Person-Centered
Planning
Person-centered planning is
a way to assist people with
disabilities and those coping
with the challenges of aging to
articulate and accomplish what
they hope for in their lives, or
at least move steadily in that
direction.
Person-centered planning
is about not going it alone, but
calling on others to assist, to
brainstorm solutions to stumbling
blocks, to be aware of and
connect with available resources
to take action toward one’s goals.
Healthy Living:
Challenge &
Opportunity
Making lifestyle changes is hard
to do even when we know they
are good for us, but up-to-date
information, the knowledge
of proven strategies and the
support of peers who are facing
the same challenges can help.
Our evidence-based Healthy
Living programs are offered in
partnership with primary care
physicians, Councils on Aging
and other community partners
and help those coping with pain,
diabetes, arthritis and other
chronic conditions.

for Nearly 40 Years
Your Life, Your Care, Your Way!
For nearly 40 years Minuteman Senior
Services has been committed to helping
people keep control of their lives as they
age by offering information, advice, support and services that enable independent
living in home and community. Since
many seniors, disabled adults and family
caregivers are unfamiliar with these resources, they turn to Minuteman for our
expertise, our experience and our longstanding and effective working relationships with other service providers to find
the solution that’s best for them.
This is true not only for families, but
for medical partners as well, who now
recognize how the home environment
impacts healthy living, especially in the
area of medication management. Here are
some of our key accomplishments over
the past year:
• Our Information and Referral Specialists received the highest number of inquiries in our 40 year history, as individuals, family members and providers
look for available and affordable resources. Unfortunately reports of elder
abuse and neglect and self neglect also
continue to rise.
• Other community supports like Meals
on Wheels and requests for health benefits counseling from our knowledgeable SHINE Counselors also showed
high demand.
• For some seniors and adults with disabilities a skilled nursing facility is
their home either on short or long term
basis. This year, our role as Transition
Coordinators under the Money Follows
the Person national demonstration project will be enhanced as resources expand to empower qualified long term
nursing facility residents to relocate
to a community setting. And, as they
have for more than 3 decades, our Long
Term Care Ombudsmen work tirelessly
to protect the rights of residents of our
15 area nursing homes and rest homes.
• Our participation in two national outreach
campaigns−March for Meals and the
Year of Elder Abuse Protection−helped
shine a light on senior hunger in our
communities and provided tips on how
people can help prevent elder abuse
and neglect.
• Our home care programs continue to
grow and last year over 1700 seniors
were able to continue living in the comfort of their own homes while receiving
a wide range of in home and community services from our time tested and
highly rated vendor network. This year
we will expand our role in a national
demonstration project that assists nursing facility residents to relocate to a
community setting as appropriate.

An Integrated Care Team: The Key to Health &
Independent Living For Our Consumers
• Our emerging role in health care reform efforts expanded this year with
an exciting pilot program with the
Lahey Health/Burlington General
Internal Medicine practice. New
models of care here and across the
nation show that pairing medical
services with community supports
produces the best outcomes for patients and payers alike.
• Ten years ago Massachusetts pioneered an all inclusive model of
care for seniors called Senior Care
Options (SCO). Minuteman has
been a member of the care team
providing Geriatric Support Services Coordination for members of
all plans that serve our area and we
have seen significant growth.
How You Can Help
• This year Massachusetts will again
pioneer an extension of this comprehensive model of service to adults age
21-64 on Medicare and MassHealth
through the One Care Plan. We look
forward to our expanded partnerships with Commonwealth Care Alliance, Network Health and Fallon
Total Care as they launch the new
One Care Plan in our service area.
As a member of the integrated care
team, Minuteman’s Long Term Services (LTS) Coordinator will arrange for services such as personal
care, transportation, in- home services and more. We are proud to
be members of the One Care team
under the name Minuteman Community Living and to extend our
expertise and assistance to disabled
adults of all ages.
We welcome your donation in any amount, and we always need volunteers to
deliver Meals on Wheels and assist in other programs. Our services are funded
in part by the Massachusetts Executive Office of Elder Affairs, with support
from local United Way and Community Chest agencies and many foundations,
corporations, and individuals.

Our keys to success are our employees, volunteers and subcontracted provider
network working together to bring person-centered care to all who turn to us for
help. We are also grateful for the generosity of our donors, large and small, that
make it possible for us to carry out our mission: To help seniors and people
with disabilities live in the setting of their choice by engaging
community resources and supporting caregivers.

difference in the lives of those we serve. Programs are funded in part through contracts with
the Massachusetts Executive Office of Elder Affairs and contributions from local city and
town governments. Since public funding has not kept pace with the growing needs of
seniors and people with disabilities, we actively solicit private financial support. In addition to
