IT IS CRITICAL THAT YOU RETURN THIS FORM! Please remember that when
you receive the monthly schedule and you know you cannot be at a Mass you are
scheduled for, it is your obligation to call Holy Name of Jesus Convent in enough time so
that other altar servers can be called! The telephone number to call is 323-4546. This is
especially critical during the summer months. Your cooperation in this matter would be
