The following document is both the Report and the Agenda. The liaison visit needs to be undertaken week 7 which is mid way through the students placement. The report needs to be completed both during and after the visit. The report is to be submitted to the Manager of Field Education, at the University within 1 week following the liaison visit. You may also cc the supervisor to make them aware of your role. Comments Introduce yourself to your student How are things going on placement generally? Has the student settled in? Student Mid – Placement Presentation The student is required to make a 30 minute (maximum) oral presentation and should address the requirements listed in the first column. The University liaison person is required to document progress against each requirement in the 2nd column and assess whether the student’s progress is satisfactory.
