THE OWNER OF THE BITCH AGREES TO:
(1) Pay all fees up front. No fee will be refunded in whole or part. Stud fee to be paid in USA dollars by cash, money order
or cashier’s check payable to Linda Bell.
(2) Provide the stud owner with: a complete 5 generation pedigree of bitch; DNA certificate if available; Hips and/or
elbows status (OFA or Penn Hip) and Eye (CERF) verification; picture or video of the bitch, pertinent breeding & progeny
information, height, weight, tail, DOB, bite and dentition information. (Fill in section below)
(3) Keep the bitch confined and assure that she is not mated with any other dog.
(4) Notify stud owner after bitch whelps.
(5) Bitch Owner agrees that at Bitch Owner’s expense, the litter produced from this mating will have their eyes cleared by a
certified Ophthalmologist between 7 and 12 weeks of age.

THIS STUD CONTRACT, EXECUTED IN DUPLICATE, IS IN FULL FORCE AND EFFECT WHEN SIGNED BY
ALL PARTIES HERETO. STUD OWNER CERTIFIES THAT THE ABOVE GENERAL INFORMATION ON THE
STUD IS TRUE AND CORRECT. BITCH OWNER CERTIFIES THAT THE ABOVE INFORMATION ON THE
BITCH IS TRUE AND CORRECT. STUD OWNER MAY REFUSE ANY BITCH FOR ANY REASON.
FROZEN SPERM: Artificial Insemination by frozen sperm.
RECOMMENDED.
INTRAUTERINE INSEMINATION IS
A Non-refundable $____________STUD FEE per draw of ___4____straws, (payable to Linda Bell). Additional fees for
withdrawal and shipping of the sperm are paid directly to the sperm bank with the BITCH OWNER’S credit card and are the
BITCH OWNER’S responsibility. BITCH OWNER is responsible for the BITCH’s costs in testing and insemination, transport
or semen, and coordination of return of any shipping containers to the sperm bank. Upon completion of the Stud Contract and
remittance and clearance of funds, the STUD OWNER will complete the Release Form for frozen sperm and send it by fax or
regular mail to the Semen Bank:

The BITCH OWNER may then directly contact the Semen Bank to direct them when to ship the frozen sperm at the BITCH
OWNER’S EXPENSE. At that time, the Semen Bank will use the BITCH OWNER’S credit card to cover the expenses
involved in withdrawing and transporting the semen plus any tank rental charges for sperm storage in transporting or storing at
the BITCH OWNER’S vet’s office. BITCH OWNER is advised that the Semen Bank needs a minimum of three days notice
for shipping and may need additional notice for shipping to a foreign country. Bitch owner is advised to speak directly to a
representative at the Semen Bank to insure timely withdrawal and shipment of sperm. STUD OWNER does not take any
responsibility for shipment of the sperm. If the sperm is not withdrawn within one year of the date of the execution of this
STUD CONTRACT, this STUD CONTRACT is terminated and considered null and void and all fees are retained by the STUD
OWNER. The sperm is specifically to be used by the BITCH named in this contract, or no registration papers will be signed by
the STUD OWNER. STUD OWNER cannot guarantee the quality of the sperm once it has left the Semen Bank.
Bitch owner shall hold Winslide Stockdogs, its owners and employees harmless from any and all lawsuits or potential lawsuits
involving their bitch.
We hereby signify agreement to the terms and provisions prescribed above. By signature below, all parties named herein, their
heirs, or assigns agree to and are bound by the terms as set forth above. Should legal action be necessary to enforce any terms
of this Stud Contract, suit shall be filed in the County and State of Stud Owner’s residence.
