commonly accepted insurance and fleet industry standards will be made to classify any “At Fault” accident. The individual’s culpability, the costs associated with the accident, and the individual’s length of service and prior driving history with Agfa may be considered in determining the appropriate penalties, if any, to be assessed to the individual. All occupants must have seat belts fastened during any movement of the vehicle. It is expected that the employee will be the primary driver for the vehicle with the intent to be driven only by the assigned employee in the conduct of business. The vehicle may be incidentally operated only by a spouse, provided they are at least 21 years of age and hold a valid driver’s license and agree to a MVR check. Personal use of fleet vehicles is permitted. However, employees who are assigned vehicles are expected to keep accurate monthly records of actual business versus personal mileage use, and to properly document this usage through ARI. Employees will be responsible for any personal use tax liability in accordance with the methodology approved by the Agfa Tax Department and administered through the Payroll Department. Use of pool vehicles (vehicles centrally garaged for multiple drivers) requires specific written authorization. Consult the fleet department for reservation requirements and authorization paperwork. PROHIBITED ACTIVITIES Agfa strictly prohibits the following: Employees must not drive under the influence of alcohol or illicit drugs. Employees must not operate the vehicle if taking a legal medication labeled with a warning that it could impair driving ability. In addition, employees must refrain from other distracting activities, i.e.; reading, eating, etc., while driving. Smoking by drivers or passengers is not allowed. Acceptance of any form of compensation for carrying passengers or materials is not allowed, and transporting of a hitchhiker is a prohibited activity. Use of the vehicle for any commercial activity or business venture other than that pertaining to an employee's job duties is prohibited. Transportation of firearms, ammunition, fireworks, explosives or other hazardous or regulated materials is prohibited. Company vehicles are not to be modified or altered in any way (i.e., adding, removing or changing of equipment or options, etc.). Installation of any aftermarket equipment or options is strictly prohibited. If special equipment or modifications to the vehicle are necessary, you must obtain advance written approval by both your operating management and the Fleet Department. Company vehicles may not be used in towing applications or modified for towing purposes, unless specifically required for Company business and first jointly approved by operating management and the Fleet Department in writing. No bumper stickers of any kind or description may be affixed to the company vehicle. Operation of the vehicle by unauthorized and or unlicensed drivers, those with provisional licenses or student permits is prohibited. The fleet vehicle is not to be operated by customers, children, relatives or other persons, regardless of license possession. Company policy requires that only Regular Unleaded fuel be used. Use of higher grades of fuel is not necessary or allowed without the express written approval of the Fleet Department. NEW VEHICLE SELECTION 533575286 3 Revised: 03/20/2013 Each model year a standard vehicle selector is recommended by the Fleet Department and ARI to meet the requirements of each specific job position. Vehicles are reviewed by senior operating management to ensure they are selected based on appropriate safety, comfort, job requirements and economic criteria established by Agfa. ORDERING Based on the Agfa policy for vehicle replacement, you will be notified approximately 90 days in advance of the projected replacement date of your current vehicle for the purposes of ordering your replacement vehicle. Replacement schedules are adopted to achieve maximum resale value and are determined by the specific use of the vehicle. Agfa normally initiates an annual fall and/or spring order replacement cycle. Vehicles are normally replaced starting at 70,000 miles or 40 months in service for passenger vehicles, whichever occurs first. Exceptions to this schedule may exist, including extending the above mentioned criteria, and will be handled at the Company's discretion. DRIVER NOTIFICATION You will be notified by email when it is time for a new vehicle order to be placed for you. Detailed instructions will be provided in the email you receive to order a new vehicle. All orders will be placed via the ARI Driver Insights web ordering site. ORDER PLACEMENT Upon receipt of a properly executed web requested order, your vehicle will be ordered from the manufacturer. ORDER ACKNOWLEDGMENT ARI will acknowledge the placement of your order after they receive a confirmation from the manufacturer. This acknowledgment should be properly reviewed and any corrections or possible errors immediately brought to your ARI and Agfa Fleet Coordinator's attention. DELIVERY Approximately 8 to 12 weeks from the time of order placement, your vehicle will be produced and shipped to a delivering dealer near your home or office. DELIVERY NOTIFICATION TO DRIVER Approximately 10 to 14 days prior to expected delivery of your new vehicle, ARI will mail a set of Delivery Instructions/Delivery Receipts to you. Confirm with the specified dealer that your vehicle is ready to be delivered, and then take the documents to that dealer. DELIVERY NOTIFICATION TO DEALER The delivering dealer will be provided with similar Delivery Instructions/Delivery Receipts for your vehicle. The dealer will be provided with all necessary licensing and titling information to properly register the vehicle for you. 533575286 4 Revised: 03/20/2013 DELIVERY PROCESS Once you have confirmed that your vehicle is ready to be delivered, you will be required to go to the delivering dealer. Remember to bring your copies of the Delivery Instructions/Delivery Receipts. If there are any problems with your new vehicle, call an ARI Fleet Coordinator prior to accepting delivery of the vehicle. Unless you have elected to purchase your used vehicle, or have been instructed differently by your Fleet Coordinator or ARI, normally you will leave your old vehicle at the delivering dealer. You and a dealer representative will be required to sign both sets of Delivery Receipts. Copies of these documents should be returned to ARI in the postage-paid envelope provided for you. DRIVER PURCHASE You will normally be offered the option to purchase your leased vehicle at the time of replacement. Instructions will be provided at that time if your assigned vehicle will be made available for purchase. This affordable alternative for obtaining a quality used vehicle with which you are intimately familiar should be considered an important benefit option. USED VEHICLE LEASING In lieu of purchasing your vehicle, ARI may also offer a Used Vehicle Lease Plan. This affordable alternative should be considered during the decision making process. Upon request, more detailed information will be provided along with your used vehicle price quotation. REPLACEMENT BY NEW VEHICLE If you choose not to accept an option to purchase or lease your used vehicle, you will be instructed to leave the vehicle at the dealership where and when you receive your new vehicle. If you are instructed to leave your used vehicle at the dealership, you will be asked to complete a Condition Report with the dealer representative. If other than normal wear and tear is present, document any damage that exists on the vehicle, including a written explanation (and if available provide digital photographs). Drivers may be held responsible for unreported vehicle damage and or excessive reconditioning costs due to poor vehicle condition at turn in. CONTENTS OF VEHICLE Be sure to remove all documents such as insurance card, registration, etc. LICENSE REGISTRATION RENEWALS / TRANSFERS TO ANOTHER STATE ARI will handle all license registration renewals for ARI-owned vehicles. If your vehicle is being transferred to a state other than where it is currently titled and registered, contact an ARI Fleet Coordinator immediately. ARI will handle the reregistration and titling paperwork for you. Please note: ARI may ask you to provide certain documentation, e.g., smog/safety certification, to facilitate this process. CHANGE OF ADDRESS WITHIN THE SAME STATE If you are moving, but staying within the same state, ARI must still be notified. Contact an ARI Fleet Coordinator and your Agfa Fleet Mgr/Administrator with your new address and telephone number so your records can be updated. This information is vitally important for tax purposes and to insure that written communications reach you. 533575286 5 Revised: 03/20/2013 TITLE AND OTHER LICENSING DOCUMENTS If at any time you receive a vehicle title or other documentation not specifically related to your annual registration renewal (i.e., Personal Property Tax Notification), you should send this documentation to ARI immediately at: Automotive Resources International License and Title Department 9000 Midlantic Drive P.O. Box 5039 Mt. Laurel, NJ 08054 PARKING AND MOVING VIOLATIONS As the operator of a Company vehicle, you are personally responsible for all parking and moving violations associated with your assigned vehicle. All fines should be settled immediately with the municipality involved. If ARI receives notification of an unpaid violation, documentation may be forwarded to you for handling. A handling fee is assessed to Agfa and may be passed on to the responsible driver. Failure to promptly comply with local violations can result in increased fines and penalties, impounding of vehicles, and/or not allowing registrations to be renewed for you or other vehicles within the Company or ARI fleet. OPERATING VEHICLE OUTSIDE THE U.S. (CANADA AND MEXICO) Business obligations requiring travel into Canada is permitted and does not require prior approval. Company vehicles may not be driven into Mexico for either business or personal use under any circumstances. SEPARATION OF EMPLOYMENT In the event a driver terminates employment from Agfa, his/her company vehicle must be returned within 48 hours of separation of employment. It is the responsibility of the operating manager to ensure that the vehicle is returned with registration, insurance card, ARI/Wright Express Co-Branded Fuel and Maintenance card or other materials. Under no circumstances should a vehicle remain in the possession of a former employee for more than 48 hours after separation from the employment relationship. VEHICLE MAINTENANCE Failure to comply with the recommended service intervals will be considered abuse, and the driver will be subject to disciplinary action up to and including termination. Following the schedule of regular vehicle maintenance will provide many benefits including: 533575286 Reduced Accidents - Proper maintenance of company vehicles can substantially reduce accidents caused by brake, tire, steering, and other mechanical failures. Reduced Downtime - Preventive maintenance minimizes work schedule interruptions; the less downtime for a fleet vehicle, the more productive a driver can be in his/her position. Reduced Maintenance - Regularly scheduled inspections provide opportunities to make minor repairs and adjustments that will help prevent unnecessary and costly repairs at a later date. Note: Each company driver is also responsible for having any required vehicle inspections done on a timely basis in accordance with the applicable state safety and/or emissions control laws. Good Sales and Public Relations - Important sales and public relations value results from clean, well-maintained equipment. Each company driver is responsible for keeping clean the interior, as well as the exterior, of the assigned vehicle. 6 Revised: 03/20/2013 In an attempt to maintain your vehicle in peak operating condition, ARI provides guidelines for operating and monitoring the overall performance of each vehicle. It is your responsibility to maintain the company vehicle within the guidelines set forth by the leasing company. These guidelines follow the manufacturer's requirements for preventive maintenance. To assist you in the maintenance process, you will receive the following in your Maintenance Assistance Kit: An explanation and instructions concerning the ARI Maintenance Management Program. An ARI maintenance schedule designed to indicate the maintenance services to be performed at specified intervals. COMMUNICATIONS WITH ARI'S MAINTENANCE MANAGEMENT DEPARTMENT The ARI Fleet Management Program is designed to assist you in maintaining your vehicle and in the proper handling of routine service in a quick and cost effective manner. To accomplish this, ARI needs important information from you. When calling ARI, please have your vehicle number and odometer reading available. If calling from a service facility, have a qualified mechanic inspect the vehicle, then call ARI. If you need help in locating a facility, ARI Service Representatives are readily available to serve you. Please give them a call for service, information or advice at: 1-877-243-2003 (1-877-AGFA003) Available 24 Hours a Day, 7 Days a Week ARI, in order to protect Agfa from expensive repairs that are untimely or unnecessary, carefully screens all purchases and thus certain requested repairs might be denied approval. You may follow up directly with ARI or the Fleet Department to resolve any issues. SCHEDULED MAINTENANCE Routine scheduled maintenance may be handled by the use of the ARI National account network. An authorized service facility will perform scheduled maintenance at the required intervals, requiring only a telephone call to ARI. Normally these recommendations require an oil change at each 7500 mile interval, and an inspection of the vehicle to determine if other services are needed. Failure to comply with the recommended maintenance intervals may be considered abuse. Your signature will verify the work was completed. Service requires a Purchase Order from ARI. To obtain this Purchase Order, have the servicing facility call ARI toll-free at 1-877-243-2003. SERVICE LOCATIONS Maintenance should be performed at the following service centers. (National Account Listing) You may call ARI for locations nearest you. BASIC WARRANTY INFORMATION As you know, your vehicle comes with a warranty from the manufacturer. Refer to your Owner's Manual for specifics. Generally, the warranty is for 36 months or 36,000 miles on most components. These repairs must be done at the manufacturer's dealership if your vehicle needs a repair covered by the manufacturer's warranty. Call the ARI Fleet Management Department if you have any questions regarding your vehicle's warranty coverage. 533575286 7 Revised: 03/20/2013 MILEAGE REPORTING The Internal Revenue Service considers the personal use of your company vehicle to be a taxable fringe benefit to you. Employees should be advised that specific tax rules including reporting of mileage for personal use apply to Company provided fleet vehicles. Agfa recommends that employees consult a tax professional for specific guidance. In general, personal mileage is defined by IRS regulations as any non-business use of the vehicle. Such use would clearly include personal errands, vacations, etc. Additionally in the USA, subject to a very narrow exception, the mileage driven from the employee's home to the first work site and from the last work site to the employee's home generally is considered commuting by the IRS and is therefore considered to be personal use. The general commuting rule does not apply where, in case of the first trip from home to the first work site, or, in case of the last trip from the last work site to home, the work site is in a location outside the "metropolitan area" where the employee lives and normally works. What constitutes a "metropolitan area" is subjective, and employees must use reasonable care and discretion in the proper recording of personal mileage. Agfa and ARI have developed a process to assist you with mileage reporting. To report your monthly mileage, you can report directly through the ARI Website (www.arifleet.com/agfa.) This system is available 24 hours a day and 7 days a week. Each month you are required to report your previous month’s mileage on or before the 10th day. (For example, January mileage information should be reported no later than February 10th). If you do not report for any period you may be assessed 100% personal use for each missing period. You will need the following information to report your mileage: Your CWID / Password = 5 Characters AGFA assigned vehicle number Agfa Client Code = 0B94 (zero)B94 / Agfa Materials Client Code = 0B95 (zero)B95/ Agfa HealthCare Corp Client Code = 2B03 2B(zero)3 Prior month’s Ending Odometer (will be displayed on the Web) Total, Business and Personal Miles ARI Web Page Reporting Go to www.arifleet.com/agfa Enter your Last Name, Client Code: 0B94 (Agfa Corporation) 0B95 (Agfa Materials) 2B03 (Agfa HealthCare Corporation) a. ARI/WRIGHT EXPRESS FUEL CARD 533575286 8 Revised: 03/20/2013 This fuel card gives users pay-at-the-pump convenience at service stations of over 100,000 national and regional fuel suppliers. The Company requires you to purchase at self-service where available. This card is normally inscribed with the words “Fuel Only”. Company policy allows that only fuel can be purchased at gas stations and nothing else, i.e. car washes, oil, etc. should stay on your Company T & E card. Note: Company policy requires that only Regular Unleaded fuel be used. Other more costly grades are unnecessary and not allowed. The fuel card is assigned to and belongs to the vehicle, and a fuel card should stay with the vehicle in the event of a transfer. You are assigned a personal identification number (PIN) for use with the fuel card. The PIN tracks your individual fuel purchases and should never be shared with anyone. The PIN can be used with a valid card assigned to a vehicle. Once the fuel card belonging to the vehicle is swiped, you must enter your PIN and odometer reading (omitting tenths). The transaction details captured include fueling location, driver’s name, date, time, product purchased, number of gallons, price per gallon, and total transaction cost. Be sure NOT to keep your PIN and the fuel card together. Do not share your PIN with anyone and do not use the fuel card to fuel other vehicles. Contact the ARI Fleet Administration Program immediately to report a lost or stolen fuel card and to request a replacement card. INSURANCE CARDS Insurance ID Cards will be sent to you prior to the expiration of the old cards. At any time, if you need replacement cards, please call ARI Fleet Administration at 1-877-243-2003. If you are picking up a new vehicle at a dealership, be advised that the card will be sent to the dealership by ARI (so that they may properly license the vehicle). Make sure that you obtain this card from the dealer prior to taking delivery. ACCIDENT PREVENTION AND SAFETY Agfa is committed to promoting the safety of all employees. It is the driver's responsibility to operate the automobile in such a manner so as to prevent injury and property damage. All drivers are expected to observe speed limits and drive within the law. Speed should be adjusted to meet adverse weather conditions. Agfa employees who operate Agfa-provided vehicles and/or other vehicles in the course of Company business shall do so in a safe manner and in accordance with all applicable law. In order to raise driver safety awareness, each driver of a company vehicle must: Certify completion of any required safety training as may be determined necessary Maintain any safety kits or equipment issued for the vehicle Insist that all passengers of a company vehicle wear safety belts while traveling in the company vehicle (front and rear seats). Exercise due care in driving a fleet vehicle and drive defensively in order to reduce risk of accidents. Ensure that child restraint equipment is used appropriately and in accordance with state and local statutes. Drivers of vehicles with air bags should take care to follow the manufacturer's recommendations (i.e., children less than 12 years of age should sit in the back seat). AUTOMOBILE ACCIDENT / PHYSICAL DAMAGE (COMPREHENSIVE & COLLISION) CLAIMS Accidents must be reported immediately to: ARI 1-877-243-2003 (1-877-AGFA003) Available 24 Hours A Day / 7 Days a Week 533575286 9 Revised: 03/20/2013 Any accident involving bodily injury and/or any property damage to your vehicle and /or other persons, vehicles or property is to be immediately reported to ARI who will also notify our automobile liability insurer. If you have sustained any injuries you should notify your manager and HR specialist and report the incident to the companies EH&S Workers Compensation Administrator. REPAIR FACILITIES An ARI insurance claims representative will direct you to a body shop for estimates and repairs. Agfa may provide a rental vehicle or substitute company vehicle on your behalf so that you are not left without transportation. Please consult with your manager and if an Enterprise/National Rental Car is to be utilized, be sure to provide the Agfa ID# (National/Enterprise ID#: XZ24518). Rental car expense will be the responsibility of the employee for payment on the corporate T&E card. In the case of a 3rd party insurance claim, please provide the rental car receipt to ARI for inclusion with the claim. ACCIDENT REVIEW COMMITTEE The Agfa Fleet Accident Review Committee consists of the Corporate Fleet Manager & Administrator, Insurance Risk Manager, a member of the EH &S team, and a member from the HR team. It is the committee’s responsibility to review and investigate vehicle accident reports to determine whether or not the Agfa driver was at fault in the accident, using guidelines established by the National Safety Council and/or other industry standards. Operating management will be notified concerning which accidents were reviewed and the finding rendered. It will then be up to the local management to implement appropriate corrective action, which at a minimum should include a review of the at fault accident cause, determination as to additional safety training requirements, and may include disciplinary action including revocation of use of fleet vehicle. DEFENSIVE DRIVING A "Defensive Driver" is one who: Is careful to commit no driving errors themselves. Makes allowances for the lack of skill and improper attitude on the part of the other drivers. Does not become involved in an accident as a result of weather, road conditions, or the actions of pedestrians and other drivers. Keeps continually on the alert, recognizing accident-producing situations in advance in order to apply the necessary preventive action. Concedes the right of way when necessary to prevent an accident. The accident review committee shall establish defensive driving behavior as the expected norm, and shall review all accidents involving a Company fleet vehicle in accordance with guidelines established by the National Safety Council and/or other such similar organization. The characterization of an accident as “at fault” preventable may include but are not necessarily limited to the standards shown in the following section. AT FAULT GUIDELINES – NATIONAL SAFETY COUNCIL Too Fast for Conditions These accidents may occur when the driver failed to adjust the speed to the traffic, weather or road conditions. It does not refer to the posted speed limits, but to speeds excessive for conditions at the time of the accident. Ten miles per hour can be excessive under “icy" conditions. Weather, traffic and road conditions are not acceptable excuses for an accident. This includes loss of control and skidding (sliding). 533575286 10 Revised: 03/20/2013 Following at Unsafe Distance The driver follows the preceding vehicle so close that he or she cannot stop in time to prevent contact when preceding vehicle changes speed or direction. Improper Judgment While not a catch-all phrase, this involves vehicles proceeding forward and includes such things as failure to consider apparent hazards in congested areas while moving at a slow rate of speed, operating the vehicle while having a physical or medical impairment, etc. Cutting and Crowding The driver cuts in and out of traffic, abruptly changes lanes, or crowds other vehicles (approaching or traveling in the same direction), on curves, grades, and intersections. It also includes failure to properly signal changes. Improper Passing The driver allowed insufficient distance when passing another vehicle traveling in the same direction, passing on a curve, approaching or at the crest of a hill, over any solid line. It would also apply to failure to indicated intention to pass. It includes sideswiping approaching vehicles, and striking fixed objects, or parked or stopped vehicles while traveling at normal speed in other then congested areas. Failed to Yield Right of Way These type of accidents occur mostly at intersection, bisecting of streets, alleys and driveways (private or public), where the driver failed to yield right of way to other vehicles. Whether the driver had the legal right-of-way or not has no bearing on the preventability of the accident. The key issue is which driver had the last possibility to avoid it. When a driver strikes the other vehicle anywhere on the side behind the front wheels with the front part of the vehicle, it must be considered preventable to the driver. The point of contact on both vehicles is essential to determine who hit whom. Failed to Obey Traffic Signal or Sign The driver failed to obey any traffic sign or signal; local, state, or federal traffic regulation; or the directions of a traffic officer. Not Watching Road The driver did not pay full and proper attention to the road ahead, vehicles along side, bisecting streets, driveways, etc. These accidents usually occur when the attention of the driver is momentarily distracted; such as, looking for street address, making notes, reading maps (directions) or operating equipment within the vehicle. Improper Turning These accidents occur from the time a turn is started until it is completed, regardless of speed. An illegally parked vehicle does not relieve the driver of responsibility. It includes making the turn from the wrong lane, improper or non-use of directional signals, or failure to properly crowd the corner prior to turning. Improper Backing 533575286 11 Revised: 03/20/2013 Driver did not pay full and proper attention to area and vehicles behind and/or along side of vehicle while operating vehicle in reverse gear. VEHICLE INSPECTIONS Maintaining and operating your fleet vehicle in a safe manner with a positive image and appearance for the benefit of our customers is an important job performance criterion. As part of the OCR process, at least once annually you and your manager should review your vehicle maintenance history, vehicle condition and appearance, and prepare a vehicle condition report. This will provide you with the opportunity to confirm that you are following the appropriate maintenance and appearance guidelines and have any needed corrective action documented and initiated. A sample condition report follows this section and is incorporated in the policy.
