Procedure for Enrolling in Health at SMC or BYU
Incoming 9th Grade Students, Fall 2012
1. Application – download the application from one of the following:
SMC – www.smc.edu. The course title is Health 10.
BYU – www.palihigh.org Counseling shortcut to open the “Application for Online &
courses taken at other schools” document. The course title is HLTH 41.
2. Complete the application.
3. Mail or bring the application to the Counseling Office at Palisades – ATTEN: Ms. Theard
Attach a stamped self-addressed envelope to the application.
4. Ms. Theard will sign the application.
SMC – the original application will be mailed back to you. Take the application
to SMC in order to enroll in the class. You will need your student’s last report
card from middle school to submit with the application.
BYU – a copy of the application will be mailed back to you. This is for your
records only. The student may enroll in the BYU course any time after June 20th*.
The student enrolls in the course by logging on to www.byu.edu and follow the
procedure for enrolling in an online course. Palisades does not enroll you in the
BYU online course. This is your responsibility.
Deadline date to enroll in this class is July 7, 2011.
5. Transcript for summer work
It is the responsibility of the student/parent to request that a sealed official transcript from
SMC, WLA, or BYU be sent to Palisades by September 13, 2012. 2Students will not
receive credit for the course if a transcript is not received by September 13, 2012.
* Student will not receive credit for this course if the student enrolls before June 20th.
