Kareena Private Hospital is currently undergoing an exciting new redevelopment to the hospital to improve patient services and expand the existing building. The new development at Kareena Hospital, costing over $20 million will include 6 new consulting suites, an additional Operating Theatre and the inclusion of the single rooms across existing wards including the Maternity unit. Kareena Private Hospital will become a modern 150 bed facility offering patients the highest possible standard of care. The development will allow Kareena Hospital to add to its current services Neuro spinal surgery, Ophthalmic surgery and bariatric surgery most of which until now Sutherland Shire residents have had to travel outside the Shire to avail themselves of these services in a private hospital setting. The street presence of Kareena Hospital will change as a new building that faces the Kingsway will be constructed above the existing car park. The Surgical ward which currently has 18 beds will increase to 36 beds. A second Operating Theatre will be partially constructed (cold shell stage) for future growth, and the Maternity Unit will receive an additional Special Care Nursery cot with 18 private postnatal rooms. Also included in the developments are expansions for both the Pharmacy and Stores Departments, and relocation of the ICU Unit and Medical Records Department. When you are unable to make contact with a Kareena Specialist to arrange the acute admission of your patient then the Ezi Access pathway will assist to facilitate the direct admission of medical patients. It is required that you have assessed the patients and they have a diagnosis which requires admission. Direct admissions organised via a Kareena Specialist by yourself will remain unchanged. In order to initiate an Ezi Access pathway, all you have to do is phone the Bed Manager on 9717 0182 who will obtain patient details and transfer you to the Senior Medical Officer who will ask you a series of questions about the patient. They will then liaise with a Kareena Specialist to accept the care of your patient. Once this confirmation is accepted, from the Kareena Specialist and a bed is available, the Bed Manager will liaise with you regarding admission protocols for patients. If your patient is acutely unwell or unstable we recommend that you call 000 for an ambulance. This is essential in the case of chest pains or cardiac related issues. Kareena Private Hospital will continue to have experienced medical support on site 24 hours a day. In addition we have recently engaged the services of Intensive Care Specialists to support our Level 1 Intensive Care Unit The Ezi Access Process Phone Ezi Access 02 9717 0182 Transferred to Bed Manager and Senior Medical Officer (SMO) In order to expedite the screening process thus ensuring your patient is assessed as quickly as possible, our risk assessment check will require you to answer a few questions. The more information you or your patient can provide, the faster we can arrange admission. • • Detailed history of the patient including; – Medications – Past medical history – Consultant determine – Infection status – DVA details – DOB Private health insurance details. We can check the level of cover and advise the patient of any out-of-pocket expenses. Which Patients are Suitable for Ezi Access? The Ezi-Access pathway is suitable for patients requiring hospitalisation for: • • • • • • Management of diabetes; Anaemia for investigation; Mild heart failure; Mild renal impairment (GFR>50); Cellulitis; Pneumonia. Medical matters The current management of gallstones by Dr Francis Chu Suite 106, 26-28 Gibbs Street Miranda, NSW, 2228 Phone : 02 9524 7778 Fax : 02 9540 3385 Surgical Updates In a new three part lecture series Dr Francis Chu will update the current surgical management of gallstones, abdominal wall hernias and liver cancer. The current management of gallstones: Gallstones are a common surgical condition. It has been stated that up to 20% of women and 10% of men have gallstones. The current risk factors for developing gallstones include: •Female •Increasing age •Pregnancy/oral contraceptive pill •Family history •Obesity •Diabetes •Increased serum cholesterol The majority of gallstones seen in Australia are cholesterol stones, which account for approximately 80% of the population. However 20% have pigmented stones which are usually smaller in size but more in number. Asymptomatic stones: Although the prevalence of gallstones is high in our society, the majority of these patients are asymptomatic and do not require any surgical management or intervention. However there should be a few caveats to this. Firstly, in those patients with multiple small gallstones (especially pigmented), they have increased risk of passing the stones into the common bile duct and hence presenting with choledocholithiasis. Secondly, females of childbearing age diagnosed with gallstones should consider elective surgery prior to falling pregnant. Another high risk group are those patients with large stones greater than 2cm for more than 20 years duration as these patients are at increased risk of developing gallbladder cancer. Symptoms: We all know the classical symptoms of gallstones could be from dull nausea after eating, to biliary colic as well as cholecystitis. An interesting point to note is that of those patients presenting with symptomatic gallstones approximately 10% will also have stones in their common bile duct. Of these, only 50% actually present with abnormal liver function tests. Surgical management: In the mid 90’s there was a movement away from open cholecystectomy to one of laparoscopic cholecystectomy. This is now the standard of care. More than 95% of patients with symptomatic gallstones can be treated via laparoscopic approach. However there will be up to 5% of patients where the surgery is either inappropriate or too difficult and has to be converted to open surgery. The current change in surgical management is the treatment of common bile duct stones. Traditionally an ERCP (endoscopic retrograde cholangiopancreatography) is used to manage common bile duct stones. However, with the advent of laparoscopic common bile duct exploration via a dormia basket or a direct choledocoscopy, up to 80% of patients with common bile duct stones can be managed in the same setting as the laparoscopic surgery. Only 20% of patients with common bile duct stones fail a laparoscopic approach and will require a post operative ERCP. There are certain surgeons offering single incision laparoscopic surgery (SILS) although this is still in the early stages of development and its efficacy will need to be addressed in the future. Complications: Apart from the general complications of surgery including bleeding, infection etc patients undergoing laparoscopic cholecystectomy are at risk of developing either a bile leak or common bile duct injury. These injuries are rare accounting for only 0.5 to 1% of all laparoscopic cholecystectomies. Once the injury has occurred, the patient should be managed in a dedicated hepatobiliary unit as the management of these complications can be complex and may require combination of either ERCP, biliary drainage or a formal operation which involves excision of the common bile duct. Summary: In summary, the management of gallstones has come a long way in the last 20 years. The key updates to note in the last five years is that the majority of patients even with common bile duct stones can be managed in one sitting with laparoscopic surgery. He completed his final year of training in Respiratory and Sleep Medicine back at St. George Hospital, and obtained his Fellowship of Royal Australasian College of Physicians in 2012. Currently, Dr Goh is a Research Fellow at St George Hospital Sleep Unit, in conjunction with UNSW and St. George Clinical School. He is also a visiting consultant physician to Wagga Wagga, supporting the health of rural communities. Dr Goh consults in all areas of Respiratory and Sleeping Disorders, with special interests in interventional pulmonology, respiratory failures and sleep disordered-breathing. In fact, his higher degree research is in the area of sleep apnoeas in orthodontic patients.
