Mrs. Smith had been discharged to a nursing
home after an 8 week stay in hospital due to a
history of falls and jaundice. She had previously
lived independently at home and was eager to
return there.
We work flexible hours throughout weekdays
and weekends according to our client’s
convenience.
What we did:
Initial assessment:
The client was using a hoist for transfers and
was unable to stand or walk. She was very
anxious and had a high level of fear of falling.
She was cognitively alert and keen to progress.
Our aim is to provide an affordable therapy
service which is personal to our clients, from a
single assessment to on-going rehabilitation.
With this in mind, we will always discuss any
fees with you in advance.
Fees

Rehabilitation:
Our Therapy input began with goal setting in
order to allow us to carry out a personalised
rehabilitation programme, involving her family
and carers in the nursing home, so that we
could deliver regular therapy sessions by our
Physiotherapist and Occupational Therapist.
Result:
After the period of rehabilitation, our client
achieved:



Independence in transfers (bed, chair
and toilet)
Independence in mobility with frame
Ability to return home with support for
personal care and domestic activities.
