As you may be aware, the new South Glasgow University Hospital and new
Royal Hospital for Sick Children are due to open on the current Southern site
early in 2015.
With this in mind, please note that positions based within the Victoria
Infirmary, Mansionhouse Unit, the Western Infirmary and the current Royal
Hospital for Sick Children at Yorkhill will change location and move to the new
hospitals.
Gartnavel General Hospital and Glasgow Royal Infirmary will also have some
services affected by moves to the new Hospitals.
These changes mean your base may change after joining us and you will be
informed as soon as possible prior to any change of base.

a)
Children’s Services across NHS Greater Glasgow and Clyde
The Royal Hospital for Sick Children, Glasgow
The Royal Hospital for Sick Children, Glasgow, is one of the largest
paediatric teaching hospitals in the UK and the largest in Scotland. It
provides secondary care for more than 900,000 people resident within
the Greater Glasgow and Clyde area, but also tertiary care for the 3m
people living across the West of Scotland and national managed
clinical network support in selected specialties for the whole population
of 5.5m. There are also 17 nationally designated services delivered
from the hospital including cardiac surgery, bone marrow and renal
transplantation, ECLS and complex airways.
There is a current bed complement of 250, which include 23 integrated
neonatal medical and surgery cots. There are 16 nationally funded
intensive care beds and 6 high dependency beds within an integrated
critical care facility. This facility has the physical capacity to extend to
26 integrated critical care beds. A new theatre complex was opened in
1998 comprising of 7 full theatres, scope room and dental suite. The
ED currently sees over 46,000 patients per year.
All paediatric medical and surgical subspecialties are represented,
including general medical paediatrics, cardiology, neonatology,
neurology, nephrology, respiratory, endocrinology, gastroenterology,
immunology
and
infectious
diseases,
dermatology,
haematology/oncology, rheumatology, metabolic medicine, audiology,
ophthalmology, ENT surgery, orthopaedics, neurosurgery and general
paediatric and neonatal surgery. A selection of child and adolescent
psychiatry facilities are located within the campus along with a recently
opened Child Protection Unit.
The hospital provides the major Undergraduate Paediatric Teaching
facility for the University of Glasgow and accommodates the University
Departments of Child Health, Child and Family Psychiatry, Paediatric
Biochemistry and Paediatric Surgery. There is on site clinical audit and
research and development support to assist with departmental
research projects.
The Radiology Department located within RHSC provides ultrasound,
CT, MRI and isotopic studies on site. All imaging is now film-less and is
accessed through the PACS system. Diagnostic laboratory facilities
including Haematology, Blood Banking, Biochemistry, Microbiology,
Virology, Histopathology and Genetics have moved to a new purpose
built Laboratory Facility on the South Glasgow Hospital site in
preparation for the move of the Children’s Hospital there in summer
2015.
b)
Other Paediatric services in NHSGG&C
There are three maternity hospitals serving the GG&C area: Princess
Royal Maternity Hospital, Southern General Hospital and the Royal
Alexandra Hospital, Paisley, all with neonatal intensive care facilities.
There is one children’s ward at the Royal Alexandra Hospital providing
local care for the children of Paisley.
There is an extensive range of specialist community based children’s
services across NHS Greater Glasgow. Managed within community
health and social care partnerships, these services are integrated with
primary care and social care services.
New Children’s Hospital – South Glasgow Hospitals
c)
The New Children's Hospital will provide state-of-the-art medical care in
a safe, child-friendly environment. It will care for all children and young
people up to their 16th birthday in a hospital that is specifically focused
on them. The new hospital will be a worthy successor to the Royal
Hospital for Sick Children at Yorkhill and will be physically linked to the
refurbished maternity unit and new Adult Hospital in order to provide
the best possible care for newborns and their mothers. With 256 beds
over five storeys, the stunning design of the hospital includes
accessible play areas, a MediCinema and a covered roof garden where
young patients can enjoy a range of activities in the fresh air. The new
children’s hospital will be a mix of single rooms, all with en suite and
parents accommodation, and four bedded wards.

Clinical Leadership
Medical services are a key component of integrated hospital paediatric
services within the Women and Children’s Directorate (of the Acute Operating
Division, NHS Greater Glasgow and Clyde).
Dr Philip Davies (Consultant in Paediatric Respiratory Medicine) is the Clinical
Director for Hospital Paediatrics and Neonatology. He is supported by a
number of Link Clinicians. The Link Clinician for Nephrology this is Dr David
Hughes.
This structure of clinical leadership is mirrored within surgical services.

Service Developments
1. Paediatric intensive care was nationally designated in April 2007 and
given uplift of £2.8m recurring investment.
2. Following the Scottish Government’s launch of the National Delivery
Plan for Specialist Children’s services in 2008, resources of £32m over
a three year period (2008 to 2011) were allocated as recurring
investment into specialist children’s services. This included support to
the national renal and urology network across Scotland.
3. RHSC now provides a formalised network of general surgery services
to the West of Scotland including outreach to NHS Boards in Forth
Valley and Lanarkshire.
Section 3: THE JOB ITSELF

Title:
Consultant in Paediatric Nephrology 1 WTE (10 PA 9 DCCPA + 1
SPA).
This is post replaces the part-time clinical duties (6 PA: 5 DCCPA +1
SPA) of the Associate Medical Director with an additional 4 DCCPA to
support and develop the clinical duties of the nephrology service locally
and nationally.

Description of the Service:
The Renal Unit in RHSC provides inpatient and outpatient services in
ward 6a which is shared with paediatric urology and paediatric
rheumatology. The ward also houses a 4 station haemodialysis unit.
The renal unit is part of the Scottish Paediatric Renal and Urology
Network (SPRUN) and provides multidisciplinary inpatient care for
children in Scotland requiring renal replacement therapy and for all
renal patients within the GGC area. Each year we have around 500
acute admissions, 8-10 renal transplants, 4-8 children on
haemodialysis and 10-14 on chronic peritoneal dialysis. We work
closely with paediatricians with an interest in nephrology in the other
children’s hospitals in Scotland (Aberdeen, Dundee and Edinburgh)
and in the District General Hospitals (DGHs) in Scotland.
We have a national contract with the National Services Division (NSD)
to provide paediatric renal transplantation for all patients under 16 in
Scotland. This work is undertaken along with 4 consultant transplant
surgeons from the adult service and 3 paediatric urologists. We have
an active living related renal transplant program. We follow around 50
children with renal transplants.
Ward 6a was expanded to 10 beds to support the development of a
nephro-urology unit allowing us to build on an existing close working
relationship with urology colleagues. The urologists in conjunction with
the CNS Continence service provide outpatient urodynamics and
biofeedback services. Complex rheumatology patients, many of whom
have renal involvement are managed by the rheumatology service
within the unit.
We work with one consultant providing acute cover for one ‘hot week’
at a time during working hours and weekends. Weekday nights are
covered by the other consultants. All patients have a named
consultant, and we work as two teams of 3 consultants covering
outpatient clinics to maximise the provision of clinics but still allowing a
degree of continuity.
Within RHSC we provide a consult service to the other services
providing care, including PICU, neonatal ICU (Ward 2B) and the ECMO
service. We provide advice for patients receiving continuous renal
replacement therapies within these areas. Plasmapheresis is provided
to inpatients by the Blood Transfusion Service. There is also a busy
cardiac surgery program and oncology/bone marrow transplant unit.
We also work closely with colleagues in radiology and pathology. There
are regular multidisciplinary meetings throughout the week with
colleagues in other specialties. We provide ante-natal counselling to
couples attending the Foetal Medicine Unit at the SGH.
As part of the SPRUN we each undertake clinics in the other Children’s
Hospitals and Paediatric Departments in DGHs across Scotland jointly
with local paediatricians. The network has undergone expansion as a
result of funding from the National Delivery Plan for Specialist
Childrens Services in Scotland and virtually all areas of Scotland are
now covered. These multidisciplinary joint network clinics are an
exciting area of continuing development within the service.
We would welcome an interest from applicants to this post in
developing management and clinical leadership skills to take forward
future clinical service developments both locally at RHSC and
nationally through the established managed clinical network, SPRUN.

Clinical Duties
Work as part of the consultant and wider multi-disciplinary team to

Provide acute cover to the Renal Unit on a 1 in 5.5 basis with
prospective cover, managing inpatients, consults within the hospital
and providing advice to paediatricians throughout Scotland

Provide outpatient services within RHSC (approximately 2 clinics per
cold week) including attending to patient administration in between
clinic visits

Provide clinical support to the haemodialysis and UTI service

Provide joint network clinics as part of the SPRUN working with local
clinical teams.

Attend multidisciplinary meetings within the unit
This is a representative sample of a working ‘hot’ week. 16 DCC PAs are
allocated to ‘hot’ week working.
