DIRECTORY LISTING POLICY STATEMENTS Accurate and efficient directory listings are an important part of customer service. The same effort and assistance should be extended in meeting customers' listing requirements as are given to other customer service requirements. The following pages contain Verizon's Directory Listing Standards (policies and procedures) applicable to white page business and residence directory listings. Exceptions require the approval of the management staff at the Verizon Directory Support Center (DSC). The DSC reserves the right to reject listings when in its judgment such listings would tend to impede the use of the Directory. DIRECTORY CLOSE POLICY A Close Date for white page listings is established for each Directory to ensure that customer listings are processed in a controllable and timely manner, permitting the careful review of all listings and the clearing of error reports and close reports prior to publishing the new directory. Therefore, the following restrictions are applied at each Directory Close: • No employee of our Company or Verizon Information Systems should promise a customer after the white page Close Date that the customer's listing or requested changes will appear in the next issue. • Due to the higher risk of error after the close of a directory, DSC will not accept listings after the white page Close Date without written communication (EMail or Fax) from a Contact Center Supervisor. • For customer-sensitive situations (universities, clinics, hospitals, etc.) involving a request after the white page Close Date, written communication will be required from the requesting center Manager to the DSC General Manager. Note regarding Neighborhood Directory Close Dates: The Close Date for a Core/Main directory matches the Close Date of the NBHD book in all states except Florida. Florida NBHD books close 4 or 5 months after the Core/Main directory, which requires a separate Close process but adhering to the same Close Policy outlined here. ADVANCED LISTINGS The close date is reflected in EZR and also in NOCV on the DIRL/DIRS/DIRC screens. This is the last date that orders submitted from Verizon Network Services via NOCV will be processed for inclusion in the next directory. If the due date of the order is after the close date and prior to the publish date (also appearing on the DIRS/DIRC screens), the order can be advanced to the upcoming issue. Any listing order issued after the final service order date will not appear in the next directory publication. It will appear the following year. (Refer to EZR>>Reference Documents>>Job Aids & Procedures – Directory>>2003 Verizon West Directory Schedule) PUBLISHING STATUS OPTIONS FOR DIRECTORY LISTINGS Every individual directory listing is assigned one Publishing Status. There are currently four (4) Publishing Status options for Directory Listings: Published, Non-Published, Directory Assistance Only, and No Listing. Emer NP & Non-Published Street Address (and Non-Published Name/Address Service) is no longer available as a Publishing Status. Continue reading for a more detailed explanation of each option. PUBLISHED LISTINGS PUB (in NOCV) When a customer chooses a Published status for a directory listing, the listing will appear in both the printed Directory and in Directory Assistance. The Telephone Company's published directory listings may be furnished to other telephone companies or to other directory publishers for the purpose of compiling, printing, and distributing printed directories. By not requesting Non-Published service, the customer agrees to the release of his name, address, and/or telephone number. NON-PUBLISHED LISTINGS NONPUB (in NOCV) A Non-Published telephone number is omitted from both the printed Directory and from Directory Assistance. Charges may apply based on state tariffs and Telephone Company policy. NON-PUBLISHED IN STREET ADDRESS DIRECTORY NO ST AD (in NOCV) (applies to California) When a customer indicates that he/she does not choose to have a free appearance in the special Street Address Directory published for their serving area, this Publishing Status allows the listing to appear in the local directory and suppresses it from the Street Address Directory. There is no charge for this Non-Published status. NOTE: This type of Publishing Status is no longer available. NON-PUBLISHED IN NAME/ ADDRESS SERVICE NO ST AD (in NOCV) (applies to Florida only) The use of this Publishing Status in Florida suppresses the customer's name and address from the Name/Address Service only. The Name/Address Service is a live voice Directory Assistance service, which provides callers with the name and/or address (excluding Zip Code) for published telephone numbers. There is no extra charge for this non-published status. NOTE: This type of Publishing Status is no longer available. DIRECTORY ASSISTANCE ONLY LISTINGS DA ONLY (in NOCV) A Directory Assistance Only listing is exactly what the name implies; the listing is omitted from the printed Directory and appears only in Directory Assistance files. There is a tariffed charge for this service. In the tariffs, this service is called NonListed Service. Non-Listed Service is tariffed in these Verizon states: California Michigan Florida North Carolina Hawaii Oregon Idaho Pennsylvania Illinois Washington Texas Non-Listed Service is also tariffed in these former Contel areas: Alabama North Carolina Missouri Pennsylvania Oklahoma South Carolina Virginia "NO LISTING" ACCOUNTS (Abbreviated NO LIST in NOCV) Some business and residence accounts, by virtue of the type of telephone service provided, are not entitled to a Directory Listing: official Telephone Company accounts, Special Billing accounts, Special Services accounts, Public Coin telephone accounts, Corporate Apartment accounts, Tele-Go Service, etc. The "NO LIST" status should only be used on the above types of telephone service; any exceptions should be referred to Telephone Company management for a policy decision. SPECIAL CLASS OF SERVICE ACCOUNTS There are certain types of class of service that require special handling. Listed below are two (2) types of special class of services. FX: Foreign Exchange A Foreign Exchange is a circuit that connects a station with a Central Office (CO) in an exchange other than that in which the station is located. In other words, a Customer who wishes to use a Telephone Number that is out of their particular calling area. Thus allowing them to advertise in another city, giving a local number for customers to call within that particular city. NOTE: California FX customers are entitled to any applicable scoping. Scoping is determined by both NPA/NXX and ACO/COMM on CA FX accounts. FC: Foreign Company FCI: (Foreign Company In) A Foreign Company-In (FCI) circuit is another LEC's dial tone terming in a Verizon exchange. FCO: (Foreign Company Out) A Foreign Company Out (FCO) circuit is Verizons dial tone terming in an other LEC's exchange. NOTE: Normally, the Service Name on a FC account reflects the LEC's name (Pacific Bell, Nevada Bell, etc.). The listing should be either Pub or Non-Pub, and should list the actual Customer's Name (Doe, John, Disneyland, etc.). TRUST ACCOUNTS Trust Account telephone service can be managed by either an individual - a Trustee, or a group who manages the trust account. The account can either be business or residential. The field installer has the authority to decide if it is a legitimate residential or business setup. There are some guidelines in which to follow: Residential Class of Service: If the premise is a residential home, the listing can be either published in a person’s name or unlisted as the Business name (Trustee or group name administering the Trust). Business Class of Service: If the premise is being used as a legitimate business location, a separate business line must be established for said business. LOCAL NUMBER PORTABILITY - LNP PORT IN: Port-In is the provisioning of Local Service using non-native telephone numbers (from other providers) or using Verizon telephone numbers in a switch that is not their local switch. PORT OUT: Port-Out is when a Verizon ILEC customer changes to another Local Service Provider and keeps his current Verizon 10-digit telephone number. The Customer must remain within the same Verizon defined Rate Center. POOLING: Number pooling is the provisioning of blocks of numbers from a common 'pool' administered by a third party. Each Local Service Provider (LSP) determines available blocks of numbers to be transferred to a common 'pool' maintained by the third party. When a LSP needs more telephone numbers to meet customer needs, it must go to the third party to request new telephone numbers. Telephone numbers are assigned by the 1,000 block. TO CHANGE PUBLISHING STATUS Requests from customers to change Publishing Status, such as to change from Published to Non-Published or vice versa, need to be handled by the appropriate residence or business Customer Contact Center in order for charges to be applied correctly per state tariffs and Telephone Company policy. Exceptions to this would be the specialized groups within the DSC that have authorization to make these changes. CAPITALIZATION RULES The Listing Name & Address fields should always be populated as it would appear in the directory using upper and lower case letters in a regular type font (non-bold). Special typographical treatment (an advertising item) is purchased from Verizon Information Systems. ACCOUNT TYPE OPTIONS FOR DIRECTORY LISTINGS The Directory Assistance system files each Directory Listing into one of four categories based on Account Type: Residence, Business, Professional, and Government. NOCV uses the abbreviations RES, BUS, PROF, and GOVT, which translate into AT1, AT2, AT3, and AT4 respectively, to reach the appropriate files at Directory Assistance. Each directory listing, whether a straight-line or an entire caption is assigned only one Account Type in NOCV. Note: A BUS listing can reside on the same Profile with a PROF listing. However, a mix of RES and PROF and/or BUS listings cannot be on the same Profile. NOCV Account Type Does this: R for Residence (RES) Sends the listing to the Directory Assistance residence AT1 file. B for Business (BUS) Sends the listing to the Directory Assistance business AT2 file. P for Professional (PROF) Sends the listing to both the residence AT1 and the business AT2 Directory Assistance files. Used on clergy listings and for business names that consist of a complete personal name in correct order (last name, first name). G for Government (GOVT) Sends the listing to the Directory assistance government AT4 file. RESIDENCE DIRECTORY LISTING REQUIREMENTS A residence Directory Listing should consist of the following components: Note: The Listing Information fields should always be populated, as it would appear in the directory, using upper and lower case letters. • CUSTOMER NAME A person's Last Name (required) A person's First Name or Initials (required) Important Information regarding First Names: Dual Name Listings (DNL) can use the first names or initials of any two persons who share the same last name. 1. The word "And" is not permitted between first names in a Dual Name Listing. An ampersand (&) is required. 2. Abbreviations for first names are no longer required: Wm for William, Edw for Edward, Jas for James, etc. Customers may list their complete first names. Middle Name (optional) Nickname (optional) Lineal Descent (Sr, Jr) Title (optional) Social titles (Rev, Dr), Military titles (Lt, Col) Degree (optional) (Refer to the TITLES & SCHOLASTIC DEGREES section) Note: If one or both persons in a Dual Name Listing (DNL) have a Title or Degree, the title/degree is entered right after the appropriate first name rather than in a separate TD field as in the following examples. The maximum of titles and degrees is three per directory listing. EXAMPLES of DNL listings with titles Jones George Rev & Ruth Smith Michael Prof & Joy Dr Exception regarding Residence customer name: Where tariffs permit the name of a ranch or farm may be used as the listed name on a residence account. Exception regarding use of a Mobile, Pager, or Cellular on a residence account: In a straight-line listing, at the customer’s request, use the DL phrase to list the verbiage Mobile, Pager, or Cellular. • STREET ADDRESS House Number (optional) Directional before the Street Name (optional) Street Name with abbreviated thoroughfare (Hwy, Av, Ave, Rd, Blvd, etc.) and trailing directional (E, SW, NE, W, etc.) (optional) (Refer to Attachment I for Street Address Abbreviations.) Refer to the ADDRESSES IN DIRECTORY LISTINGS section for more specific information about street addresses. The OMIT ADDRESS option in NOCV: A residence customer may choose to omit a street address from the Directory Listing. If so, the OMIT ADDRESS option must be selected for a straight-line listing. For a caption listing, the address is simply not typed in. • COMMUNITY The community name can be derived in all straight-line residence listings. The OMIT COMMUNITY option in NOCV: A residence customer may choose to omit the community. If so, the OMIT COMMUNITY option must be selected for straight-line listings. For caption listings, the community is simply not typed in. NOTE: A California customer who wishes to publish their Street Address must also publish their Community. • TELEPHONE NUMBER The ten-digit number consisting of NPA/NXX and the four-digit line number assigned to the customer. FICTITIOUS NAMES IN RESIDENCE LISTINGS The use of fictitious listed names is discouraged. Exceptions require the approval of Contact Center management, and a retained remark (NT) is required in the Directory Listing section of the customer's residence Profile to record the circumstances and the name of the approving manager. Fictitious names should only be accepted when Contact Center management is convinced there is no intent to defraud the Telephone Company. NOTE: Hawaii requires a Guarantor's Form. ADDITIONAL RESIDENCE LISTINGS Additional straight-line residence listings may be furnished in the names of relatives or other persons residing with the responsible party. Establish additional residence listings following the standards previously described for Residence listings. (For residence caption listings, refer to CAPTION LISTING REQUIREMENTS.) CHILDREN'S DIRECTORY LISTINGS The majority of the time, telephone numbers listed for children are indented under a Caption Header that contains the name of a parent/guardian. The caption will have a Residence or Professional account type. An acceptable alternative would be to list the children's actual names as straight-line residence listings (limited to two first names in a Dual Name Listing). A straight-line listing such as "Gardner Children," or just "Children," is unacceptable from a Directory Assistance perspective. It also does not meet the requirements that a residence listing must consist of a person's last name and first name; the word "Children" is not an actual first or last name. Caption Information phrases may include, but are not limited to, the following: Children's Phone Family Phone Daughter's Telephone Son's Telephone Family Telephone Teen Phone BUSINESS LISTING POLICIES A business listing is questionable when it appears to be designed for the purpose of the customer's obtaining prominence or a preferential position in the directory. A Accent On Style (Will file first under the A’s in the White pages and in the Yellow pages under the customer’s Yellow Page Heading). WISE GUYS Sports Bar (will stand out like a bold listing in the white pages) Sometimes an unacceptable acronym (capital letters that form a word) is used in an effort to obtain prominence or preferential position in the directory. Unacceptable acronyms are those in which each letter does not stand for an individual word, such as those that follow: JAY’s Diner CREPACO Inc UNI-MART Corporation Customers should be questioned regarding what each letter stands for when they request business words to appear all in caps. Examples of acceptable acronyms are: MADD Mothers Against Drunk Drivers) CUNA (Credit Union National Assn) SAL's Place (Steve And Larry's Place) A business listing is questionable when it appears to be designed for the major purpose of directing persons to the telephone number of a customer from whom a particular service or commodity may be purchased. VCR Repair Pizza…….See Anna’s Family Restaurant A business listing is questionable when it appears that two or more unrelated business names are being combined to avoid an additional business listing charge. Ray's Body Shop & Ruth's Catering Red's Pharmacy & Red Dot Video Primary Listings (PL) and Additional Listings (AL) can be questioned. Questionable listings are unacceptable unless the customer can provide evidence (i.e., letter of registration, DBA) that the business is operating under the name in question. Verizon will not undertake to determine the customer's legal or contractual right to the use of a business name. When a DBA (Doing Business As) is requested, it is recommended that a CR be used in lieu of a LI with the DBA. Example: PL International Business Systems LI dba IBM TN 123/456-7890 Preferred listing: CR IBM CN 123/456-7890 RJ See International Business Systems When a business name is questionable, the customer must accept full responsibility for furnishing evidence of authorization to use the business name or to prove that business is actually conducted under that name. When such business name authorization is received, the following information should be recorded in the Directory Listing section of the customer's account in a retained remark (NT): BUSINESS NAME AUTHORIZATION ON FILE, XX-XX-XX (date). USE OF SPECIAL CHARACTERS IN DIRECTORY LISTINGS Special characters that can be used in white page directory listings are limited to the following: apostrophe ( ' ), hyphen ( - ), virgule ( / ), single quotation marks (' '), quotation marks (" "), ampersand (&), parentheses ( ), colon ( : ), asterisk (*), period (.), at (@), number (#), dollar ($), underscore (_), plus (+), tilde (~), question mark (?), exclamation point (!), minus (-), backslash (\), caret (^), comma (,) and brackets ( [ ] ). Special characters now require a File As when they are used in lieu of a word/name. Refer to the FILE AS section for additional information. Refer to Internet Listings for the use of other special characters in internet listings. USE OF THE WORD "The" IN BUSINESS NAMES The appearance of the word "The" as the first word in a business name is to be discouraged because of the difficulties that Directory Assistance operators, as well as users of the directory, will encounter in locating the listing. If the customer still insists the word "The" be used, suggest the following options: • Subscribe to an additional business listing omitting "The" as the first word: Primary Listing (PL): The Great National Bank Additional Listing (AL): Great National Bank The • Place the word "The" at the end of the Business Name. Primary Listing (PL): Great National Bank The If the customer is not receptive to either suggestion, accept the business name with "The" appearing first, and add a retained remark (NT) to the Listing section: 'THE' USED PER CUSTOMER REQ. BUSINESS DIRECTORY LISTING REQUIREMENTS A Business Directory Listing should consist of the following components: NOTE: The Directory Listing Information fields should always be populated as it would appear in the directory, using upper and lower case letters. • BUSINESS NAME Defined as the name under which the business is operating, which may include: • the name of a company, corporation, agency, etc. • the name of an individual person, including title or degree NOTE: For a Dual Name Listing (DNL) to be acceptable as a PL on a business account, both persons sharing the same last name must be associated with the business. DNL's are more acceptable as a business AL, not a PL. For example: Davis, John Dr & Mary. NOTE: Hawaii's tariff allows dual name listings on residence only. For more information about the acceptability of business names, refer to the following sections: BUSINESS LISTING POLICIES PROFESSIONAL DIRECTORY LISTING REQUIREMENTS TITLES & SCHOLASTIC DEGREES • BUSINESS DESIGNATION (optional) Consists of an abbreviated word or phrase in lower case following the Business Name and which describes the general nature of the customer's business. Designations appear only in the white pages. NOCV users should always establish a designation with a DE keyword. Do not establish as part of the "PL" listing text in NOCV. EXAMPLES of Business designations: Bottoms Up tavrn Fantastic Futures invstmt mgmt Smith, John atty • STREET ADDRESS House Number (optional) Directional before the Street Name (optional) Street Name with abbreviated thoroughfare (Hwy, Av, Ave, Rd, Blvd, etc.) and trailing directional (E, SW, NE, W, etc.) (Refer to Attachment I for Street Address Abbreviations.) Refer to ADDRESSES IN DIRECTORY LISTINGS for more information. The OMIT ADDRESS option in NOCV: A business customer may choose to omit a street address from the Directory Listing. If so, the OMIT ADDRESS option must be selected for a straight-line listing. For a caption listing, the address is simply not typed in. • COMMUNITY The community name will be derived in all straight-line business listings. The OMIT COMMUNITY option in NOCV: A business customer may choose to omit the community. If so, the OMIT COMMUNITY option must be selected for straightline listings. For caption listings, the community is simply not typed in. NOTE: If the customer wishes to publish their Street Address, the community must be published as well. This applies in all areas. • TELEPHONE NUMBER The ten-digit number consisting of NPA/NXX and the four-digit line number assigned to the customer. CLASSIFIED HEADINGS A Classified Heading (CH) indicates under which category a Business, Professional, or Government Listing will appear in the yellow pages of a directory. Customers are entitled to one complimentary yellow page appearance for each Primary Listing (PL). DDR & EZR contains a complete list of acceptable headings. If the customer does not wish to appear in the yellow pages, the word "NONE" is used as the CH entry. This ensures a yellow page sales rep will not contact the customer. If the Contact Center employee and the customer cannot determine the exact heading for the order but the general nature of the business is known, the rep should enter a general heading as the CH entry, i.e., Attorneys, Physicians & Surgeons, MD, Dentists, Churches, Contractors-General, etc. – this field should not be left blank. (PAVA VA: Refer to EZ Reference>>Reference Documents>>PAVA-Job Aids & Procedures>>Classified Headings (fGTE)) Classified Headings must be established on all new installs ("I" orders) for published Business, Professional, and Government listings. CH's must also be established when a customer regrades from Residence service to Business or Professional service. The CH appears only on the pending service order and is not retained on the Customer Profile. Subsequent customer requests to change a Classified Heading should be referred to VIS. NOTE: In California – Tariff states that customers may omit their addresses except where the type of business involves the removal of appliances, furniture or articles of value from the owner’s premises, or where required by law, the address at which the customer is conducting business must be included in the business listing. ADDITIONAL BUSINESS LISTINGS Additional straight-line business listings may consist of other names under which the business of the customer may be known. Dual Name Listings (DNL) are available as additional business listings but require a Professional account type. (Refer to PROFESSIONAL DIRECTORY LISTING REQUIREMENTS for more information.) NOTE: Hawaii's tariff allows dual name listings on residence only. Additional business listings follow the standards previously described for Business listings. They appear only in the white pages unless the customer contracts for an appearance of the listing in yellow pages. Additional listings (AL's) usually use the same address as the Primary Listing (PL). However, business customers may choose to use the address of an off-premises extension, of a side street (if on a corner), or an address other than the one where main telephone service is located, provided they are actually doing business at that location. NOTE: Refer to the BUSINESS LISTING POLICIES for guidelines in determining the acceptability and unacceptability of an additional business name. For business caption listings, refer to CAPTION LISTING REQUIREMENTS. (Applies only to Hawaii) When a customer wants a Business listing to appear with a Residence primary listing, the customer's name and office telephone number (and the name of his business if desired) will be furnished as an Additional Business Listing, as in this example: 0 1 1 100 CP Adams, Jack 100 TD AIA 150 CI Partners Inc 150 TN 502/949-3333 200 CI Res 200 DA 1431, , Hiikala Pl 200 TN 502/732-5555 PROFESSIONAL DIRECTORY LISTING REQUIREMENTS A Professional listing consists of the name of an individual (last name and first name, in that order), whose class of service is business. The listing may include an optional Title, Degree, or Business Designation. No other business words (such as Company, Agency, Shop, Insurance, etc.) may appear in the listed name of a straight-line listing or in a Caption Header line. The presence of a business word or a reversal of the customer's last name and first name requires a Business account type, not a Professional one. EXAMPLES of listed names that require a Business account type: C H Helwick & Sons Henley Tom Auto Service Edward Jones & Associates J J Walsh atty (DE) J J Walsh Atty (TD) J J Walsh Attorney (Attorney is part of the listed name) A Professional account type puts a customer's listing in both the Business and Residence files at Directory Assistance, which is why the following restrictions apply to the customer's name in the listing. NOTE: The Listing Information fields should always be populated as it would appear in the directory, using upper and lower case letters. Last Name (required) First Name (required) Lineal Decent (ex: Jr, III, etc.) (optional) Title (ex: Dr, Rev, Atty) (optional) Degree (ex: MD, DDS, OD) (optional) Business Designation (ex: atty, ins, rl est) (optional) Additional Listings (AL's) for individual residents of a nursing home or apartment complex must be established with a Professional account type when the listings are all set up on the business Profile with the Primary Listing (PL) for the business. Customers who operate a business from their home (and are paying business rates for the telephone service) may establish an Additional Listing in their personal name(s). A Professional account type (PROF) would be used on the AL or DNL. NOTE: Dual Name Listings (DNL) consist of the first names or initials of two persons who share the same last name. For a Dual Name Listing (DNL) to be acceptable as a PL on a business account, both persons sharing the same last name must be associated with the business. DNL's are more acceptable as a professional AL, not a PL. For example: Davis, John Dr & Mary. If one or both persons have a Title or Degree, the title/degree is entered right after the appropriate first name rather than in a separate TD field. Examples of DNL listings with titles/degrees: Jones George PhD & Ruth RN Smith Michael Jr & Joy The listed Street Address, Community, and Telephone Number in a Professional listing should follow the same standards as those for Residence and Business listings. DIRECT INWARD/OUTWARD DIAL (DID/DIOD) LISTINGS The purpose of DIOD Trunks is to provide both in and out service on the same trunk. These two-way trunks eliminate the need for separate incoming-only and outbound-only trunk groups since all calls use the same single group of trunks. DIOD applications are no different from DID applications and is appropriate in every new and existing DID application. Typical DIOD users are PBX applications which might include: Hospitals Colleges/universities School systems City/County governments Telephone answering services Large manufacturing Large retail Banking DID numbers are not entitled to a free directory listing. If the customer wants a listing with that trunk number the listing would be established as an AL and would be charged for an additional listing. NOTE: Centrex Service is similar to PBX accounts (DIDs). Like the PBX, the main TN is entitled to a free listing; if the customer wishes to list one or more of the station TN’s in the local directory it should be established as a AL along with the appropriate IOSC(s). NOTE: When handling DID's, or station lines (centranet) and you are establishing an ML into another directory, establish the first appearance as a PL and use the appropriate IOSC. FAX LINE/COMPUTER LINE/MODEM LISTINGS Published listings for business or residence Fax Line and Computer Line (or Modem) telephone numbers may appear in straight-line format or caption format. In a straight-line listing, a RJ keyword should be used instead of an LI keyword to identify the TN as a Fax or Computer or Modem line, as in the following example: PL 100 Walters, George DA 233, , Livingston Rd CM Anytown RJ Fax TN 403/234-5678 In a caption listing, enter the appropriate text in a CI (Caption Information) phrase, as in the following example: 0 200 CP Anna’s, Pizzeria 1 250 DA 122, W, Ontario St 1 250 CM Anytown 250 TN 402/222-2222 300 CI To Fax Orders 300 TN 402/222-3333 In Hawaii only - Use DL instead of DE and omit address on fax lines or modems. CELLULAR/MOBILE & PAGER LISTINGS Cellular Phone or Pager service usually involves a customer and a vendor (supplier of the Cellular/Pager service). Often the vendor is unknown. Do not assume that our Company is the vendor when there is a Profile for the TN in NOCV. Our Company may be leasing those lines to a different vendor who does not expect to pay charges for a listing. Listings for Cellular Phone and Pager service are usually established as billable Foreign Listings on BFIC Profiles, with the bill going to the customer who actually purchased the Cellular/Pager service. If a Verizon customer has a local account and requests that we list his Cellular or Pager number, a BFIC/RFIC account should be established, unless the telephone number is established with a local number in a caption arrangement. Most published Cellular Phone/Pager listings are established in caption format because the customer has other local telephone service. If this is the case, use an appropriate CI (Caption Information) phrase, such as Cellular Phone, Cell Phone, Mobile Phone, or Pager. In a straight-line listing, at the customers request, use the RJ keyword to list the verbiage Mobile, Pager or Cellular. COIN TELEPHONE LISTINGS Telephone numbers associated with semi-public coin telephone service and with customer-owned coin phones should be set up as either Published or NO-LIST. Telephone numbers associated with public coin phones should be set up as NO-LIST.** See below for the Class of Service values for coin telephones. BCTS = Coin - Semipublic BCOP = Customer owned pay telephone BCTP = Coin - Public **Note: Embedded Base Coin Accounts may reflect the IOSC 05051 – Non-Pub/No Charge. Going forward, New Coin Accounts will either be Published or NOLIST. CALL FORWARDED LISTINGS & ENHANCED CALL FOWARDED LISTINGS Directory Listings for Remote Call Forwarded/Enhanced Call Forwarded business/residence telephone services are optional and may use any of the available publishing statuses, except NO LIST. RCF listings should follow the standards for regular business/residence listings, except for the address, which may include whatever address the Customer wants to list. It can be a house number and street name, community name, or a combination of them all. The listing should never include the service address, which is the Central Office address not the customer's premise. If the customer selects the OMIT ADDRESS option, encourage the appearance of at least the local community (CM) in the listing. Personal ECF listings should follow the standards for regular business/residence listings, except for the address, which should only include the Customer's community, whether it be local or not. The listing should never include the service address, which is the Central Office address not the customer's premise. Existing ECF is a calling feature (not a stand alone telephone number). Existing ECF works off the customer’s existing telephone number and does not get a listing. NOTE: The delivery address on the NOCV DIRM screen should be changed to BA (Billing Address) and the NOD (Number of Directories) changed to 0. NOTE: California RCF/ECF customers are entitled to any applicable scoping. VANITY NUMBER LISTINGS "Vanity" telephone numbers are those that "spell" something, like 234-BABY or 666-EYES. Vanity Numbers, also known as Customized Number listings, can only be offered in those states where they are tariffed. Because of limitations in VAST and Directory Assistance requirements, all entries in Telephone Number fields must be numeric. Vanity Numbers should be entered as DL phrases (at no charge to the customer). In a caption arrangement, a DL phrase will ensure that the Vanity Number will appear on its own line within the directory. The Vanity Number will also go to the Yellow Pages. No address, community, or telephone number should appear in Cross Reference listings; the OMIT ADDRESS and OMIT COMMUNITY options should be selected for straight-line Cross References. In place of a TN field in the listing, a Control Number (CN) entry containing the Profile's main telephone number is included to enable us to locate the Profile on which the CR appears. NOTE: A 10-digit number is required in the CN entry (NPA + 7 digits, or the complete toll free number). Cross Reference listings may be furnished at no charge when, from a service standpoint, our Company determines one is necessary to facilitate the use of the Directory and not as an advertising medium. See The Blue Government Pages EXAMPLE of a straight-line CR listing in NOCV format: KEY SEQ# DIRECTORY LISTING INFORMATION CR Fiss, -Jones Funeral Home 301 CN 804/333-9999 RJ See Jones Funeral Home JOINT USER/SHARED SYSTEM SERVICE LISTINGS Joint User, Shared System or Shared Tenant service is an arrangement whereby an individual, firm, corporation or association doing business under a separate name, occupies the premises on which the main customer's primary business or off-premises service is located, and shares in the use of the telephone service. A Shared System or Joint User directory listing may carry the same telephone number or a different number when separate lines, groups of lines in rotary sequence, or station lines of a Centrex or DID system are assigned to the customer. Listings may contain a Business name or an individual's name. A JU is a type of Additional Listing, it is also a chargeable item, and so therefore when establishing in NOCV, it should be established with a Sequence Number in the 01 series (i.e. 101, 203, 310). When a JU is entered into a caption arrangement, and it is entitled to a CH, the CH can be entered on line below the listing and VIS will associate that CH with that listing. NOTE: Reminder that a CH key is not retained on Profile. It will 'fall off' upon completion of the order where it was added. Changes to a CH should be directed to a Yellow Page representative at Verizon Information Systems. Requests for this service must be handled by the appropriate Customer Contact Center to ensure regulations are followed. Very few areas still offer Joint User/Shared System Service. In most states, the service has been grandfathered or eliminated. Those areas that still offer Joint User service include the following: California Hawaii In some former Contel areas, the service is still tariffed. Those areas include the following: Alabama California Indiana South Carolina Arizona Illinois Virginia Pennsylvania INTERNET LISTINGS & E-MAIL ADDRESSES - WHITE PAGES In all states, there is a New ‘EA’ key word for the entry of EMail and Website Addresses in the white pages for both business and residential customers. • The new Key Word ‘EA’ is used for entry of E-Mail or Website Addresses for our Customers on the DIRS/DIRC/DIRE screens. The ‘EA’ key word will place the customer’s E-mail/Website address on a separate line as part of his listing. • The charge for an ‘EA’ listing is the same as an Additional Listing in that state. • Be aware and inform a Business Customer that an EMail or Website Address entered as an ‘EA’ will print ONLY in the White Pages. • Business customers requesting a listing of the E-Mail or Website Address in the Yellow Pages as well must be referred to Verizon Information Systems for assistance with their advertising needs • A URL (Website address) may contain special characters and/or numbers, and as well, any combination of upper and lower case letters. The address must include a period and be followed by one of the many URL extensions (i.e. com, org, net, gov). The address should not include a space. • An E-mail address may include special characters, numbers, and any combination of upper and lower case letters. It must also contain an At symbol (@) and a period. It should not include any spaces. • The following special characters are allowed in the ‘EA’ field on the DIRS/DIRC/DIRE/DIRM screens: @ # $ _ +\.~?%={}><`!*&()-:‘/, • A customer can request an internet URL as his listing name (PL, AL, JU). If the listing name is an internet URL, i.e. Drugstore.com, the listing can be entered using the following special characters. • These special characters will be accepted in a PL, AL, JU, CP, MC, CR, RJ, and DL key word. NOTE: The listing is filed without regard to the special characters: @ # $ _ + \ . ~ ? ! - ' / & : ( ) , The finding word should consist of the complete URL and be followed by the comma. EXAMPLE of a Internet Address using the PL keyword in NOCV: HomeGrocery.com, Inc. It will Auto Populate on the SERV screen after an ‘EA’ key word has been entered. The IOSC's are 00320 Internet Address (Business) and 00322 Internet Address (Residence). Additionally, there are 2 IOSC's on the FRGN screen that are used for billing intercompany foreign white page listings. Unlike the SERV screen ‘EA’ IOSC's, these FRGN IOSC's will have to be manually selected, they will not auto populate. The IOSC's are 60278 Foreign Internet Address (Business) and 60279 Foreign Internet Address (Residence). • You must enter the LI/EA's in the order in which you wish them to appear in the directory. If you are adding LI/EA's to an existing listing set, you must delete the existing listing set and re-enter it, making sure that you key the LI/EA's in the order in which you wish them to appear in the directory. Both LI's and EA's will appear on their own line and as indented one level in the directory when used in straight line listings. When used in captions, the actual sequence number and indention level used will determine the directory appearance. • Limited special characters are allowed in a LI. They are - / : ( ) , . * &. An internet address should not be entered in a LI field. The rules for the entry of a LI have not changed. A Line of Information (LI) contains descriptive or directional information for the purpose of facilitating the use of the directory. Monthly charges apply according to the appropriate state tariff. Acceptable Lines of Information include supplementary address information, hours of operation, or mailing information. LI’s may not be used to advertise the services or products of the customer. TELE-GO SERVICE LISTINGS Tele-Go service, like cellular service, allows a customer to carry his/her phone wherever he/she goes and use the same TN for home and wireless service. According to Company procedures established for Tele-Go service, the account can have no listing. The Publishing Status should be established as NO LIST in order to minimize fraud for the user. IN-CONTACT SERVICE LISTINGS In-Contact service is a paging service similar to Tele-Go in that the customer can be reached wherever he/she goes. Even though the majority of customers do not want the number listed, it is an option. Therefore, the Listing Status can be Published or Non-Published. NOTE: This service is no longer available. CORPORATE APARTMENT LISTINGS Large corporations rent residential apartments or condominiums for employees or customers to use for short periods of time. The Service Name on the profile should contain the Corporation's name. According to Telephone Company procedures, these are established as residence service with no listing; the listing status should be established as NO LIST in NOCV. A retained remark should be entered on the account (not in the listing section) identifying it as a corporate apartment arrangement. NOTE: Some existing corporate apartment listings may have been established as professional listings. Consider these accounts "grandfathered" and handle them accordingly. (Refer to Attachment IV: GLOSSARY OF DIRECTORY TERMS for an explanation of Grandfathered Listings.) NOTE 2: Not applicable in Hawaii. Listing appears in Corporation name. NOTE: Some existing Corporate Apartment listings may have been established as Professional listings. Consider these accounts "grandfathered" and handle them accordingly. (Refer to Attachment IV: GLOSSARY OF DIRECTORY TERMS for an explanation of Grandfathered Listings.) DISTINCTIVE RING LISTINGS (Formerly Smart Ring) The Publishing Status on an account that has Distinctive Ring service should be Published or Non-Published (not NO LIST). Residence customers may publish a Distinctive Ring telephone number in a straight-line or caption listing. Business customers may also select a straight-line or caption format and are allowed a Classified Heading for the Distinctive Ring number. (Except in Hawaii, where Business customers are allowed a listing in white pages only). RESERVED TELEPHONE NUMBER LISTINGS For a monthly fee, customers may disconnect their telephone service but reserve the telephone number for future reconnection. This option is often selected by seasonal businesses, such as resorts, landscapers, and amusement parks. The only choices for Publishing Status on a Reserved Number account: PUB (Published) or NON-PUB/DA ONLY. SOHO (Small Office/Home Office) LISTINGS Verizon Network Services has not approved SOHO service (Small Office/Home Office) for residence customers. However, in some states Verizon Information Systems sales personnel are selling advertising to residence customers who qualify as SOHO candidates. The residence customer must be operating a business out of the home and must advertise under the residence name as it appears in the Primary Listing. Until further notice, customers who request SOHO service should be referred to Verizon Information Systems. TOLL FREE/BUSINESS LINE 800 LISTINGS In October 1996, a new service offering was developed to replace the Business Line 800 (BL800) service. The new service is called TOLL FREE, and it allows the customer's toll free number and local telephone number to be on the same Profile (combine-billed) rather than on separate Profiles, each generating a bill. Going forward, all new requests will result in Toll Free service, but existing BL800 accounts still remain in NOCV. Listings for Toll Free numbers may be established on the local profile and can be entered in straight-line or caption format. If the customer wanted the Toll Free number listed in another directory other than his local, the listing could be established as a multi-book, if the foreign directory is the same company code as the local account. If the Toll Free number is not combine-billed, a BFIC/RFIC account would need to be established in the correct NOCV environment for the Toll Free number. A straight-line listing requires a FL Code. (Refer to FOREIGN LISTINGS for an explanation of FL Code usage.) A caption listing established on a local Customer Profile does not require a FL Code if a local telephone number is the first number to appear in the caption. However, if the toll free number is listed first in the caption, a FL Code is required. Currently there are eight Toll Free numbers that are used today. These are: 800, 822, 833, 844, 855, 866, 877, and 888. CHURCH & CLERGY LISTINGS Individual state tariffs (Verizon Illinois and Verizon South Carolina) allow churches to be established with residence rates on business orders. (South Carolina has expanded the policy to include schools.) Other states apply the same rules and regulations to churches and clergy that they apply to all businesses. If the name of the church is listed, business rates apply to the service. If member of clergy wants listings in yellow pages, business rates apply to the service. Therefore, residential accounts can NOT carry a business or professional listing. If member of clergy wants listing in the yellow pages, business rates apply to the service. Service for members of the Clergy may be established at residence rates if the directory listing contains an individual's name. The listing may also include a clerical title, such as Father, Fr, Reverend, Rev, Sister, Pastor, etc. Common abbreviations for clerical titles are encouraged but not required. The Verizon Illinois and Verizon South Carolina state tariffs further allow members of the Clergy a complimentary appearance in the yellow pages under the Classified Heading of "Clergy." Generally, clerical titles would follow the individual's Last Name or First Name entry, but members of the Clergy have the option of appearing alphabetically by Title rather than by Last Name in the directory. Here are examples of how members of the Clergy can be listed: Directory Appearance PL entry in NOCV Olson Daniel Fr Olson, Daniel (title Fr requires TD key) Olson Father Daniel Olson, Father Daniel Father Daniel Father, Daniel NOTE: The embedded database which contains existing church or clergy listings on residential accounts are considered grandfathered, until there are customer requested directory listing changes made on the account. At that point, the account will no longer be considered grandfathered and necessary action to correct the account must be taken. SCHOOLS - DIRECTORY LISTING POLICY Listings for schools must be established on accounts that carry a Business or Government class of service. NOTE: South Carolina tariff allows schools to be established on Business accounts with Residential rates. GENERAL FILING INSTRUCTIONS FOR BUSINESS LISTINGS Directory Listings are filed alphabetically, following the same guidelines as a dictionary. When two or more listings are exactly alike up to the address, numbered streets (1st Av) will precede named streets (Oakwood Dr). Numbered streets will be listed numerically. Named streets will be listed alphabetically. When a business name is unusual (a group of capital letters, a number, or a combination of alpha letters and numbers), special filing instructions must be included with the directory listing to alphabetize it properly in Directory Assistance and in the printed Directory. The Letter Sequence option and "File As" phrases provide the directory and Directory Assistance systems with the information needed to file these listings correctly. Each one is described in detail in the next two sections. LETTER SEQUENCE The Letter Sequence (LS) option should be included with a listing when the first word consists of a multiple letters rather than a complete word provided there are no other words in between. Names beginning with capitalized letters (A, AAA, etc) will now be sorted in ascending order, regardless of whether or not they are followed by a word (AA Motors, AAA Construction). Hyphens are treated as a space and are ignored. The Letter Sequence option files the listing in alphabetical order near the beginning of the appropriate alphabetical section of the Directory and Directory Assistance: NOTE: An LS is not required on a listing if the finding word consists of a single letter. See examples below. Ex. A, Beautiful Place (no LS required) B, Loved Flowers (no LS required) EXAMPLES of listings that require the Letter Sequence option: A & B, Beverage Mart R E, Smith & Sons J B's, Health Club*** A 'N E, Construction* ABC, Variety Stores A And E, Construction* L&K, Motel The WXIK AM, 97 3 A-Z, Plumbing Inc Rbuzz, Learning Center WKEY FM, 108 9 USAir.com, EZTel, Communications E Solutions.com, * The &, 'And', and 'N should be treated as spaces for filing purposes. *** When the last letter has the possessive 's', it can be negotiated with the customer and treated as a word. A FA must be used to ensure proper placement in the directory. See below for example. Ex. NOCV PL: J’s, Pool Hall NOCV FA: J, Pool Hall When letters are combined to form an acronym (word) in a business name, each letter must stand for an individual word as in the following examples: MADD (Mothers Against Drunk Drivers) CUNA (Credit Union National Assn) SAL's Place (Steve And Larry's Place) Use of the Letter Sequence option on the above listings is not required. The customer has a choice as to where the listing appears in the Directory, near the beginning of its alphabetical section (LS) or in alphabetical position based on the complete spelling of the word (FA), or in word position (Madd). The filing of a listing is always based upon customer preference, and a retained remark (NT) should be included with the listing to record the customer's request. Refer to BUSINESS LISTING POLICIES for more information about acceptable and unacceptable acronyms.) Read the next section, "FILE AS" REQUIREMENTS, which describes two situations that require both the Letter Sequence and a "File As" phrase. "FILE AS" PHRASES File As phrases should always be established according to customer preference. This is very important because the File As phrase determines the placement of the listing in the directory. (In many cases customer verification may be needed.) The FA phrase itself must meet the following requirements: 1. A comma follows the first word in the File As phrase. 2. No special characters can be used in the FA phrase. I.e., asterisk, hyphen, apostrophe, ampersand, virgule, etc. 3. All abbreviated words must be spelled out. 4. A business designation (DE) should not be included. 5. Special characters must be spelled out according to customer pronunciation when the character is used in lieu of a word/name. The word "and" or its' variations (&, an, n) are not included. When a Business listing starts with multi-words and/or prefixes, such as those included in the list in this section, all the parts that form the first word of the business name are considered the filing word and are followed by a comma in NOCV. LLC, LLP & PLLC are titles that are affiliated with businesses, therefore any listing that includes one of these titles should have a Business account type (AT2). * These titles (LLC, LLP & PLLC) should be included in the business name (PL, AL, CP, etc.) rather than in the TD keyword. The following list contains suggested abbreviations for common Titles. This list is not all-inclusive, and the use of these abbreviations is encouraged but not required. In addition, the use of periods in the TD keyword is discouraged. (Refer to Attachment VI for a more detailed list of Title & Degree abbreviations.) EXCEPTION: Hawaii can list last names only with Mr. or Mrs., etc. Multiple titles for a customer (maximum of three), such as Jr Dr, are also acceptable. They are both listed in the TD entry with spaces (no commas) between them. First, enter the title the customer was born with; then enter the title or degree earned later in life. (Refer to Attachment II for suggested Military Title abbreviations.) Note: Titles entered on Professional and Business listings will appear in both the white and yellow pages. ADDRESSES IN DIRECTORY LISTINGS An address is universally thought of as having two parts: a street address and a community name. For Directory purposes, the two parts are considered separate listing entries and are described in separate sections of this document The street address in a directory listing may consist of three parts: 1. a house number, 2. a directional, such as N,S,E,W,NE,NW,SE,SW, 3. the name of a street, office building, firm name, shopping center, mall, apartment complex, mobile home park, or a rural route and box number. The NOCV system expects all three parts to be included in a Directory Address (DA). If any part is omitted, spaces must be used in place of the missing items, as shown in the following examples of DA entries: 123, W, Parker Rd 123, , Parker Rd ,W, Parker Rd , , Parker Rd A customer may choose to omit the entire street address from the Directory Listing. If so, the OMIT ADDRESS option in NOCV must be selected for a straight-line listing. For a caption listing, the address is simply not typed in. NOTE: Street addresses are not permitted in business Cross Reference (CR) listings. (Refer to CROSS REFERENCE DIRECTORY LISTINGS for more information). Likewise, street addresses are not acceptable in caption lines that contain Alternate Call (AC) phrases or "Or" phrases. (Refer to ALTERNATE CALL NUMBERS IN CAPTIONS and USING THE WORD "Or" IN CAPTION LISTINGS for more information). Generally, the address in a Residence or Business listing will be that of the premises at which telephone service is furnished. A listed address does not have to be an exact match of the service address, but if it is, the listing address can be derived by using the Key Word 'SA'. SA (Service Address) in NOCV The address in a listing should: 1. suit the needs of the customer, 2. enhance the content of the directory, 3. and conform to the requirements that follow. If the customer wants to list an address that differs even slightly from his Service Address, the NOCV Key Word DA (Directory Address) must be used to establish the street name entry. Usually, all listings for the same business will use the same street address, the one where main telephone service is located. However, the business customer may want to use the address of an off-premises extension, a side street (if on a corner), or an address other than the one where the main telephone service is located, all of which are acceptable provided they are actually doing business at that location. Listings for Remote Call Forwarded (RCF) and Enhanced Call Forwarding (ECF) telephone service may include the Customer's terminating address, whether it is local or not. The Service Address should not be used because it is the CO's address, not the customer's premise. If the customer selects the OMIT ADDRESS option, encourage the appearance of at least the local community (CM) in the listing. The use of common abbreviations for streets and thoroughfares is encouraged in directory listing addresses but not required. Refer to Attachment I: STREET ADDRESS ABBREVIATIONS for a list of the most common ones. NOTE: The abbreviation St for Street is an acceptable abbreviation and can be used at the customer's request. It is recommended that St be used when the street name consists only of an alpha letter as in 144 A St. The following four (4) kinds of addresses are not encouraged but are available upon customer request: 1.) CORNER ADDRESSES are acceptable upon customer request. (Example: Main & Broadway) Both street names should appear in the ST (Street) field and must include an ampersand (&). No house numbers may appear with corner addresses. If a customer wants the words "Corner Of" actually to appear in the listing (EX: Corner Of Main & Broadway), the corner address becomes a chargeable Line of Information. 2.) SUBLOCATIONS are acceptable upon customer request. Address sublocations that consist of five letters or less do not require abbreviations: Apt Floor Lot Room Suite Pier Slip Unit Actual numbers associated with sublocations are permitted (but not encouraged) at the customer's request, such as Apt 23, Lot 2-H, Suite 120, Unit H44, PMB 4A, Slip J-40, etc. The list of other common sublocations and their abbreviations as shown below is not all-inclusive. The charge applies to the address line considered to be the "extra" one, which, in the following examples, is the first one. The "main address" should be entered following regular listed address standards. A complete mailing address requires two lines; therefore, two LI charges apply, as shown in the example. If a customer opts to omit his/her street address from the listing, the LI charge is still applicable for the mailing address. Substituting a mailing address for a street address to avoid the LI charge is not permitted. 5) ZIP CODES may be included upon customer request. A mailing address with zip code is acceptable in a Line Of Information (LI) phrase. The complete mailing address will usually require the use of two or more LI's. The ability to determine which telephone numbers should appear in a directory based on NPA/NXX is being greatly reduced; therefore SERVICE SCOPING in VAST is used to determine which phone numbers belong to a designated physical area for inclusion into a specific telephone directory. The Zip Code value is a required entry with new order activity. When establishing a straight-line listing, NOCV will prepopulate the ZP keyword and derive the zip code data just as it does SA, CM, and TN. When you have an existing straight-line listing and the Zip Code needs to be added, add the ZP keyword to the listing group and NOCV will derive the 5 digit Zip Code from the Service Address. It is required on every straight-line listing regardless if the customer has omitted the address and/or community from the listing. On caption listings, Zip Codes (ZP keyword) will be required on all SA or DA lines of caption. ZP keyword characters must be 0 through 9 or A through Z (caps only). A Zip Code cannot be all zeroes (00000) or all nines (99999). A hard edit requiring a Zip Code (ZP keyword) will appear whenever a SA or DA is used, and the 'ZP' keyword is not entered. COMMUNITY NAMES IN DIRECTORY LISTINGS Communities (CM) can be derived in NOCV on all straight-line listings. If a customer wants to omit the community name, the OMIT COMMUNITY option must be selected for a straight-line listing. For caption listings, the community name is simply not typed in. A postal community is a required entry when customers request to list a PO Box instead of a street address in the Directory. Communities are not permitted in Cross Reference (CR) business listings or in lines of captions that contain Alternate Call (AC) telephone numbers. NOTE: If the customer wishes to publish their Street Address, it is recommended that the community be published as well. In California, the community is required with a published street address. CAPTION LISTING REQUIREMENTS **For Hawaii Requirements see CAPTION LISTING REQUIREMENTS-HAWAII ONLY Caption directory listings consist of at least 3 lines and two telephone numbers. The first line, the Caption Header, may only contain the residence or business customer's name, plus an optional title and/or designation. An address nor telephone number can be included in a Caption Header line. At least two indented lines must appear under the Header, each containing a telephone number. NOTE: You can have up to six degrees of indention in a caption. The indents can increase only one indent at a time. To decrease, the levels can decrease in any increment. The header line is always 0 degrees of indention. Acceptable: 0 Header Name, 1 Address, 2 Ofc, 2 Res, 3 LI No calls after 10 pm Unacceptable: 0 Header Name, 1 Address, 3 Ofc,3 Res Increased indents must be sequential. The text in each indented line varies, depending on whether the line contains a Different Address (DA), Caption Information (CI), Line of Information (LI), Alternate Call (AC) phrase, or Dialing Instructions (DL). All of these options are described in greater detail in their separate sections. NOTE: With CA conversion to National Directory NOCV, there are embedded base caption listings with telephone numbers that do not have any type of preceding text. This is termed as 'leader dots'. In the actual printed directory, there are only 'dots' in the left hand column across from the telephone number. Ongoing, a telephone number in a caption listing should have some type of preceding text. Street addresses and community names appear in captions based on the customer's preference. The customer may want the address and community to appear only once in the caption listing, or he may want an address and community to appear on each indented line. Both options are acceptable. The Service Address (SA) can be derived to an indented line, but more than likely a Different Address (DA) will be entered. Omitting an address and/or community from a caption listing is easily done by just not entering them. Caption listings may contain all business telephone numbers, all residence telephone numbers, foreign telephone numbers, or a "mix" of telephone numbers. FORMAT REQUIREMENTS for caption listings: All new Caption listing arrangements must be established in the format described in the first paragraph of this section. Standardization is required to meet the requirements of both the Directory and Directory Assistance systems. Each sequence number within a caption arrangement denotes a separate line in the directory. Indention levels are determined by the customer. SUBCAPTION A subcaption is listing text, within a caption arrangement, that does not have an associated telephone number attached. Therefore, any time you have a subcaption, then the line immediately below must be indented one more than the previous subcaption. PROCEDURES for caption listings with INCORRECT FORMAT In some regions, there are imbedded directory listings that contain more than one telephone number, but they were not set up in correct Caption Listing format. The header lines of these two-line "captions" include an address and telephone number, and they usually have just one indented line under the first line. Notice the first address is also located on Line 100, the same as the header line, or caption name. The address should be located on its own sequence number and being the first line in the caption, it should have an indention level of 1. A header line should only include the following keys: CP, NN, TD, DE, and FA. Refer to separate section entitled DIALING INSTRUCTION PHRASES. Caption Information (CI) phrases enhance a caption listing because they indicate to the directory user where the call will be answered. There are no additional charges associated with CI phrases, and indented lines containing CI phrases transfer to yellow pages. A CI without a TN is called a subcaption. Subcaptions have other text or addresses associated. Those associated lines must be indented one more than the previous subcaption. CAPTION REFERENCES Whenever possible, the Caption Listing itself should be established on the Profile whose telephone number appears first in the listing. For all other telephone numbers in the caption that have separate Profiles, references to the Caption Listing must be established and maintained with the use of two Caption Reference entries. 1. MAIN CAPTION REFERENCE (MC) The MC (Main Caption) reference is a two-part entry that identifies the Profile on which the caption listing itself is located. The two NOCV Key Words used in this reference are as follows: KEY SEQ# THE LISTING INFORMATION ENTRY MC The Caption Header from the listing 100 TN 100 The telephone number on the Profile that contains the caption listing itself 2. NOTE: If the caption above were located in a Directory other than the customer's local one, the name of the other Directory should be included with the references. Therefore, the Directory Name would be added to the MC line after the Caption Header information, either following a virgule or in parentheses. Names of individuals are not permitted in AC phrases, and no street addresses or communities may appear in them. Additional monthly charges are associated with AC phrases. Alternate Call entries in Caption Listings only appear in the white pages of the printed Directory unless the customer contracts for a yellow page appearance. NOTE: The word "Or" can be used as an AC or a CI. Refer to USING THE WORD "Or" IN CAPTION LISTINGS for clarification of its usage. DIALING INSTRUCTION PHRASES Following is a partial list of standard Dialing Instruction (DL) phrases. Usage is optional in straight-line or caption listings, according to customer preference. NOTE: There is no charge associated with verbiage of "Toll Free-Dial '1' & Then" or "Toll Free". NOTE: A Dialing Instruction phrase is not required on Toll Free listings, but a note must accompany the listing to indicate that the DL was omitted per customer request. When used in captions, the DL phrase should be established on a separate line by using the next sequential sequence number and the indention level should be increased by one. Each phrase is usually followed by a TN Key Word. The ONLY Key Words that can be used on the same sequence number as the DL are the RJ and TN. NOTE: The embedded database listings are grandfathered. However, on a going forward basis the sequence number should be corrected as listings are addressed. EXAMPLES of DL phrases: There Is A Charge To Call This Number Ask For Extension 55 Press '2' After The Announcement Call 234-EYES (Vanity number) There are two options available for entering the "Toll Free" status of an 800 series number: 1. the familiar universal DL phrase: Toll Free-Dial '1' & Then.....888/234-1234 2. a shortened version in an RJ (Right Justify) phrase: …………………………….Toll Free 888/234-1234 (which will put the words "Toll Free" right next to the telephone number in the directory) Note regarding the Toll Free RJ phrase: In a Caption Listing, the RJ phrase cannot be on a line alone. It must be preceded by a Caption Information (CI) phrase or Address information. When used in a caption listing, the DL phrase should be assigned its own Line Sequence number and indented one more level than the preceding line. This is now the standard format; do not add the DL phrase to the same line as the CI phrase. Be consistent using Toll Free options within a caption listing. Do not switch from DL phrases to RJ phrases in the same listing. Discontinuation of 1+ entries in RJ phrases Because of the impact of NPA overlays and splits, 10-digit dialing, and Equal Access, the appearance of 1+ with a 7 or 10 digit telephone number to denote long distance calls has been discontinued. On a going forward basis, remove all RJ phrases with 1+ entries when encountered in NOCV. When applicable, replace the entry with an acceptable Dialing Instruction (DL) phrase. TOLL FREE TELEPHONE NUMBERS 800 numbers were the first "toll free" telephone numbers. Because of the increase in demand for 800 numbers, the supply began to reach the point of exhaustion. As a result, there are eight Toll Free numbers established: 800, 822, 833, 844, 855, 866, 877, and 888. Directory listings that use Toll Free numbers should include a Dialing Instruction (DL) phrase "Toll Free-Dial '1' & Then" or a Right-Justified (RJ) phrase "Toll Free." A retained remark (NT) must accompany the listing if the customer requests to omit the DL or RJ phrase associated with the Toll Free number. There are universal telephone numbers beginning with 900and 976- that are being used for marketing purposes. These are sometimes toll free but not always. Directory listings using these TN's should include a phrase telling the user whether or not charges apply for placing a call to the number. LINES OF INFORMATION A Line of Information (LI) contains descriptive or directional information for the purpose of facilitating the use of the directory. There is usually a monthly charge associated with LI's. In those states where a Line of Information IOSC is not available, the Additional Listing IOSC is to be used. Exceptions will be described later. Lines of Information in business listings do not transfer to the yellow pages. A straight-line or caption listing may contain one or more LI's. In a straight line listing the LI will appear in the directory on it's own line and be indented one level. In a caption, each LI should be established on a separate line with its own sequence number. LI's never include a TN entry. Acceptable Lines of Information include supplementary address information, hours of operation, mailing information, or Licensing numbers. LI's may not be used to advertise the services or products of the customer. NOTE: 'No Solicitation' is not an acceptable LI in Oregon. Refer to USE OF NAMES IN CI & LI ENTRIES for more details. Combinations of two addresses in a Residence or Business listing are acceptable as long as one address is charged as a Line of Information. Refer to ADDRESSES IN DIRECTORY LISTINGS for examples of combination addresses. LINES OF INFORMATION - TELEPHONE TEXT (TTY) SERVICE There is no charge for a Line of Information (LI) associated with Text Telephone (TTY) listings. The TTY identification is optional, according to customer preference. NOTE: You may see identifiers for this type of listing as TT and TDD, which still may appear in some listings. Going forward, TTY should be used. Lines of Information used to identify telecommunications devices for the hearing or speech impaired include, but are not limited to, the following: TTY only TTY & Voice TTY for the Hearing Impaired Text Telephone TTY for the Speech Impaired Voice/Text Telephone WHEN TO USE 'CI' vs 'LI' IN CAPTIONS Following are guidelines for determining whether a line of text in a Caption Listing should be established as a CI or LI entry. It still remains a judgment call by the person establishing the listing, but the important thing is to be consistent in establishing a customer's listing in multiple directories. Once the CI's and LI's have been established for a particular caption listing, they should be entered the same way in each Directory for consistency in both appearance and billing. CI (Caption Information) phrases: • enhance the listing and are critical to the clarity of the caption listing. They clarify where a call will be answered. Without the CI phrases, the caption listing would make no sense. • may or may not be a Telephone Number (TN) associated with them. • A CI without a TN is called a subcaption. • Subcaptions have other text or addresses associated. Those associated lines must be indented one more than the previous subcaption. • transfer to yellow pages. • have no monthly charges associated with them because they are critical to the clarity of the caption. LI (Line of Information) phrases: • contain "information" that is not critical to the clarity of the caption listing. LI's are "extra" information or "nice to know" information that could be omitted from the caption listing and nothing would be lost in quality or format. • must be on a line by themselves (may never include a TN entry in the same line). • do not transfer to yellow pages. • have an extra monthly charge associated with them. (Refer to Attachment IV: GLOSSARY OF DIRECTORY TERMS for definitions of Primary and Additional Listings.) The use of "Or" in the primary appearance of a telephone number requires a Caption Information (CI) phrase followed by a Telephone Number (TN) entry. In a business caption listing, this "Or" line will appear in both white pages and yellow pages. The second or additional appearance of a telephone number in a caption listing that appears in the same directory requires the use of an Alternate Call phrase (AC), which has it's own IOSC and additional monthly charges associated with it. In a business caption, this "Or" line does not transfer to the yellow pages. GOVERNMENT/911 LISTING REQUIREMENTS Government listings, both straight-lines and captions, appear in the white pages of a Directory or in the blue page Government section, if the Directory has one. A Government caption listing for a city, county, state, and federal office is required to use Upper Case letters on the Caption Header, First Indent and the word "Emergency". The blue pages are divided into five Government categories: City (Village & Township), County, State, United States, and Public Schools. If the pages are maintained by issuing service orders, a unique FL code found in EZR is associated with each category. The purpose of the FL code is to file the listing in the appropriate category in the blue page section of the correct Directory. (Refer to EZR>>Reference Documents>>DSC - Job Aids and Procedures>>Government Location Codes (Vz West) GOVERNMENT LISTINGS-HAWAII REQUIREMENTS All Published Government listings that are to appear under County, State or United States government must be established within the existing caption listings for each islands’ respective directory. There are no straight line listings for each branch of government. Each island has a section for each branch of government. There is no requirement to enter an FL code on the listing because each branch of government is set up on a fictitious account. "911" NUMBERS IN CAPTION LISTINGS The most frequent appearance of "911" is in Government captions for Fire, Police, Rescue Squads, and Sheriff departments. "911" numbers are non-standard telephone numbers because they have less than 10 digits. In order to be found in NOCV, all "911" listings must be in caption format along with at least one standard 10-digit telephone number on whose Profile the caption listing can be established. The other telephone number is usually a local non-emergency number for the Fire, Police, Rescue Squad, or Sheriff. The other telephone number is usually a local number. Below is a list of some "N11 numbers. NOTE: This list is not all-inclusive. "211" For individuals facing serious threats to life, health, and mental well being that are not addressed by dialing 911. "311" For non-emergency police assistance. "411" Information "511" Nationwide access to travel information services. "711" Access to TTY "911" Emergency EXAMPLE of a caption with a N11 number (located on the Profile for 813/123-4567) IND SEQ# KEY LISTING INFORMATION 0 100 CP United, Way 211 1 150 CI Non-Emergency 150 TN 211 200 AC Or 200 TN 813/123-4567 2 FOREIGN LISTINGS ILEC: A Foreign Listing (FL) is any directory listing that is not local to a particular ILEC directory, therefore the telephone number is actually foreign to that particular directory. CLEC: A Foreign CLEC Listing is determined by service address, therefore the service address is actually foreign to that particular directory. Foreign listings have associated monthly (or annual) charges that apply regardless of whether or not they appear in a main Directory or in a Neighborhood directory. The following kinds of telephone numbers are "foreign" in all telephone directories: • 800-, 822-, 833-, 844-, 855-, 866-, 877-, 888- (toll free numbers) • 900- or 976- numbers (may be toll free or not) • Cellular, Pager, Mobile, and Voice Messenger numbers • Non-standard numbers like those listed below that do not conform to the 10 digit telephone number format 112, 114, 611, 411, 1411, 911, JL5, JR2, JT4, YJ3, YK2, YP8, YR5 NOTE: Non-standard Commerce, Enterprise and Zenith numbers are obsolete. Embedded listings containing these numbers should be referred to the Directory Support Center. Another required entry associated with Foreign Listings is the name of the Directory in which the listing appears. Foreign Listings require the entry of a Foreign Listing (FL) code, which sends the listing to the appropriate files at Directory Assistance and to the appropriate section of the Directory, and also the entry of the Directory name and state. The FL code entry consists of either 5 digits (EX: 30245) or an alpha letter plus 4 digits (EX: H4775). FL codes are required on all types of Foreign Listings, on CLEC Listings (refer to CLEC "FOREIGN" LISTINGS for more information), and on Government listings (refer to GOVERNMENT CAPTION LISTINGS for more information). An example of a complete FL code would be: B8836 Warsaw VA In NOCV, FL codes are required (or not required) for caption listings based on the following three criteria: • what kind of TN's ("local," "foreign," or both) appear in the caption, • what kind of Profile (local or fictitious) on which the caption listing itself is located. • what kind of TN (local or fictitious) is the first TN in the caption listing. An FL code entry is required when: • the caption listing is located on a BFIC/RFIC "true" foreign Profile. • it contains all "foreign" TN's, or a mix of "local" and "foreign" TN's. • the caption listing is located on a foreign Customer Profile. • it contains TN's that are all "foreign" to the Directory in which the listing will appear. • or it contains a mix of "local" and "foreign" TN's and the first TN in the caption is "foreign." • the caption listing is located on a “local” Customer Profile and the first TN in the caption is “foreign” to that directory. An FL code entry is not required when: • it contains a mix of "local" and "foreign" TN's and the first TN in the caption is "local." WHEN TO ESTABLISH A BFIC/RFIC ACCOUNT: • If there is a local profile with a combined Toll Free number, the Toll Free listing and associated charge may be established on the local account, or the customer may request those charges on a separate bill. If a separate bill is requested, a BFIC/RFIC account would need to be established for billing. • If a customer has a local account and requests that we list his Cellular or Pager number, a BFIC/RFIC account should be established, unless the telephone number is established with the local number in a caption arrangement. • A "true" foreign listing customer requests that we list his telephone number in one of our directories. DIRECTORY SCOPING (applies to California/Florida only) Directory Scoping in California/Florida refers to multiple directory appearances (scoping) of listings based on an ACO and the Customer's Service Community. Based on this, a Business or Residence directory listing could appear in as many as four directories. For example, a Customer who lives in the community of Bellflower, would be entitled to a 'P' (Primary) directory appearance in the Downey directory, as well as 'B' (Book) appearances in the LB/Lakewood/Cerritos, Compton, and Long Beach directories. Scoping is designed to expand white page coverage for communities with common interests. Refer to EZ Reference for detailed lists of ACO/Community combinations and their applicable scoping. Refer to EZR>>Reference Documents>>DSC - Job Aids & Procedures>>California Directory Scoping (Vz West) Refer to EZR>> Reference Documents>>DSC - Job Aids & Procedures>>Florida Scoping Table (Vz West) California Foreign Exchange (FX) listings are entitled to scoping. Foreign listings and Multi-Book Listings are not entitled to scoping. MULTI-BOOK LISTINGS Any appearance of our customer's listing in another of our directories other than its local book is referred to as a MultiBook Listing (ML); applicable charges apply. NOTE: It is now required that a directory listing has to be first established in its local Directory before it can appear in another directory as a Foreign Listing. There are certain customers who have telephone service in one of our exchanges, but their service address (postal boundary) is that of an adjacent exchange of ours. If listings for the two exchanges appear in separate directories, the customer is entitled to a free Multi-book Listing (ML) in both directories for each Primary Listing (PL). POSTAL ADDRESS LISTINGS (PALs) (applies to Illinois only) PAL's (Postal Address Listings) apply to Illinois customers who have telephone service in one exchange but the Community (CM) in their directory listing is one that is scoped to a different directory. State tariffs entitle the customer to a free appearance of his Primary Listing (PL) in the other directory. The offering involves all Illinois telephone companies and all Illinois directories, not just ours. Procedure for Handling Requests Should an Illinois customer indicate he is not listed in a particular directory and should be, notify the Directory Support Center (DSC) by fax or memo, and include the following information: • Customer's Name, Address, and TN (including NPA) • Statement requesting the PAL be added, removed, or hanged, and specify in which Directory (and whether it is ours or another company's directory • Originator's Name, Office Location, 10-digit CBR number DUAL DIRECTORY LISTINGS (applies to Washington only) A Dual Directory Listing is a free listing for a residence or business customer, included in the alphabetical section of the directory that covers the community shown in the customer's directory listing. The purpose of the Dual Directory Listing is to provide improved directory listing service and Directory Assistance for specific fringe area communities in adjoining directory areas. Contact Centers are responsible for establishing Dual Directory Listings. Washington State tariffs entitle these customers to free Dual Listings. This offering applies to communities in the directories of Eastside, Snohomish County, and Snoking. NEIGHBORHOOD DIRECTORIES A Neighborhood Directory (NBHD) consists of alphabetical white pages and classified yellow pages that contain preselected "neighborhood" areas. The NBHD book is generally smaller in size than the Main or Core Directory from which specific exchanges, prefixes, and/or listings are extracted. Customers who reside in a "neighborhood" area receive a copy of the Main or Core Directory as well as a copy of the NBHD Directory. There is no extra charge to have a listing in both books if a customer is "entitled" to an appearance in both. The following states currently have Neighborhood (NBHD) publications: California Indiana Florida Michigan Hawaii Oregon Illinois Pennsylvania NOTE: The Close Date for the Core/Main directory matches the Close Date of the NBHD book in all states except Florida. Florida NBHD books close 4 or 5 months after the Main or Core directory, which requires a separate Close process. Foreign Listings may appear in NBHD directories with associated monthly charges. They are established in NOCV like any other Foreign Listing (FL) except: • Selection Control 5 is required on the listing. This allows the Foreign Listing to print in the NBHD directory and prevents it from appearing in the Core/Main directory. A customer must pay for two FL's if he wants a Foreign Listing in both the Core and NBHD books. • The FL Code entry must be the one associated with the NBHD book. SELECTION CONTROL Selection Control (SC) options do exactly what the name implies; they actually "control" where a listing appears. Only one Selection Control option can be assigned per listing (straight-line or caption). There are eight (8) options available. Most commonly used today are Selection Controls 1, 5, 6, and 7. Although you may encounter in the embedded base or under special circumstances Selection Controls 2, 3, and 4. SC OPTION WHERE THE LISTING APPEARS 1 in the local Directory 2 in local D.A. (Directory Assistance) 3 in the local Directory & local D.A. 4 in Long Distance D.A. 5 Directory only in the Neighborhood (NBHD) 6 in both Local & Long Distance D.A. 7 in the Main/Core Directory, Local D.A.,and Long Distance D.A. (not in a NBHD book) D a listing used to delete an existing SC option from If no special code were entered on a listing, it will automatically appear in both local and long distance D.A. as well as print in the local directory. Today, all of the above codes may be encountered in the embedded listing database, but the only Selection Control requirements left are as follows: Use 7 on Caption Listings that are to appear in a Main/Core Directory. • if it contains a mix of "local" and "foreign" telephone numbers and some of the local TN's are entitled to an appearance in a NBHD book. • If it consists entirely of "foreign" TN's. Use 7 when the FL Code entered with a Foreign Listing would send it to a NBHD book but the listing contains a TN that is not entitled to appear in appear in the NBHD without additional charges. • This applies to both straight-line and caption Foreign Listings. • Customers can pay additional charges to have a Foreign Listing appear in a NBHD book. Use 5 on directory listings that are established specifically for a NBHD book. • when creating straight-line or mini-caption listings for "local" TN's located within the Selection Control 7 Caption Listing referenced above. NOTE: There are no charges to customers for listings created for NBHD books. Straight-line listings should be created as AL's. Use 5 if a Foreign Listing customer wants his listing to appear in a NBHD directory, and be sure the listing meets the following requirement. • The FL Code must be the one that is associated with the NBHD book. PROCEDURE: Based on the few remaining requirements for Selection Control, when Contact Center or DSC employees encounter SC options on listings that do not require them, they should issue an order to remove them. If Selection Control 7's are removed, the order writer must also delete any associated Selection Control 5 listings that are no longer needed. Northwest Fringe Area DIRECTORY ASSISTANCE SELECTION CONTROL REQUIREMENTS (The following situation applies only to Directory Assistance in Oregon) Use a Selection Control 1 and 6 combination on listings to fulfill an Oregon State Commission requirement. SENTENCE STRUCTURE-HAWAII ONLY An account’s listing category may contain a combination of straight line and caption listings. All caption listing data, including the telephone numbers must be entered in the Directory List-Caption (DIRC) screen. Caption Listing sentences require four basic elements: Caption Listing Key ( CP = major key) Sequence Number (100, 200, 300, etc) Indention Instruction Descriptor) (IN0 – IN6) Directory Listing Information In addition, Account Types of Bus, Prof, Govt and Caption Control Number are also required. SEQUENCE NUMBERS-HAWAII ONLY Main Caption Account: A sequence number is a three-to-five digit number that controls the sequence in which a caption appears in the directory. The sequence number 100 is assigned to the initial main caption header. Sequence numbers assigned to the main caption headers on multiple caption listings are in increments of 1000, with the last three digits being 100 (e.g. 1100, 2100, 3100, 4100, etc). Subordinate Caption Account: Each Subordinate Caption Account is usually assigned a sequence number of 100 digits higher than its associated caption listing set. Line of Information (LI): Line of information is usually assigned a sequence number of two digits higher than its associated listing. This is to allow multiple insertion of LI should the need arise. Classified Listing Heading (CH) Classified Listing Heading is usually assigned a sequence number of 100 digits higher than its associated listing (line containing the telephone number). Control Number The caption Control Number uniquely identifies each caption listing set. The control number is obtained from NOCV, and is entered in all brand new caption listing set. A control number is assigned per telephone directory caption listing set. For instance, a customer with Oahu, Leeward, and Windward caption listings will be assigned a total of three control number: one per directory. CP (Caption Listing ID Major Key) The CP major key is used to distinguish a caption listing from a straight-line listing. An account may have multiple CP entries, same or different names from the service name. For example, the primary listing and the additional listing are set up as caption. IN (Indention Level) The IN level indicates in the directory the number of positions a caption listing is to be indented from the left margin. For example, the caption header is not indented from the left margin and is assigned an indent level of zero (0). The Subcaption lines are indented under the main caption header, and are assigned indent values ranging from 1 to 6. An indention level is required for each listing group, but only one per listing group is valid. Indention levels are not valid for the CH, NT, or XX keys. SC7 (SELECTION CONTROL 7)-HAWAII ONLY Directory's VAST system extracts caption listing data based on the location code of the main caption account's profile. Whenever the prefix (NXX) of the main caption is one that is in the neighborhood area, an SC7 is entered on the main Oahu caption. The SC7 is entered in the DIRC screen, Selection Control field in the Options category. **The purpose of SC7 is to prevent auto-duplication entry of the entire Oahu caption listing set in a neighborhood directory. For example: Main caption prefix: Subcaption #s prefix: 456 247 732 373 (Leeward prefix) (Windward prefix) (East Honolulu prefix) (East Honolulu prefix) Without the SC7 entry on the main caption account (Leeward), the Windward and East Honolulu numbers will be printed erroneously in the Leeward directory at no charge. Only business numbers that belong to the neighborhood directory will be listed in the neighborhood directory at no additional cost. So, if the above customer requests all numbers be listed in the Leeward neighborhood directory, (3) business additional charges of $0.90 will apply: (1) for the Windward prefix, and (2) for the East Honolulu prefixes. If the same customer wants to list the identical Oahu caption listing in the East Honolulu directory, then (2) business additional listing charges will apply: (1) for the Leeward prefix, and (1) for the Windward prefix. EXCEPTION TO THE RULE: The above rule would be superseded if the listing is or consist of a legitimate personal name listing, AND the last name (not the first name), is the finding word. This is entered in the DIRC screen, Selection Control field in Options. It is also used to delete an SC1 entry for straight line listings. CI (CAPTION INFORMATION)-HAWAII ONLY Caption Information (CI) enhances the caption listing and is critical for the effectiveness of the listing. Because they are entries for the clarity of the caption, no monthly recurring charges are associated with it. LI (LINE OF INFORMATION)-HAWAII ONLY A Line of Information (LI) is used to enter descriptive or instructional information that assists the directory user. It may not be used to advertise the services or products of the customer. An LI requires a sequence number; and it appears on a line by itself in the printed directory without a telephone number. A caption listing set can have multiple LIs, and are listed in the white pages only for a monthly recurring charge. Customers requesting to print LIs in the yellow pages are referred to Super Pages. For purposes of consistency, though, follow these four standards in regard to establishing Military Titles: 1. If a customer is retired from the military, the abbreviation "Ret" may be usedfollowing any title. EXAMPLES: Maj Ret Gen Ret LtCol Ret MajGen Ret 2. If the military title starts with the words "First or Second" and the customer indicates no preference for the words, use the numeric "1" or "2" as a prefix on the abbreviated title (omitting spaces). EXAMPLES: First Lieutenant = 1Lt First Sergeant = 1Sgt Second Lieutenant = 2Lt 3. If the military title ends with a numeric indicator, such as First Class, Second Class, or the numerics 1,2,3,4,5,etc., and the customer indicates no preference for the letters FC or SC, use a numeric suffix on the abbreviated title (omitting spaces). EXAMPLES: Sergeant First Class = Sgt1 Private First Class = Pvt1 Chief Warrant Officer 2 = CWO2 Petty Officer 3 = PO3 Specialist 4 = Spec4 4. If the title consists of a combination of two military titles, combine the abbreviations for each title (omitting spaces).
