Award Categories & Nomination Form
Faculty Awards
Academic Integrity: for a faculty member who has contributed significantly to academic integrity (on
campus or in the larger, international arena) through at least one of the following: research (on academic
integrity or related areas such as ethics, character education), teaching (e.g., talking about academic
integrity in the classroom, teaching about professional integrity), and service (e.g., Academic Integrity
Review Board; Academic Integrity Office Advisory Council; Committee on Educational Policies)
Research Integrity: a faculty member who has contributed significantly to research integrity (on
campus or in the larger, international arena) through at least one of the following: research (on research
integrity or related areas such as research ethics, responsible conduct of research), teaching (e.g.,
teaching courses or seminars on research integrity, talking about research integrity in the department or
research group), and service (e.g., Institutional Review Board, Independent Review Committee,
Institutional Animal Care and Use Committee)
Ally Awards
Academic Integrity: for a campus member or organization (e.g., student organization, staff member,
campus department) whose support of academic integrity (through activities or service) surpasses
expectations because academic integrity is not a job or organization requirement
Research Integrity: for a campus member or organization (e.g., student organization, staff member,
campus department) whose support of research integrity (through activities or service) surpasses
expectations where research integrity is not a job or organization requirement
Student Awards
Academic Integrity: for a student or alumnus, who has contributed significantly to creating a culture of
academic integrity through the volunteering of their time and energies (e.g., Peer Educator, AIM
member, AIRB member) AND/OR by exhibiting courage in speaking up against cheating, stopping
peers from cheating, or reporting cheating in the classroom.
Research Integrity: for a student or alumnus, who has contributed significantly to creating a culture of
research integrity through the volunteering of their time and energies AND/OR by exhibiting courage in
speaking up challenges to research integrity.

specific examples. Letters or other documentation in support of the examples are encouraged.
Describe the impact that the nominee has had on academic and/or research integrity on the UCSD
campus. This could be a micro impact (e.g., in a specific class or lab, or with a specific person) or
macro impact (i.e., the larger campus). Again, letters or other documentation to illustrate the
impact are welcome.
