RELEASE OF LIABILITY AND NOTICE OF INHERENT RISKS
Greenbriar Veterinary Hospital and Luxury Pet Resort shall exercise reasonable care to maintain a clean, safe and secure environment for
the dog while in Greenbriar’s care. The dog shall be fed properly and regularly; shall be housed in its own quarters; shall be cleaned and
groomed as needed; and shall not be allowed off of Greenbriar’s premises without the consent of the owner.
If the dog shows evidence of a medical problem while under the care and control of Greenbriar, Greenbriar shall have the right to take such
measures as it deems necessary, without be liable to owner, for the health and safety of the dog, including the right to administer
medication or obtain an evaluation and treatment by our veterinarians at Greenbriar. If, in the opinion of Greenbriar and doctors, the cost of
treatment will exceed the amount of seventy-five dollars ($75.00), reasonable attempts will be made to contact the owner, and/or the
veterinarian designated by the owner, above, for further instructions. In the event that the owner or designated veterinarian cannot be
contacted, Greenbriar, without liability to owner, provide such emergency medical treatment as is deemed necessary to preserve the life and
health of the dog. Owner shall pay the cost of all treatment provided for in this paragraph, in addition to the contract price, prior to the dog
being picked up by the owner.
Greenbriar agrees that, while the dog is on its premises and under its control for boarding and training, Greenbriar will exercise the utmost
diligence and care to protect the health and safety of the dog. However, owner agrees that Greenbriar has no liability for loss of or damage
to the dog or owner’s other property resulting from fire, disease, illness, virus, escape, theft, death or other avoidable causes. After the dog
has been returned to the owner by Greenbriar, and is off of Greenbriar’s premises, Greenbriar cannot and does not accept any responsibility
for the illness, injury or death of the dog or of any person or animal, or damage to any property, caused by the dog.
Owner represents that owner is the legal owner of the dog; that the dog is free and clear of any mortgage or other encumbrance; that the
dog has not been exposed to distemper, rabies, parvo or any other disease with the past thirty (30) days; that the dog has been inoculated as
shown above; that the dog is currently and properly license; and that owner has read this agreement in its entirety.
Owner understands that if the dog becomes aggressive during its training and is unable to be cared for by the kennel staff under normal
protocols, the dog will be upgraded to a Penthouse suite and additional charges will be added to the final bill at pick up. In the event this
happens we will notify you by phone/email.
If, at the completion of the agreed boarding or training time, the dog is not picked up by the owner and payment of all charges made in full,
including applicable medical, veterinarian or boarding charges as provided herein, Greenbriar shall notify owner by certified mail at the
address shown above that the dog will be held for an additional ten (10) days following the date of mailing the notice. At the end of the tenday period, if there is no response from the owner, the dog may be sold at public or private sale. The proceeds of such sale shall be first
applied to the charges of Greenbriar and the balance, if any, shall be returned to owner at the address shown above. Owner shall be charged
for any such additional days following notice at Greenbriar then existing boarding rate.
To pay for reasonable attorney’s fees incurred by GREENBRIAR VETERINARY HOSPITAL & LUXURY PET RESORT in the
collection of any boarding, training, grooming, or other charges incurred by me or my agent.
