Donation Application Guidelines
Thank you for including Whole Foods market in your community outreach efforts. Please help us to serve you and
our community better by reading this form completely and submitting your application according to the following
required guidelines:

Complete the Whole Foods Market North Wales Donation Application.
Submit a formal letter of request.
Attach a copy of your organization’s federally issued letter certifying 501(c)(3) status.
Submit completed application to Whole Foods Market North Wales Customer Service desk or via email.
Due to the volume of applications that we receive, we regret that we cannot grant every request. We
evaluate applications based on the following guidelines:
Whole Foods Market accepts donation applications from local, non-profit organizations with 501(c)(3) status. The
event, fundraiser, or program must serve the local community. Whole Foods Market cannot make donations to
political or religious organizations unless the donation will be used for a local, non-secular community servicerelated event or program. Completing an application does not guarantee that we can accommodate your request.
We do not donate to individuals, individuals or teams seeking pledges, multiple requests from the same
organization or organizations and/or causes outside of the local area. Past support of an organization or an event
is not a guarantee of future support.
In order to accept and review your donation application, it must be accompanied by a copy of the federally issued
statement of your organization’s 501(c)(3) status and your nine (9) digit Federal Identification Number. This letter
comes from the Treasury Department and is different from a state tax-exempt form. If your application is not
accompanied by a copy of the 501(c)3 letter, it will not be considered. This form is required for our accounting
purposes.
Additionally, a letter of request on your organization’s letterhead must accompany the request. If it is a special
event, please also include any flyers or mailers you may have promoting the event.
Please attach any relevant literature. Requests by phone are not accepted. Please submit a request to one store
location only. That store should be closest to the community that your organization serves. Please do not turn in
a donation request to multiple stores for consideration.
The application and accompanying paperwork must be received at least eight weeks prior to the “pick up”
date in order to be considered. Applications are reviewed on a first-come, first-serve basis. Because the
donation budget is limited, early submission of your application is recommended. Last minute or incomplete
forms will not be considered.
Correctly completed applications are reviewed in a timely manner. If your application is approved, you will generally
receive notice via e-mail two weeks prior to your event. Due to the volume of applications we receive, phone
requests and applications without proper paperwork will not be considered. Each approved organization is eligible
for no more than one donation per calendar year.
