The format needs some fixing but I decided not to correct it until your colleagues can see exactly what changes were made by UD legal. I see no content/policy changes to what we have been proposing other than some boiler plating and formatting modifications made by legal. If these changes are acceptable at your end, I would add the name of our UD Deputy Provost, Nancy W. Brickhouse to the authorization section 9 of the draft. Note Nancy is in charge of all UD academic programs reporting directly to the Provost who reports directly to the President. Since this affiliation involves our College of Arts & Sciences we would also have the Dean of this College sign the agreement. We will have to wait to officially announce the agreement until the UD Faculty Senate approves the name change to “Medical/Dental Scholars Program” which should take place next spring making the program officially in place for incoming freshmen and sophomores next fall. Would it be possible to ask that you/Dean Ismail send a brief electronic endorsement that I can use in my documentation to our Faculty Senate. Similar documentation will be sought from our Deputy Provost and the Dean of our College of Arts & Sciences. This support combined with the already received DIDER recommendation and the unanimous endorsement of our Advisory Board should provide sound footing to obtain Senate approval. PCOM-UD PREMEDICAL POST BACCALAUREATE CERTIFICATE PROGRAM AGREEMENT Unanimous endorsement was granted to support this agreement between PCOM and the newly formed “premedical post baccalaureate certificate program”. 6. ST. GEORGE’S UNIVERSITY MOU The Board decided to postpone supporting the proposed UD-SGU medical school affiliation at this time. However, all members felt that the Center should not delay in seeking an affiliation with the SGU Veterinary School which has already received an endorsement from the Dean of CANR. CPHPS was asked to proceed with this later portion of the proposed affiliation/MOU that would support the interest of some animal science majors in CANR. 7. SIDNEY KIMMEL MEDICAL COLLEGE – UD POST BAC AFFILIATION POSSIBILITY Unanimous agreement was given to seek an agreement between our premedical post baccalaureate certificate program (PMPB) and Jefferson. Discussions indicated that Jefferson would likely entertain such an agreement supporting qualified UD PMPB students. 8. CENTER FOR PREMEDICAL/HEALTH PROFESSIONS 3rd YEAR EVALUATION The Advisory Board directed the Chair to work with one of the members of the Board to create criteria for evaluation of Center. The Board would receive the selfappraisal before initiating its own evaluation of the effectiveness of the Center in meeting its mission over the past two to three years. The CPHPS Director asked Board Member Dr. Don Lehman to work with the Center Director to conduct the self-appraisal. 10. FINANCIAL PLANNING SERIES FOR PREMEDICAL AND HEALTH PROFESSION STUDENTS Dr. Brannick distributed and discussed a proposal, “Life-stage Financial Planning for Pre-health Professions” series. Dr. Brannick designed this series in collaboration with Professor Barbara Emery, Program Coordinator, Economic Education, and Center for Economic Education & Entrepreneurs. Ideas discussed include targeted student populations, incorporating the series into study abroad opportunities, and having an online format. 11. TENTATIVE DATE FOR SPRING 2015 ADVISORY BOARD MEETING No specific date for a spring meeting was determined. Kornberg School of Dentistry and The University of Delaware, for its Medical/Dental Scholars Program AFFILIATION AGREEMENT BETWEEN Temple University, for its Maurice H. Kornberg School of Dentistry and The University of Delaware, for its Medical/Dental Scholars Program to create an EARLY CONDITIONAL ACCEPTANCE BACCALAUREATE/D.M.D. PROGRAM This Affiliation Agreement (hereinafter, “Agreement”) is made on the date indicated in Section V. hereof by and between Temple University, a fully accredited university and constituent part of the Commonwealth System of Higher Education located in Philadelphia Pennsylvania, on behalf of its Maurice H. Kornberg School of Dentistry, and the University of Delaware, a tax-exempt, not-for-profit corporation chartered under the laws of the State of Delaware with its principal place of business in Newark, Delaware, on behalf of its Medical Scholars Program. I. RECITALS A. This is a proposal to create a unique pre-dental eight-year academic program jointly sponsored by the University of Delaware and the Maurice H. Kornberg School of Dentistry at Temple University (Baccalaureate/DMD) in Philadelphia. The undergraduate program of study would be modeled after and incorporated with the Medical Scholars Program (hereinafter “the MSP”) of the College of Arts & Sciences at the University of Delaware. B. At the undergraduate level, the program would be managed by the MSP, which would change its name to Medical/Dental Scholars Program (MDSP). It would require identical academic program requirements/parallel tracks for both medicine and dentistry. It would be specifically designed for those high-achieving undergraduate students who are certain that they want to become dentists and who desire early conditional acceptance into a program of study that blends the scientific and cultural requirements of the Dental Profession. This program will be accomplished by providing undergraduate students pursuing dentistry with a balanced education in liberal arts, sciences, and professional studies leading to a Bachelor of Arts Liberal Studies (BALS) Degree. As in the MSP, the curriculum for C. students in the dental track not only includes the traditional courses in the life/physical sciences required in medicine and dentistry but also the opportunity to specialize in one of three areas related with Bioethics, Administration and Public Policy, or Translational Research. Small group discussions, problem-based instruction, and practicum experiences in clinical/institutional settings are special aspects of the program (ATTACHMENT). II. A. B. C. TERMS OF AGREEMENT COLLEGE OF ARTS & SCIENCES (PROGRAM DIRECTOR). The Dean of the College of Arts & Sciences at the University of Delaware will appoint a Program Director for the MDSP who will: 1. Serve as the primary liaison between the University of Delaware and Temple University – Kornberg School of Dentistry and co-coordinate, with Temple, joint program events, assessment, and agreement revisions with the College of Arts & Sciences, coordinate recruiting efforts and admission of students into the Baccalaureate degree – phase of the program; 2. Advise, evaluate academic progress, and maintain communication with program participants while at the University of Delaware; 3. Organize academic and co-curricular programming related to the MDSP at the University of Delaware; 4. Assist undergraduate students with completing Temple applications, registering for DAT’s, and scheduling interviews for rising juniors prior to matriculation at Temple University-Kornberg School of Dentistry. KORNBERG SCHOOL OF DENTISTRY (PROGRAM COORDINATOR). The Dean of the Temple University-Kornberg School of Dentistry will appoint a Program Coordinator who will: 1. Serve as the primary liaison between Temple University-Kornberg School of Dentistry and the University of Delaware and co-coordinate with the University of Delaware, joint program events, assessment, and agreement revisions with members of the Temple University Admissions Office, coordinate marketing efforts and admission of students into the D.M.D.-phase of the program; 2. Assist undergraduate students with completing Temple applications, scheduling interviews, and compliance with academic/admission requirements. EARLY CONDITIONAL ACCEPTANCE (MDSP SOPHOMORE-JUNIOR YEAR). Under the Early Conditional Acceptance Baccalaureate/D.M.D. Program Agreement, qualified University of Delaware students at the completion of their sophomore year will be allowed to enroll in the MDSP Dental Track as conditionally accepted candidates for admission into the Temple University-Kornberg School of Dentistry provided that they meet all the following requirements: 1. Enrolled full-time matriculated students at the University of Delaware; 2. Join the MDSP “Interest Group” as freshmen and actively participate in fall/spring semester sponsored programs of the MDSP; 3. Submit an application for acceptance into the Early Conditional Acceptance Program with Temple University during the spring semester of the sophomore year; 4. Be accepted as Dental Scholars in the MDSP program as rising juniors provided following conditions are met: a. Demonstrate an overall GPA of 3.5+ with junior status/60+ credits b. Attain a science GPA of 3.5+ in the Core-Prerequisite courses c. Obtain six positive letters of recommendation (3 science professors, 2 non-science professors, and 1 dental preceptor) d. Provide a well written personal statement demonstrating commitment and passion for the pursuit of a professional dental career e. Accomplish a minimum of 100 hours shadowing/volunteering under the supervision of dental professionals f. Successfully complete an on-campus Dental School interview at Temple University following Temple’s receipt of MDSP application materials during the fall semester of the Junior year g. Receive a jointly signed letter of Early Conditional Acceptance from the University of Delaware (MDSP) and Temple University (Kornberg School of Dentistry-Admissions) during the fall of the junior year h. Fully commit to the Kornberg School of Dentistry without consideration of all other dental school programs of professional study i. Submit a letter to the University of Delaware MDSP Program Director during the fall semester of the junior year stating their intention to utilize the affiliation to attend Temple Dental D. MDSP GRADUATING SENIORS. During the senior year, MDSP students will be granted acceptance into the Kornberg School of Dentistry (within 1-year of baccalaureate degree completion) provided the following conditions* have been met: 1. Attained a baccalaureate degree (BALS) upon the completion of 8 academic semesters/4 years; 2. Graduated with an overall GPA of 3.5 or higher 3. Attained a science GPA of 3.5 or higher in the required MDSP Core-Prerequisite courses; 4. Achieve an average DAT score of 19 or higher; 5. Maintained a record of academic honesty and free of judicial violations while at the University of Delaware; 6. Actively participated in a significant majority of University of Delaware/Temple University Program co-curricular activities; 7. Met Temple University-Kornberg School of Dentistry program requirements which include: a. Submission of the American Association of Dental Schools Application Service (AADSAS) application designating Temple University as the sole applicant recipient and pay a processing fee; b. Completion of the Dental Admissions Test (DAT) exam, providing both the University of Delaware and Temple University with official score reports and earning a composite score with an average of 19 or higher; c. Comply with all 1st year financial obligations related to attending Temple University – Kornberg School of Dentistry; d. Written notification of final acceptance/compliance with all early conditional acceptance criteria from the Admissions Committee of Temple University – Kornberg School of Dentistry. *Graduating seniors who fail to obtain any of the above conditions will not be granted the automatic conditional acceptance into the Kornberg School of Dentistry. However, they may still earn a BALS degree from the MDSP and will be encouraged to apply to TUKSOD as well as other dental schools through AADSAS. E. INITIATION OF AGREEMENT/ENROLLMENT LIMITATION. The Early Conditional Acceptance Baccalaureate/D.M.D. Program Agreement will become effective for students entering the University of Delaware in the fall of 2014 or later. Upon initiation of this agreement, the Kornberg School Dentistry agrees to provide at least five (5) spots in their entering classes each academic year for qualified students. UNDERGRADUATE – GRADUATE PROFESSIONAL PROGRAM TRANSITION. To facilitate the transition of University of Delaware MDSP graduates to Temple University Kornberg School of Dentistry, in accordance with the foregoing guarantee, the parties of this affiliation agree to the following: F. 1. a. b. c. d. e. f. g. h. i. j. Reviewing applications of, admitting and enrolling students into the MDSP Dentistry track and Early Conditional Acceptance Baccalaureate/D.M.D. Program. Providing a Program Coordinator to oversee University of Delaware program-related activities and to coordinate with Temple University – Kornberg School of Dentistry, as noted above. Maintaining an academic program of study and programming with input from Temple University – Kornberg School of Dentistry. Delivering the prescribed program curriculum, such that enrolled students can complete required coursework in four years/eight semesters. Providing formal and informal Health Professions Advisement and peer mentoring of MDSP dental track participants, which may include, but is not limited to: 1) Specialized academic advising by Honors Program and Science Department faculty; 2) Enrollment in specific courses as a cohort and, once the Program is established, the expectation that upper-division Program students will serve as mentors for underclassmen; 3) Establishment of an on-campus pre-dental student society to provide Program participants with unique academic, career, and social opportunities; 4) Establishment of DAT exam study groups. Awarding a baccalaureate degree to the prospective dental school student following successful completion of all University of Delaware requirements Developing affiliations with local dental professionals/DIDER to obtain internship and research opportunities for program participants. Updating Temple University – Kornberg School of Dentistry’s Admissions Office with names and profiles of students intending to apply and interview at Temple. Developing and implementing a program assessment plan. 2. Obligations of Temple University – Kornberg School of Dentistry a. Advertising the Early Conditional Acceptance Baccalaureate/D.M.D. Program in its promotional literature and on its website. b. Providing opportunities for University of Delaware program participants to (1) tour Temple University – Kornberg School of Dentistry facilities and (2) shadow dental professionals and students in academic and clinical settings. c. Providing formal and informal mentoring of University of Delaware Program participants by current dental students. d. Interview and select rising junior candidates recommended by the MDSP at the University of Delaware who meet conditional requirements as rising juniors addressing standards required for admission into Temple University – Kornberg School of Dentistry; e. Awarding the D.M.D. degree to the prospective student following successful completion of all curricular and graduation requirements of Temple University – Kornberg School of Dentistry. 3. Joint Obligations: a. To consult with each other through appropriate channels prior to implementing major changes in policy or curricula that directly affect students participating under the terms of this Agreement and keep each other informed of any other changes of policy or curricula that affect those students. Both University of Delaware and Temple University – Kornberg School of Dentistry will review this Agreement annually and make any changes upon mutual agreement, as needed. Such changes will become effective when both University of Delaware and Temple University – Kornberg School of Dentistry, sign the revised document. b. To agree to jointly develop, review, and implement advertising and promotional efforts to communicate the Early Conditional Acceptance Baccalaureate/D.M.D. Program to potential applicants and the general community. c. To designate a representative or representatives (Program Directors) who will coordinate the Early Conditional Acceptance Baccalaureate/D.M.D. Program between the two institutions. d. To exchange data and documents annually that will contribute to the maintenance and improvement of the Early Conditional Acceptance Baccalaureate/D.M.D. Program, enhance the transition process, and promote effective cooperation between institutions. III. EFFECTIVE DATE, REVISION, RENEWAL AND TERMINATION AGREEMENT The designees for the Dean of the College of Arts & Sciences of the University of Delaware and the Dean of Temple University – Kornberg School of Dentistry are responsible for identifying and communicating to each other changes in the policies or requirements of their respective institutions that affect this Agreement. This Agreement will be in effect, as of the date of its signing, for students entering University of Delaware in the fall of 2014 or later. It will be reviewed annually by the appropriate parties at each institution and will be renewed automatically until superseded by new Agreements or formally terminated. Either institution may terminate this Agreement at any time by written notice at least one year in advance of the effective date of termination. IV. MISCELLANEOUS PROVISIONS A. Nondiscrimination. The parties agree that no person shall on the grounds of race, color, religion, national origin, sex, sexual orientation, or physical disability, be excluded from participation under the terms of this Agreement. B. Amendment. This Agreement may be amended by mutual written agreement of the parties. C. Counterparts. This Agreement may be executed in as many counterparts as is necessary or convenient each of which shall be deemed an original but all such counterparts shall constitute but one and the same agreement; however, the parties understand and agree that only Englishlanguage counterparts shall be in effect and control. Facsimile and electronically scanned signatures on this Agreement shall have the same force and effect as an original signature. D. Confidentiality of Student Records.
