Abstract Submission Form for Presentation at the
2015 Annual Emergency Preparedness Conference
The Joint Commission and Joint Commission Resources (JCR) are seeking proposals for practitionerbased, change focused processes and practices for the 2015 Emergency Preparedness Conference, April
30-May 1, 2015, at the Hilton Hotel Walt Disney World in Lake Buena Vista, FL.
The focus of the session(s) should include new processes, technologies, techniques and/or tools,
creativity, and innovation used within any health care setting. The presentation should demonstrate a
successful approach to at least one of the four phases of emergency preparedness:
1. mitigation,
2. preparation,
3. response, and
4. recovery
It is required that the relevant Joint Commission accreditation standard(s) be identified/included.
All faculty/speakers presenting at this continuing education activity are required to disclose all relevant
financial relationships with any entity with a commercial interest (e.g. biomedical device manufacturers
and/or corporations whose products or services are related to pertinent therapeutic areas, and
pharmaceutical companies). All information disclosed must be shared with the audience.
Upon completion and submission of this abstract proposal you are agreeing to these Terms and
Conditions upon notification of your proposals acceptance for presentation:






JCR will provide a complimentary full conference registration for one (1) speaker. This does
not include registration for any pre or post conferences.
JCR will provide one complimentary hotel night, prior to the day of presentation, at the
designated conference hotel.
JCR will provide for a $500 voucher for purchase of JCR publications (excludes AMP product)
JCR does not reimburse for any other travel expenses, i.e. air fare, meals.
A maximum of one speaker per session will receive reimbursement as listed above. If there
are co-presenters, they are required to register for the conference.
o Co-presenters must also provide the same information as required of the primary
presenter, including completion of faculty documents and agreement to the Terms
and Conditions.
Submissions will not be considered if they appear to directly promote any products or services
of a commercial nature. If the presentation proposal is selected and appears to contain
commercial content, JCR reserves the right to request specific modifications. If modifications
are not made, the presentation proposal will not be included in the conference.

Instructions:
 Length of presentation is limited to a 60 minute time block.
 2 – 5 Tools/Deliverable are required at the time of submission of this proposal; this does not
include your PowerPoint presentation. Your PowerPoint presentation, if one will be used, will
not need to be submitted until two months prior to the conference. You will be notified of the
submission due date upon receipt of the formal faculty documents.

5. The scope – Provide a description of the session with the key components clearly stated,
including the main issue that your organization addressed, data that supported the
improvement/change, outcomes, lessons learned and any other practical information that
potential attendees will be able to implement/use at their respective organizations.

6. Identify which applicable Joint Commission accreditation standard(s) or National Patient
Safety Goal(s) is/are addressed by this initiative. This is a required element.

10. Deliverables/tools – Each proposal must include at least two deliverables/tools/takeaways that
the participants can use post-conference (e.g. checklists, policies, procedures, journal articles
that have permission for reprint, videos, reports). All tools must be submitted in an electronic
format. These tools will be made available on the JCR learning management system, postconference. Abstract submissions without these deliverables will not be considered.
