IVA-AE2 Diagnostic Report - Extended Test Name: Doe, Jane Age: 21 Sex: F Report Date: 8/25/2015 Test Date: 9/16/2004 09:22 AM On Meds: N The use of this Diagnostic Report requires that tests be administered in accordance with the specified test guidelines under the supervision of a licensed health care professional who is qualified to diagnose ADHD. The test by itself is not to be used as a stand-alone instrument in making any diagnosis. Its primary purpose is to provide examiners with a working diagnosis to consider as part of their comprehensive evaluation of clients who present with ADHD-type symptoms. This diagnostic report will only be generated after clinicians answer all of the relevant questions and then determine, using their professional judgment, a client's clinical diagnosis. This report is only to be distributed to others after it has been carefully reviewed, modified as needed, and signed by the examiner. She is a 21 year old female who presents with the primary symptoms of disorganized thinking, impulsive behavior, and poor academic performance. Symptoms were noted as occurring in the school and home settings. I have determined that these symptoms clearly impair her functioning. In addition, this individual has been identified as having anxiety. Significant inattentive symptoms were observed during the clinical intake, but no significant ADHD-type symptoms were observed during the IVA-AE2 test administration. This individual's ADHD symptoms are not due to an inability to understand instructions. They are also not due solely to hostile or oppositional behavior. This individual has not previously been diagnosed as having ADHD. Her symptoms have persisted for a sufficient period of time to support a diagnosis of ADHD. ADHD symptoms were identified to be present when this individual was a child. There is no family history of ADHD symptoms. This individual was not taking any medication at the time of testing that was likely to affect her test results. The test results were fully valid, and this individual demonstrated sufficient motivation in completing this test. Her IVA-AE2 test scores supported the possible working diagnoses of ADHD, Inattentive presentation, Mild Neurocognitive Disorder, and Other Mental Diagnosis. Based on my clinical judgment and overall evaluation, this individual's diagnosis is ADHD, Inattentive presentation, Moderate. In order to help her at this time, based on her diagnosis, I am making the following treatment recommendations: cognitive training and neurofeedback. I am recommending that the following reasonable accommodations be made to help this individual in academic settings: allow the student to record classes and arrange more frequent meetings with the instructors. I have reviewed the test scores in this interpretative report and have modified them as necessary in accordance with my comprehensive evaluation, the client's history and other relevant clinical data.
