Reviewing objectives and performance is especially critical during the employee’s first year at Ryerson. The review provides an opportunity to compare overall results obtained to overall performance expected during this first year. For MaC employees this review does not replace the formal performance appraisal required on an employee’s anniversary date. Page 1 of 13 Transfer from Probationary to Regular Staff Form INSTRUCTIONS Completing the Evaluation Form The Transfer from Probationary to Regular Staff form contains three sections: SECTION A contains 9 factors against which the employee's performance should be assessed. Review each factor carefully and assess the employee's performance in relation to the descriptive information. Comment in the space provided. Note: You may omit the Leadership factor if it is not a requirement of the employee's job. If the employee is non-supervisory, but still exercises some degree of leadership in his/her position, this factor should be included in the evaluation. SECTION B deals with developmental plans for the employee. 1. Identify and list the employee's principal strengths. 2 Identify and list principal areas for improvement and development. 3. Outline the agreed upon course of action, and plan for implementation. SECTION C is provided for summary comments and signatures. Page 2 of 13 Transfer from Probationary to Regular Staff Form FACTOR: Job Knowledge Consider the employee’s knowledge of the job responsibilities and tasks. Does the employee understand how the job fits into the larger organization? Consider also technical knowledge under this factor; is the employee familiar with the equipment he/she is required to use, policies, procedures, etc. FACTOR: Quality of Work Does the employee strive to complete work thoroughly and accurately, correcting errors by self-checking. Is preparatory work to the main task carried out in a complete manner? Consider the quality of the final product. Page 3 of 13 Transfer from Probationary to Regular Staff Form FACTOR: Quantity of Work Consider the employee’s ability to keep up with the normal pace of work. Is he/she able to adjust to fluctuations in workflow? Can the employee handle a substantial work volume or a number of assignments simultaneously? FACTOR: Initiative/Work Commitment Under this factor, please comment on the effort, perseverance and willingness the employee exhibits. Is the employee resourceful in finding solutions to problems, actively seeking ways of doing things better? Consider the amount of supervision required and initiative shown to learn new skills and assist others. Are the work plans realistic given time and resources available? Is the employee able to plan his/her work effectively, and address the degree of supervision required to do so? Consider the new methods or procedures that the employee may develop to meet changing conditions. FACTOR: Dependability/Reliability Comment on the degree to which the employee can be counted on to complete tasks/assignments in a timely manner; does the employee meet deadlines? Consider follow-up systems. An important part of this factor is punctuality/attendance; does the employee attend at work regularly and does he/she arrive at work on time? Page 5 of 13 Transfer from Probationary to Regular Staff Form FACTOR: Interpersonal Relationships/Human Relations How does the employee interact with others: clients, peers, supervisors? Is the employee cooperative and helpful and recognized as a positive element in the work environment? Does the employee accept direction and criticism? Are controversial or delicate matters handled skillfully? FACTOR: Judgment Consider the necessity and degree to which the employee must use judgment to foresee and evaluate the impact of decisions, the use of common sense. Comment on the employee’s ability to think clearly, calmly and positively under stress. Are staff developed and criticism used in a constructive and instructive manner? Are staff given required scope and credit for good performance? Are rules applied consistently? Do others respect and seek the employee’s guidance and counsel? If applicable, are subordinates’ performance appraisals completed regularly?
