OPTION 1 – “CREATE-A QUIZ”
If you choose this option, you will create a quiz that you would give to someone
after reading the book. There is a specific format that you must follow:










FORMAT: Your questions for each category should be similar to the following:
Title Page: Full title of book and author

1.) You’ve read about the rough life of Helen Keller and how she overcame obstacles. What
obstacles have you overcome in life? How did you accomplish this?
NEATNESS and CONVENTIONS
Your book report must be done neatly, preferably typed. If it’s not typed, printing would be your
best bet. Watch your conventions – spelling, punctuation, correct grammar usage…you need to
turn your book report in on time!!

Non- fiction Book Report Option
If you choose this option you will create a picture book that shares important
information from your reading. There are specific requirements.

Cover – 60 total points
 Title
 Original Author
 Original Illustrator (if applicable)
 Retold and Illustrated by: your name
 Original Illustration

Each Page: (10 individual pages required) – 10 points a page
 3-5 sentences on each page
 Illustration to help represent information given

Organization: 10 points
 Pages should be organized in such a manner that each page
naturally follows another. In other words, the order of the
pages should make sense. There should be an
introductory page and some type of concluding page.
This does not count towards your 10 pages.
