COMPASS Math Placement Exam Transfer Form
A student who has completed COMPASS math placement exam for another college or
university may transfer his/her scores to meet required placement testing prerequisites
for courses at Illinois State University. The student must follow all directions below for
scores to be accepted.
Only official COMPASS math placements tests will be accepted. Test results are not
official unless sent directly from the institution where the student tested and are in
official COMPASS result format. (The results should not be a copy of student’s records)
COMPASS math placement exams may be configured differently for each institution
based on course placement requirements. There is no guarantee that another
institution’s COMPASS math placement results will be accepted for placement at Illinois
State University. Please contact the University College Testing Office if you have any
questions.
1. Complete the information below and send this form back to the Illinois State
University College Testing Services office. The Testing Office will not enter scores
on a student’s record until this form has been completed.

2. Contact the institution where you completed the placement testing and request
that an official copy of your test results be sent directly to the University College
