Course Title:

Northwest Catholic Educational Leadership Institute 2012

Course Instructor: Bena Kallick, Peggy Olcott and Nancy Skerritt

Credits

The credits offered are 900-level post-baccalaureate credits that may be applied towards state educator certification and/or advancement on the state salary schedule. However, 900-level credits may not be applied toward degree work without prior approval from an academic advisor.

If you’re not sure whether your advisor will approve of the credit, please take the credit registration form with you and sign in on the attendance roster each day. After you have received the acceptance of these credits for your program of study, mail the form along with payment in to the Seattle University Professional Development Office.

The total credit registration fee is $50.00 (each credit costs $25.00). Payment is accepted by personal check made out to ‘Seattle University’ or credit card (Visa or Mastercard only).

Clock Hours

You may earn 20 clock hours for attending all three days of the Institute. Please fill out the

Continuing Education Clock Hour form and sign in on the attendance roster each day.

The total fee for clock hours is $40 ($2 per hour with a minimum of fee of $15) and payment

must be made by personal check made out to ‘Seattle University.’ If you do not have a check to pay this fee at the time of registration, please take the Clock Hour registration form with you and mail it in with payment as soon as you can. After payments are processed the Associate Dean of

Professional Development will sign the form and the original will be sent to the address listed in

Section I of the form.

Requirements

In order to receive professional development of any unit type, attendance must be verifiable.

Please sign-in on the attendance roster each day.

Transcript Requests

Participants earning post-baccalaureate credits through Seattle University may obtain a transcript reflecting this coursework after registration forms and payments have been processed and attendance is verified. Up to 10 transcripts may be requested, free of charge, each quarter. To request a transcript go to www.seattleu.edu/registrar and select the link for “Transcript

Requests.” At the bottom of the page click the link for “Transcript Request Form” under

“Options for Requesting Transcripts with Additional Forms.” Please fill out this form and mail or fax it in to the Office of the Registrar.
