Abnormalities in these tests should direct one to the biliary tract and an early ERCP, particularly if cholangitis is in the differential diagnosis. Rockey 2012 Liver Abscess • Distinguishing pyogenic liver abscesses from amebic liver abscesses may be very difficult on clinical grounds as the presentation is very similar. • The best clinical clue to differentiate between PLA and ALA is the epidemiologic setting. A history of diarrhea is usually not helpful, but recent travel to endemic areas of amebiasis is often present in patients with ALA. • Amebic antibodies are very sensitive in the setting of ALA. • Lesion aspiration may also be very helpful.
