WEATHERTIGHT SERVICES FAP FACT SHEET Final payment claim - April 2014– Page 1 Leaky Homes Financial Assistance Package (FAP) Final payment claim fact sheet Introduction This fact sheet provides guidance on making a claim for a final contribution payment at the completion of your repair. Send the completed form to the Ministry along with all the documentation listed in the form, i.e.: When to make a claim ›› copies of all invoices related to the approved repair work A final payment claim is made when all repair work is complete, your repair has been issued with a code compliance certificate, and you have finalised the amount of all invoices for the project. It enables the Ministry of Business Innovation & Employment (the Ministry) to confirm the actual cost of your repair based on your invoices, taking into account any variations that arose during the course of the repair How to make a Final Payment Claim Complete the final payment claim and statements form, which is an excel spreadsheet. The form requires you to enter all invoices for your project – including invoices for approved repairs, ‘other repairs’, betterment items, variations work, and associated costs. Where your project included ‘other repairs’ and betterment work, you will need to specify what percentage of your associated costs related to approved repairs. The spreadsheet will then calculate the costs which you are claiming for contributions. ›› copy of code compliance certificate ›› construction contract invoices ›› details of expenditure on monetary allowances and variations ›› costs outside construction (i.e. associated costs) ›› timber remediation statement. You may wish to wait until you have made your final payment to your builder before submitting the claim. To have a claim accepted under the Weathertight Homes Resolution Services Act 2006 and to qualify for the Weathertightness Financial Assistance Package homeowners must meet certain criteria and conditions. MB12572.1.31 WEATHERTIGHT SERVICES FAP FACT SHEET Final payment claim - April 2014– Page 2 Ministry’s review of invoices On receipt of a claim for a final contribution payment, the Ministry will review all your invoices, and ensure that the amount claimed for each of the following can be supported: ›› expenditure on approved repairs in accordance with the approved repair plan, reviewed against the amounts in the approved payment plan ›› expenditure on monetary allowances and variations, in order to adjust contributions levels in accordance with these changes, reviewed against the information in your variation orders, timber remediation statement, and the rates in the approved payment plan The Ministry and council will not contribute to the following: ›› amounts in excess of those in the approved payment plan, unless they are supported by clear evidence that a variation was necessary to the scope of approved repairs (amounts that were approved through formal change requests will already meet this requirement) ›› any costs relating to betterment or ‘other repairs’ ›› expenditure on associated costs, reviewed against the amounts in the approved payment plan ›› Your council, if contributing, will also need to review and approve any increase in total payments above the approved repair and associated costs in the approved payment plan. The Ministry manages this process. ›› The Ministry will then confirm the total amount of Crown and council (if appropriate) contributions to be paid.
