Be Our Voice is a project of the National Initiative for Children’s Healthcare Quality (NICHQ) along with the American Academy of Pediatrics, the California Medical Association Foundation and the RWJF Center to Prevent Childhood Obesity. The goal of Be Our Voice is to reverse the childhood obesity epidemic trend across the nation by training, supporting and providing technical assistance to healthcare professionals in becoming advocates for change within their communities. Be Our Voice encourages the healthcare provider to focus on promoting change within the 5-2-1-0 Framework: 5- servings of fruits and vegetables per day; 2- limit screen time to 2 hours or less; 1- hour of physical activity per day; 0- limit sugar-sweetened beverages; BF- promote breast feeding; and BMI- screen patients for elevated BMI. While this directory focuses on encouraging physical activity, it is recognized that other lifestyle changes are critically important to prevent obesity and optimize health. All healthcare providers are encouraged to become involved in this important initiative. More information is available at: http://www.nichq.org/advocacy. 1 INTRODUCTION Obesity is a critically important public health problem in the U.S. Nationally, 35.7% of U.S. adults are obese (1). Obesity frequently starts in childhood. In 2010 in the U.S., 12.1% of children ages 2-5 years were obese, rising to 18.0% of 6-11 year olds and 18.4% of 12-19 year olds(2). Texas ranks 12th in the nation for its obesity rate in adults (30.1%) and 7th in the nation for its obesity rate in children and adolescents (20.4%) (3). Healthcare providers for children in San Antonio face this problem every day. According to FITNESSGRAM data for students grades 3-12 in Bexar County, 30% of students were found to have an unhealthy weight(4). Increasing physical activity, along with dietary modification, is essential to fighting childhood obesity (5). In addition to decreasing body fat, physical activity has been shown to have other important health benefits for children, including improvements in HDL-C and triglyceride levels, blood pressure in mild essential hypertension, bone density, symptoms of anxiety and depression, concentration, memory and classroom behavior (6,7). Adults also greatly benefit from physical activity. Documented health benefits include decreased rates of obesity, cardiovascular disease, type 2 diabetes, cancer, osteoporosis, and depression and improved cognition and sleep (8,9). The 2008 Physical Activity Guidelines for Americans were published by the U.S. Department of Health and Human Services (8). These guidelines recommend 60 minutes or more of physical activity for children and adolescents, with most of the time spent in either moderate or vigorous intensity aerobic activity. Adults should do at least 150 minutes per week of moderate-intensity or 75 minutes per week of vigorous intensity aerobic physical activity throughout the week. Unfortunately, most Americans are not following these guidelines. According to statistics from the American Heart Association (10), only 37% of 9th graders and 32% of 12th graders reported 60 minutes of physical activity for 5 or more days of the week. Using accelerometer counts, only 42% of 6-11 year olds performed 60 minutes of moderate-to-vigorous physical activity on 5 of 7 days studied. Of adults studied, 33% reported no leisure-time physical activity. Only 47% of adults report meeting the 2008 Physical Activity Guidelines. It is the goal of this directory to equip healthcare providers in San Antonio to provide effective and specific anticipatory guidance to their patients and families about physical activity. This directory focuses on those opportunities for physical activity that are free or low cost. It does not include the multitude of state-of-the-art gyms and health clubs that are available. It also does not include many of the sports leagues and activities offered by religious organizations in San Antonio. These groups provide outstanding opportunities for children and adults, but at a cost that many families find prohibitive. Many of these groups do have financial assistance available for these programs, so interested families should inquire directly. The first part of this directory contains general information about some important resources related to physical activity in San Antonio. This is followed by zipcode- specific lists which could be copied and given to the family. It is hoped that families in San Antonio will take advantage of the fantastic parks and other resources available throughout our city, but we are advocating daily physical activity for our patients. Realistically, this means that the resource must be in close proximity to where the families live. There are a number of zipcodes with no listing. The general handout on pages 19 and 20 can be given to these families. Alternatively, a handout on a nearby zipcode can be used. Everyone can benefit from physical activity, but it must be done in a safe manner. Children require ongoing health supervision by their healthcare provider. Adults who are embarking on a new physical activity program should have clearance by their healthcare provider. Anyone who develops discomfort or other symptoms while exercising should seek prompt medical attention. In addition, we must all pay attention to our surroundings and 2 take appropriate measures to ensure our physical safety. Given these precautions, there are an amazing number of wonderful opportunities to be active in this beautiful city. SAN ANTONIO PARKS AND RECREATION DEPARTMENT- FITNESS IN THE PARKS Outdoor fitness stations have been installed at 19 city parks. The installation was made possible by an Americans Recovery and Reinvestment Act (ARRA) funded grant entitled Communities Putting Prevention to Work (CPPW). Use of the equipment is free. SAN ANTONIO PARKS AND RECREATION GREENWAY TRAILS Linear Park Development Program: 34 miles of developed Greenway Trails are now open to the public. This includes trails along Salado Creek, Leon Creek and the Medina River. These parks were funded through sales tax approved by San Antonio voters in 2000 and 2005. These beautiful trails are perfect for hiking, jogging and biking. Maps and information about trailheads are available at the above website. Mission Reach will be an 8 mile linear park along the San Antonio River. Parts of this park are open now. It is scheduled for completion in 2013. There will be a pedestrian gateway from Mission County Park from the river to Mission San Jose. The H-E-B Body Adventure will replace the H-E-B Science Treehouse exhibits. The goal of this exhibit is to empower visitors to optimize their own health empowering them to “make healthy choices, move your body and relax your mind.” There will be over 30 interactive visitor experiences, including 13 which are linked to a Power Pass card system. Power Pass System: visitors will sign in and select a buddy who will act as their learning companion throughout the exhibit. Twelve people from South Texas volunteered to act as buddies and went through an eight week training and education program lead by various community leaders in health and wellness, including the Culinary Institute, The Synergy Studio and the San Antonio Botanical Garden. Videos of the buddies will be displayed to the museum visitors as they go through the exhibit, telling personal and encouraging messages, helping to interpret the exhibit experiences, and providing feedback. At the Power Pass sign in area, the visitor will enter their first name, age and sex. They will receive an ID card the size of a credit card with their information and their name encoded on it. This card will be used to activate the 13 Power Pass components throughout the building. At the time of exiting the exhibit, the visitors will sign out and receive a personalized Power Profile printout that will include a preprinted reminder message on one side along with a personalized printout of exhibit experiences on the other side. Healthy food, EKG, and stride length are some of the personalized information that will be included. The H-E-B Body Adventure is designed to be a national model, building on the Museums and Gardens Move-It program lead by Michelle Obama, with components including: 1. Move It! This interactive “dance floor” was the first component of the exhibition to be installed in 2009, it was part of a larger study to see how visitors reacted to health and wellness messages. It will be later updated with an infrared camera that shows the difference in body heat before and after use of the floor called What Color Is Heat? 2. Mount Witte: The iconic climbing tower was the next phase of the exhibition, installed in 2010. With a base camp and stretching and muscle information, this helps visitors think about both exercise and body self-awareness. 3. H-E-B Body Adventure Container Gardens and Geocaching: In the fall of 2011, container gardens that showcase vegetables, herbs and medicinal plants along with both low tech and high tech walking courses were opened. Healthy vending machines were also installed at this time. 11 4. South Texas Trailblaze: visitors will use a stationary bicycle to cycle through a South Texas landscape featuring an on-screen avatar who races against other visitors. The visitor’s heart rate will be displayed along with information about achieving a target heart rate and messages from his or her buddy. 5. See Your Own Heartbeat: visitors will grip handlebars to see their own EKG, next to a display with two plastinated heart specimens explaining why Your Heart Never Sleeps. 6. Pick Up Your Pace: a video image will be captured of the visitors moving across a 12 foot walkway as energetically as possible and an energy score will be generated from the speed and length of the stride. An investigation station area will allow them to find their stride on a screen with other visitors’ strides, including mystery guests like San Antonio Spurs members. 7. Build a Balanced Meal and Clean Out My Kitchen Games: two computer stations will engage visitors with nutrition messages, including healthy choices with whole grains and lean meats and easy substitutions to less healthy traditional South Texas favorites. 8. Supersize Challenge: visitors use a step machine alongside a giant-sized soda cup. They will learn how long it would take them to burn off the calories from a beverage selected from a variety of supersized portions of sugar added drinks One hand crank machine will be available as well. 9. Relax to Win: visitors will pair up and wear headbands with electrodes that measure alpha and theta waves which are produced by a calm, relaxed state of mind. 10. Relax Your Brain: a station where visitors will utilize biofeedback to practice relaxation. 11. Demonstration Kitchen for live demonstrations and video cooking segments. 12. Toddler Garden: for children 3 years and younger with a vegetable and fruit themed fantasy landscape. This includes an obstacle course where children can crawl through a giant apple and slide down a celery stalk. 13. The child must have at least one risk factor: obesity, heart condition, diabetes, acanthosis nigricans, high cholesterol, or hypertension. This is a 12 week program offered three times per year, with sessions beginning in January, May and September. They typically take 60 children per session and there is a wait list. The program meets three times per week on Mondays, Wednesdays and Thursdays from 5:30-7:15 PM. Each session includes 45 minutes of nutrition education and 45 minutes of exercise. Participants are screened before and after the program for cholesterol, glucose, weight, BMI, fat %, fat mass, muscle mass and blood pressure. The program includes a grocery store tour with instruction on reading food labels as well as 3 cooking demonstrations.
