A PPLICATIONS & R EQUIRED M ATERIAL S
Applications for admission are available for Fall 2015 prospective students and are usually available online. Go to your prospective college’s website. Locate admissions deadlines/policies for your school. Apply as early as possible. If you plan on applying to private LA schools or out-of-state colleges/universities, please see Dr.
Fru gé in Guidance PRIOR to applying. There are different requirements for these colleges/universities.
Check application, housing, scholarship, and financial aid deadlines. Go to the school’s website for deadlines. Pay careful attention to the types of admission policies for private or out of state schools.
L ETTERS OF R ECOMMENDATION
If a letter of recommendation or secondary school report (SSR) is required, you must give your teacher and/or counselor at least three weeks notice for either one. Be sure to check deadlines! If a letter is required, obtain a Request for a Letter of Recommendation Form from Guidance or obtain from College Counseling page on STM website. Complete form, submit to teacher/counselor.
Teacher will usually upload your letter online. This form is a physical reminder. You do not need to obtain the letters from the teachers.
Public colleges/universities in LA do not require letters of recommendation unless college or university requests.
T RANSCRIPTS
Review your transcript and TOPS transcript for accuracy (mailed home).
LA public institutions are on a state system, and you do NOT need to request a transcript from the registrar. Transcripts for non-public colleges and out-of-state colleges must be requested. Come to
Guidance for a Transcript Request Form or obtain on STM website. Give at least five days notice.
Complete and return the form.
Colleges want OFFICIAL transcripts sent directly from the high school unless otherwise specified.
Students do not mail or email these.
ACT/SAT
Take the ACT and/or SAT and if required the SAT subject tests. Go to colleges’ admission pages on their website to determine what is needed. Send ACT/SAT scores to all colleges to which you are applying.
Send ACT/SAT scores to TOPS (LA Tuition Opportunity Program for Students), ACT code (1595),
SAT code (9019).
Send to NCAA Eligibility Center if interested in college athletics (code 9999).
Take the ACT with writing section at least one time . LSU requires this for Honors College and some scholarships. Some out of state colleges require it. Send scores to colleges of your choice.
Colleges/Universities want OFFICIAL scores. OFFICIAL scores are sent directly from the testing agency NOT from STM.

If you are interested in being invited to UL Lafayette’s or LSU’s early orientations, go to their websites for the ACT & GPA requirements.
C OLLEGE V ISITS
Visit college campuses (two days excused absences are allowed but still counts as an absence); obtain College Visit Form from Guidance, complete form, submit to front office PRIOR to your scheduled visit. Parent and teacher signatures are required.
TOPS & S CHOLARSHIPS
To apply for TOPS, first apply for a PIN for the Free Application for Federal Student Aid
(FAFSA). Both you and a parent apply for a PIN at www.pin.ed.gov
. The PIN serves as your esignature for on-line FAFSA which is required for TOPS and any federal aid (more instructions below). Do NOT lose this number.
Most colleges/universities have priority deadlines for scholarships. Know what the deadlines are for your prospective colleges/universities.
Most deadlines are mid-fall to early winter. Contact the college/university or go to their web-site.
Pay attention to emails from Guidance regarding both college and outside sources for scholarships.

Your application must be on file by this date. Your application for admission to LSU serves as your application for scholarships.

Last ACT UL Lafayette will use in evaluation for scholarships.

Suggested date for student completion of all college applications unless due earlier. Many deadlines are well in advance of this date.
FAFSA available on-line. Complete 2015-2016 form online at www.fafsa.ed.gov
. This form must be completed for TOPS and for all federal financial aid including grants or loans. Some colleges also require the College Scholarship Service (CSS profile

).
Each college has a different priority filing date for the FAFSA and/or CSS Profile for financial aid. Check their website.

UL L AFAYETTE
Regular academic scholarship deadline for UL Lafayette.
Must have applied for admission and meet certain academic requirements. ACT must be taken by this deadline.
Separate application for scholarships required. Obtain in Guidance or on-line.
Submit to Guidance Counselor for completion of information.
A PRIL 18, 2015
Last date to take the ACT to be eligible for TOPS for eight semesters of award.

Candidate Reply Deadline (must pay deposit at college of choice).

Last date to take the ACT to initially qualify for TOPS, not increase level of TOPS award. If you qualify on this test date, you will be eligible for 7 semesters instead of 8.
