Welcome to our Surgery Website
We aim to provide a high quality medical service to our patients with the help of other members
of our practice team - by working with patients to optimise health, medical treatment and disease
prevention
We hope to use our surgery website as a way of presenting all our patients with an up to date
resource for all information relating to our practice. We will keep it current with any news,
developments & details that are relevant to the practice & our patients.
As well as a source of information about our Practice, the staff & the services we provide, we
hope you use the website as a useful resource with links to other reliable websites for health
related information. If there is topical health information we will also add this to the website.
We would like to encourage all patients to update their contact details regularly. This includes
email addresses. Please note no information is shared with any outside parties.
Online Appointments & Prescriptions
You can now order your repeat prescriptions and book your appointments online, via this
website. To use this service you must first register your interest in person. Please come to
reception and they will supply you with the necessary forms to fill out and then you can begin
using our online services.
Dr Kidd runs a specialised contraception clinic, please make an appointment if you wish to
discuss options.
The treatment room operates an appointment system - you need to contact them directly in
person or by phoning them on 028 94413977 after 10:30am.
We are a designated Yellow Fever vaccination centre for both registered and non-registered
patients. Please contact reception for this service
Repeat prescriptions can be requested by telephone, post, in person or online.
Alternatively you can use the EMIS Access system which will allow you to do the following:




book an appointment
cancel an appointment
order repeat prescriptions
change your contact details
To use this system you must complete registration forms in the practice to acquire a username
and password. Please come into the surgery and speak to one of the receptionists who will be
able to help you. You will need to provide photographic Identification. Please note not all scripts
you recieve regularly may be on your repeat medication list, for example painkillers, diazepam,
antidepressants etc. - if they are not you will not be able to order these through this website but
instead need to contact reception. You will not be able to order scripts that you are not usually on
or any one-off scripts through this website.

There is also a facility to pre-register with the Practice which may be particularly useful to
students.
HOW DO I REGISTER FOR EMIS ACCESS?
You must be a fully registered patient, and need a letter from us containing personalised access
codes . To get this letter you need to come to Reception and ask for Registration for EMIS
Access, you will need to show some photo ID, e.g. your passport, driving licence or your student
ID card. The Receptionist will find you on our computer system and print out your access codes
and registration instructions. Once you have the access codes and instructions you can connect to
the web site at any time and register.
HOW DO I GET ONTO EMIS ACCESS?
You can visit this page by clicking on the following image from anywhere in this website The
first time you use the site you will be asked to create your account. Here you will need to enter
the four registration codes given to you by the practice.
REPEAT PRESCRIPTION REQUESTS
If one of our doctors has arranged a repeat prescription for you, and the review date has not
expired, then you can use EMIS Access to request a repeat. Prescriptions can be collected two
full working day after request. Once you have logged onto EMIS Access you can view any
medication items by clicking List of your repeat medication in the Repeat Prescriptions section
of the screen. Tick the items you require and then click Make Request. There is no need to add a
message relating to your request. You will then see a confirmation screen, providing you are
happy with the selection you made, click Confirm.
VIEWING YOUR CONTACT INFORMATION
Clicking on View/Update Your Contact Info (opposite Welcome on the EMIS Access home
page) will show the current details we hold for you. If you need to make any changes click Edit
but please note the following points which are very important: Your address: Your address may
appear in an abbreviated format on this screen, this is because our computer system limits the
address field to a specific width, which affects addresses for University accommodation. If you
address is abbreviated there is no need to change it unless it is technically incorrect (i.e. wrong
flat number). E-mail: it is useful for us if you can provide a current University e-mail address particularly if you are using our repeat prescription request service. After entering any changes
click Submit. You then need to confirm the changes by clicking Confirm. We review all changes
when we receive them, and accept them providing they comply with our requirements.
SECURE MESSAGING
The EMIS Access messaging system provides an encrypted secure method of communicating
electronically with us. Click on Send a message to your surgery. Complete the subject line, and
enter your message in the message field. Your message is limited to around 450 characters. The
EMIS Access home page tells you whether you have messages. You can then click to view them
Important notes: a copy of your message and our reply will also be attached automatically to
your medical record. We aim to respond to your secure message within 2 to 3 working days.

Patient Registration
To register with the practice, please bring your medical card to reception. If you cannot find your
medical card, speak to the practice receptionist who will advise you. Please complete our health
questionnaire before visiting the practice, this will speed up the registration process. This can be
printed off by clicking below:

All new patients will be given an appointment to see the doctor for a new patient medical.
Online Registration
If you wish to pre-register click on the link below to open the form. When you have completed
all of the details, click on the "Send" button to mail your form to us. Please note we ask all new
patients to have a medical carried out following registration. This allows patients to meet the
practice staff and ensure that a full medical history is taken. If you have completed the online
registration form we wouls ask that you attend the practice in the near future to sign the
registration forms and book an appointment.

Travel Information
If you require any vaccinations relating to foreign travel you need to make an appointment with
the practice nurse to discuss your travel arrangements. This will include which countries and
areas within countries that you are visiting to determine what vaccinations are required. There is

It is important to make this initial appointment as early as possible - at least 6 weeks before you
travel - as a second appointment will be required with the practice nurse to actually receive the
vaccinations. These vaccines have to be ordered as they are not a stock vaccine. Your second
appointment needs to be at least 2 weeks before you travel to allow the vaccines to work.
Some travel vaccines are ordered on a private prescription and these incur a charge over and
above the normal prescription charge. This is because not all travel vaccinations are included in
the services provided by the NHS.

Contraception
The practice provides a range of contraceptive choices. Implant fittings and coil fittings are
provided by Dr Kidd.
Emergency contraception - This can be either an oral tablet taken within 5 days of unprotected
sexual intercourse, or an emergency coil.

Sickness Certificates
You do not require a doctor's sickness certificate for any illness lasting seven days or less. Your
employer may however require you to complete a self-certification form (SC2) which is
available from your employer or on the HMRC website (www.hmrc.gov.uk/forms/sc2.pdf) .
Evidence that you are sick
If you are sick for more than seven days, your employer can ask you to give them some form of
medical evidence to support payment of SSP (statutory sick pay).
It is up to your employer to decide whether you are incapable of work. A medical certificate,
now called a 'Statement of Fitness for Work’ (see below) from your doctor is strong evidence
that you are sick and would normally be accepted, unless there is evidence to prove otherwise.
You could also provide evidence from someone who is not a medical practitioner, e.g. a dentist.
Your employer will decide whether or not this evidence is acceptable. If your employer has any
doubts, they may still ask for a medical certificate from your GP.
Statement of Fitness for Work - ’Fit Note'
The 'fit note' was introduced on 6 April 2010. With your employer's support, the note will help
you return to work sooner by providing more information about the effects of your illness or
injury.

Yellow Fever Vaccination
This practice is now a designated Yellow Fever Centre. Yellow fever vaccination is mandatory
for travel to certain areas in Africa and South America. This service is available both to
registered patients of the practice and to patients registered elsewhere. This service is not
provided on the NHS and will attract a fee which will include the price of the vaccine as well as
giving the vaccine.
Only if you are travelling to certain areas within the following countries you will require a

The practice nurse is available by appointment for chronic disease
management, health screening, travel vaccinations, smoking cessation
advice, weight management and general advice. She is also available to
give Yellow fever vaccination to both patients registered with the practice
and those registered elsewhere.
