Frederick Koralewski was operating a 2011 Chevy Silverado (OH-PGV8467) on southbound IR-475 in the right hand lane
leading to southbound IR-75. Charles Stafford was operating a 2012 Ford F350 (MI-CE80625) also on southbound
IR-475 in the right hand lane behind Koralewski. According to both, a semi truck and trailer merged from the junction to
northbound IR-75 into the junction for southbound IR-75 in front of Koralewski, causing Koralewski to brake and Stafford
to strike the back of Koralewski's vehicle. Stafford was cited for ACDA, 4511.21(A) MM. Koralewski was able to drive
his vehicle from the scene, and Stafford's employer will remove his vehicle from the side of the roadway were it was
parked and secured.

Call Short Narrative
While traveling northbound on SR-25 crossing over IR-475, I observed a vehicle on northbound IR-475 in the left hand
lane. The vehicle was passing another vehicle in the right hand lane at what appeared to be a very fast pace. I got on
to northbound IR-475 and attempted to catch up with the vehicle. I caught the vehicle at mile post 2.5 and observed a
white Mitsubishi Lancer followed by a grey truck, both in the left hand lane. I activated radar unit E27139 in moving
mode, and I observed that my vehicle's speed was 80 MPH, and it corresponded with the speedometer. I activated my
over head lights and the white Mitsubishi (NC-ZVH3678) stopped and the grey truck continued on. I identified the driver
as Lalona Young. Young advised that she was just driving home to Tecumseh , MI from Columbus, OH. I issued Young
a citation for Speed, 4511.21(B)(13) MM, and gave her a court date of 12/21/12 at 0845 hours.The calibration of radar
unit E27139 was checked and found to be accurate at 0520 hours on 12/19/12.

I was stationary at 235 W. Indiana Ave. I observed a white Saturn Aura OH-FRF6378 traveling Westbound on W.
Indiana between Walnut St and Pine St. The vehicle appeared to be traveling faster than the posted speed limit of 25
mph. I activated laser unit #UX014977 and it displayed a reading of 48 mph at 732.8 ft from cruiser. There were no
vehicles in front or behind the vehicle. I stopped the vehicle and I spoke to the driver, Amanda Smith. Miss Smith
advised that she thought the speed limit was 45 mph, but she didn't know how fast she was going. She told me that
she had to be home at 2100 hrs, but she had to take her brothers to different events since her mother was unable to do
so. Miss Smith was cited for speed and was given a court date for 01/16/2013 at 1645 hrs. A calibration check was
performed on the laser unit and it was functioning properly.

Alexia Byrne was traveling eastbound on U.S. 20 in the right lane. Linda Stewart, Robert Frankforther, and Shannon
Irmen were all stopped for traffic ahead of Byrne. Byrne was unable to stop her vehicle and struck the rear of Stewart's
vehicle. The impact forced Stewart's vehicle into the rear of Frankforther's vehicle. Frankforther's vehicle was then

Unit #1 (Karen Grant) and Unit #2 (Justin Calkins) were stopped on IR-475 S off ramp to SR 25. Unit #1's vehicle stalled
and rolled back and struck Unit #2's front bumper. Unit #1 and Unit #2's damage was under the state reportable dollare
amount. Mr. Calkins' wife was two cars back when the crash occurred and saw that Mrs. Grant did not stop when the
crash occurred. Mrs. Calkins drove after Mrs. Calkins and confronted Mrs. Grant at her residence about the crash. No

While conducting traffic enforcement on Findlay St, a white Ford Fusion was observed traveling south on Findlay St
at what appeared to be an excessive speed. A check with radar in stationary mode indicated the Fusion was traveling
at a speed of 48 MPH on the 25 MPH roadway while it was passing Garfield Dr. A traffic stop was initiated and David
Good was identified as the driver.
Mr. Good stated he was not from the area and had no knowledge of what the
speed limit was for Findlay St. He was issued a citation for Speed Above Posted Limits (48/25) 4511.21B2 ORC (MM)
and given a court date of December 21, 2012 at 8:45 AM.Radar: PYT831001931Calibration check: conducted at 4:00

Ruth Schafer stopped on station to report two suspicious people at Fat Jack's. Ruth did not give any
description or elaborate on why they were suspicious. Upon arrival at Fat Jack's I did not note anything
out of the ordinary. I did speak with the bartender who advised that she has not had problems with
anyone.
