ICD-10 LifeLine for physicians - Feb. 26 Welcome to the fourth edition of the ICD-10 Lifeline. We are excited to announce that the ICD-10 Lifeline is now more accessible than ever! All issues of the ICD-10 Lifeline (past and present) have been added to the Physician Portal (http://dr.beaumont.edu) along with a ICD-10 Countdown clock to remind us that the migration to ICD-10 is coming soon. Look for the link on the right side of the home page, just underneath the “Quick Links” section. ICD-10 Diagnosis & Documentation Tip of the Week In the last issue of the ICD-10 Lifeline, we emphasized the redundancy built into the new code set. The diagnosis we are covering this week, Congestive Heart Failure, shows that some of this redundancy was present in the ICD-9 code set. Diagnosis: Congestive Heart Failure Key Documentation Requirements: Severity: Acute, Chronic, Acute on Chronic Type: Systolic, Diastolic, Combined systolic and diastolic Etiology, if known (e.g. due to dilated cardiomyopathy) ICD-9 428.0 428.1 428.20 428.21 428.22 428.23 Descriptor Congestive Heart Failure unspecified Left heart failure Systolic heart failure, unspecified Systolic heart failure, acute Systolic heart failure, chronic Systolic heart failure, acute on chronic ICD-10 I50.9 I50.1 I50.20 I50.21 I50.22 I50.23 428.30 428.31 428.32 428.33 428.40 Diastolic heart failure, unspecified Diastolic heart failure, acute Diastolic heart failure, chronic Diastolic heart failure, acute on chronic Combined systolic and diastolic heart failure, unspecified Combined systolic and diastolic heart failure, acute Combined systolic and diastolic heart failure, chronic Combined systolic and diastolic heart failure, acute on chronic I50.30 I50.31 I50.32 I50.33 I50.40 428.41 428.42 428.43 I50.41 I50.42 I50.43 Descriptor Heart failure, unspecified Left ventricular failure Unspecified systolic (congestive) heart failure Acute systolic (congestive) heart failure Chronic systolic (congestive) heart failure Acute on chronic systolic (congestive) heart failure Unspecified diastolic heart failure Acute diastolic (congestive) heart failure Chronic diastolic heart failure Acute on chronic diastolic heart failure Unspecified combined systolic and diastolic (congestive) heart failure Acute combined systolic and diastolic (congestive) heart failure Chronic combined systolic and diastolic (congestive) heart failure Acute on chronic combined systolic and diastolic heart failure 428.9 Heart failure, unspecified I50.9 Heart failure, unspecified When diagnosing CHF, it is key to document the severity, type and etiology of the problem along with the diagnosis. Note: If you have a diagnosis that you would like to see discussed in a future issue, please let us know by taking the survey below! IT Update Information Technology is preparing for Stage 2 conversion of oneChart, also known as Clinical Go Live, which allows users to get familiar with ICD-10 codes and workflows. To assist clinicians, oneChart will offer a new feature called Diagnosis Calculator. The Diagnosis Calculator will appear during clinical workflow when adding a diagnosis and help guide the clinicians with options to select from a Generic term to a Specific term. For example, consider a patient with the problem “Broken Nose,” which is a generic term. The Diagnosis Calculator prompts the clinician for the required details, such as fracture type (closed fracture or open fracture). Stay tuned for more information and education tools. Training & Education Update We are listening to your feedback! Based off of suggestions gathered from surveys and our physician champion group, we are looking at many different ways for getting ICD-10 material out to Beaumont physicians, including some options that are worth CME credit. We already are working on presenting ICD-10 material at department Grand Rounds and staff meetings. Please check with your meeting coordinators to see if your department has one of these presentations scheduled. We’re also considering several other training and education opportunities and will keep you informed as plans are finalized: - Implementing computer-based training material for physicians to review. - Broadcasting ICD-10 training material on TVs inside of physician break rooms. - Printing pocket-sized tip sheets detailing increased documentation requirements. In the meantime, if you have a suggestion for another way we could get this information out to out physicians, please let us know using this month’s survey! Give us your feedback through our short online survey We hope you found this edition of the Lifeline to be useful! Please take just a couple of minutes to answer our four-question survey here: https://www.surveymonkey.com/s/SQ3XYRX Thank you for reading! Look for the next issue to come out on Mar. 12.
