STANLEY COUNTY ELEMENTARY TRANSITION PLAN Early Childhood Screenings Stanley County School District holds early childhood screenings four times a year to assess developmental milestones ages of children from birth-5 years of age. Information is disseminated through the school web site, local newspaper, monthly school newsletter, and letters sent to families. At-risk children are referred for a formal evaluation through the early childhood special education department. Children in a Birth-3 program are transitioned to a part B IEP prior to turning age 3 through testing and meetings with the parents/guardians. This is done cooperatively with the SD Cares program. Early Childhood Programs into Kindergarten Stanley County School District provides services for high-risk children ages 3-5 through the use of Head Start and private preschools with the assistance of special education and related services. Plans are in place to assist children from early childhood programs into kindergarten. Children, who are 5 years of age on an IEP, are tested and meetings are held with parents and teachers (current and receiving) to review test results and develop a new IEP, if he/she is eligible for continuation of services. If a child is on an IEP or exiting from one, the "receiving" teacher meets with the IEP team to discuss IEP goals, modifications, services, times, frequency, and reintegration planning. This same process occurs throughout the elementary school years. Kindergarten In preparation for this transition, Head Start students visit the kindergarten classrooms each spring. Parents may bring their children on an individual basis to visit the kindergarten/school setting. An all school open house is held before school starts to assist with the transition process. Stanley County School District holds a Kindergarten Roundup for school-aged children and their parents/guardians each spring to informally assess the children through a variety of kindergarten activities and to provide the school information. Parents are asked to provide and complete the following information: Stanley County School District New Student Personal Data Form Collection of a certified birth certificate copy Documentation of immunizations. These documents are placed in the student’s cumulative record file and confidentially stored at the elementary school's office. In addition, the child's computer-based informational record is completed at the early childhood site upon enrollment. The cumulative record file and computer informational records are updated as changes occur through out the child's early childhood and elementary school year's experience. Building Grade Level Transitions Each spring, Stanley County Elementary teachers meet with the grade level teachers and principal to discuss needs of students transitioning out of their classrooms. Discussions are held concerning academic needs, behavior issues, and appropriate placement with fellow students. In the fall, sending and receiving teachers meet to discuss needs of the students transitioning to new grade level. These meetings have proved to be valuable for the teacher and help assure the smooth transition of students to the next grade level. Additionally, second grade students are given a tour of Parkview and the two third grade classrooms in late May in order to visit and familiarize them with the new school. Parents also have input on teacher recommendation throughout this process. Elementary to Middle School Transition The Stanley County School District cumulative record file and computer informational records are updated and continue on to middle school with the students. The middle school counselor meets with the fifth grade students to aid in transition from grade level and building. The athletic director speaks to fifth graders about required physicals to participate in sports. Special education teachers meet with the receiving teachers of students on IEPs reviewing goals, services, frequency, location, modifications and accommodations to make transition smooth for the students with special needs. Parents and students attend a 6th grade orientation before school starts in the fall. It allows time for parents and students to hear information about middle school and get acquainted with their new surroundings.
