RESIDENT SIGN-IN SHEETS

It is mandatory for each resident and Fellow at the VANO is to sign in and out daily. The sign-in sheet for Ophthalmology is located in Dr. Carriere’s office (9G123). The sign-in sheets are the official records of time spent on service. These sheets are submitted to the head of Medical Education, Dr. Pappas, and each month. The respective medical schools are then paid by the VA based on the time worked by each resident.

VACATION / EDUCATIONAL LEAVE REQUESTS

All Leave Requests must be officially approved by Drs. Metzinger and Estrada at least 6 weeks prior to a rotation. Without official approval the resident in question will still be responsible for his/her scheduled clinic and will be considered AWOL.

Each resident slot accrues annual leave time based on time worked; therefore only a limited amount of vacation time is available. If excess vacation is taken or leave is not officially approved, the medical schools will not be paid for this time.

For Educational leave, residents must submit the official meeting pamphlet as well as an official copy of travel itinerary.

NON OR Procedures at the VA require:

All residents help each other regardless of school affliation—THIS IS ONE EYE

CLINIC—A JOINT SERVICE

No one leaves until ALL the eye clinic patients are seen.

Ask other providers if they are finished—if not—help them finish

Ask in the hall and in the waiting room if there are any patients waiting for

Ophthalmology—if so—See them

Never leave drops unattended in the clinic.

They must be on your person or in a locked drawer—OTHERWISE THIS IS A JOINT

At the end of the day, shred all lists with patient information—PT INFORMATION

NEEDS TO BE PROTECTED AT ALL TIMES FROM POTENTIAL IDENTITY

Va orientation

Prior to your rotation at the VA, all residents must complete a Graduate Medical

Education Application in order to get a VA provider number.

After this initial application, Surgery does its own orientation packet which allows the residents to get their computer codes.
