CHILDREN’S SERVICES ACROSS NHS GREATER GLASGOW
AND CLYDE
The Yorkhill Campus hosts the Royal Hospital for Sick Children and the Queen
Mother’s Maternity Hospital. Located outwith the two main hospitals, but on campus
grounds, is a full facility of on-site laboratories.
A description of the two main hospitals is provided below.
a)
The Royal Hospital for Sick Children, Glasgow
The Royal Hospital for Sick Children, Glasgow, is one of the largest paediatric
teaching hospitals in the UK and the largest in Scotland. It provides
secondary care for more than 900,000 people resident within the Greater
Glasgow and Clyde area, but also tertiary care for the 3m people living across
the West of Scotland. There are also 17 nationally designated services
delivered from the hospital including cardiac surgery, bone marrow and renal
transplantation, ECLS and complex airways.
There is a complement of up to 279 beds, which include 23 integrated
neonatal medical and surgery cots. There are 16 nationally funded intensive
care beds and 6 high dependency beds within an integrated critical care
facility. This facility has the physical capacity to extend to 26 integrated critical
care beds. A new theatre complex was opened in 1998 comprising of 7 full
theatres, scope room and dental suite. The ED currently sees just under
40,000 patients per year.
All paediatric medical and surgical subspecialties are represented, including
general medical paediatrics, cardiology, neonatology, neurology, nephrology,
respiratory, endocrinology, gastroenterology, immunology and infectious
diseases, dermatology, haematology/oncology, rheumatology, metabolic
medicine, audiology, ophthalmology, ENT surgery, orthopaedics and general
paediatric and neonatal surgery. A selection of child and adolescent
psychiatry facilities are located within the campus along with a recently
opened Child Protection Unit.
The hospital provides the major Undergraduate Paediatric Teaching facility
for the University of Glasgow and accommodates the University Departments
of Child Health, Child and Family Psychiatry, Medical Genetics, Human
Nutrition, Paediatric Pathology, Paediatric Biochemistry and Paediatric
Surgery. There is on site clinical audit and research and development support
to assist with departmental research projects.
The Radiology Department located within RHSC provides ultrasound, CT,
MRI and isotopic studies on site. A new national PACS system is being rolled
out due for completion at the end of 2008. All imaging is now film-less.
Diagnostic laboratory facilities on site include Haematology, Blood Banking,
Biochemistry, Microbiology, Virology, Histopathology and Genetics.
b)
Other Paediatric services in NHSGG&C
There are three other maternity hospitals in Glasgow; Princess Royal
Maternity Hospital, Southern General Hospital and the Royal Alexandra
Hospital, all with neonatal intensive care facilities.
Paediatric neurosurgery is located in the Institute of Neurosciences at the
Southern General Hospital.
There is one children’s ward at the Royal Alexandra Hospital providing local
care for the children of Paisley.
There is an extensive range of specialist community based children’s services
across NHS Greater Glasgow. Managed within community health and social care
partnerships, these services are integrated with primary care and social care
services.
c)
New Children’s Hospital
In order to provide child, maternal and adult services on a single site a new
children’s hospital is currently being commissioned on the existing Southern
General Hospitals campus with an anticipated opening date of calendar year
2015. Maternity services in the QMH will be re-provided across SGMH and
PRM units from 2009 / 2010.
2.
WORK OF THE DEPARTMENT
The otolaryngology department undertakes the full range of paediatric ear, nose and
throat services, including a National Services Division-designated national service
for complex airway reconstruction and a supra-regional referral service for airway
disorders in general. The only service not provided on site is cochlear implantation,
which is provided as a national service at Crosshouse Hospital in Kilmarnock.
Yorkhill is the main tertiary referral centre for paediatric airway disorders for
Scotland.
All airway conditions are dealt with including choanal atresia,
laryngomalacia, subglottic stenosis, recurrent respiratory papillomatosis and tracheal
pathology. This work is made possible by the support of a large paediatric intensive
care unit, skilled anaesthetists, a national paediatric transport and retrieval service
and experienced ENT nurses. Equipment available in the operating theatre includes
a CO2 laser, a KTP laser, a microdebrider, microscopes, various rigid and flexible
telescopes and video equipment. We have recently purchased half a million
pounds-worth of new endoscopes for airway work.
Funding was obtained within the last few years to support the provision of digital
hearing aids for all children, and for modernisation of audiology facilities in the
hospital and various community settings. Audiology facilities in the hospital include
otoacoustic emissions testing and ABR. Universal neonatal screening for hearing
impairment was introduced in 2005. That year also saw the appointment of Dr Juan
Mora, Consultant in Paediatric Audiological Medicine. The department has a boneanchored hearing aid programme, and undertakes surgery for chronic otitis media
and prominent ears on a regular basis.
The department also provides a service for children with head and neck lesions,
including vascular malformations, branchial cleft anomalies and neck masses.
