TYLER CARDIOVASCULAR CONSULTANTS JOB DESCRIPTION TITLE: LVN GRADE: DEPARTMENT: NURSING 16 REPORTS TO: LVN SUPERVISOR DATE: JANUARY 8, 2013 SUMMARY: Provides professional nursing care under supervision for clinic patients, according to established standards and practices while providing excellent quality and service. Responsible for assisting with the preparation of patients for clinic including monitor hookups and EKG testing. Performs various lab and radiological tests and procedures when necessary. ESSENTIAL FUNCTIONS: WEIGHT EF#1 Accompanies assigned physician on hospital rounds/outreach clinics, assists with admitting/discharging patients, serves as patient liaison, returns pages or physician and communicates questions/answers. 25 EF#2 Assists physicians with educating patients on their medical conditions, medications, test results and courses of treatment. 15 EF#3 Performs telephone screening of patient’s complaints, notifying physician and relaying physician’s recommendations to patient as appropriate. 15 EF#4 Assists physicians with examinations, procedures, scheduling on-and off site procedures and appointments for patients and other processes related to direct patient care. 10 EF#5 Obtains vital signs, medical history, medications list and pertinent medical information from patients in preparation for examination by a physician. Performs EKG Testing, 24 hour holter monitor hookup, ppm hookup, 30 day hookups as needed. 5 EF#6 Prepares patient for various cardiac procedures and performs various cardiac procedures as ordered or indicated. Documents all procedures appropriately. 5 EF#7 Authorizes refills via e-scribe, telephone calls or faxes. 5 EF#8 Received consults from referring doctors and assigns them to the appropriate CVC Physician on call. 5 EF#9 Updates physicians patients list in hospitals, including signing out patients to appropriate physician. 5 EF#10 Records appropriate charges for clinic visits, hospital visits and procedures. 5 EF#11 Faxes orders for appropriate procedures to hospitals. 5 *Duties subject to change according to Management.
