Will your proposed research will involve assistance from Fisheries and Oceans Canada, U.S. Fish & Wildlife Service, or
U.S. Geological Survey (the contracted agents of the Great Lakes Fishery Commission)? Include any anticipated
facility, staff time, training, and equipment requests along with estimated dates (months and years) of assistance
needed here:

If your proposed research will involve live or dead sea lamprey specimens supplied by the control program agents, please
enter the number in the table below. Please note that large ammocoetes, transformers, parasitic animals, and live
spawning-phase male lamprey are costly to obtain and are limited in availability. Investigators must be conservative
with their requests.

(989-734-4768) of U.S. Geological Survey regarding adult lamprey availability prior to submitting your full proposal to
the Great Lakes Fishery Commission. If your proposed research needs will require lamprey supplied over multiple years,
it is your responsibility to remind control agents of those needs annually, before September 1 of the prior year.
