REFERRAL COORDINATOR

GENERAL SUMMARY OF DUTIES: Support mission and goal achievement, organizational quality and patientcentered care. Comply with organizational policies, procedures, standards of care and related laws and regulations
including HIPAA and OSHA regulations. Protect the dignity, privacy and confidentiality of patients and their families, as
well as co-workers and others.
The role of the Referral Coordinator is to assist the Primary Care Team by coordinating referral and related care needs of
individual patients. Function as the primary point of contact for the transmission of information between providers and
patients and providers and managed care plans, specialists and community referral organizations. Assist patients and
families to navigate the referral process. Serve as a patient advocate in communicating patient requests for interventions
and or action by the Care Team.
The Referral Coordinator will have access to PHI during the course of his/her work activities. The Referral Coordinator
will use this information to prepare both patient and medical records for visits with the provider and to keep the flow of
work going smoothly in the clinic. The Referral Coordinator will have access to the full electronic medical record and the
EPIC Practice Management system.
Provide excellent customer service to all internal and external customers, which includes team members, patients,
volunteers, students, visitors, and vendors, by consistently exceeding the customer’s expectations.
SUPERVISION EXERCISED: Supervision is not a responsibility of this position
SUPERVISION RECEIVED: Reports to the Clinic Site Manager
ESSENTIAL RESPONSIBILITIES: Referrals:
1. Provide an effective communication link between patient, specialists and community referral organizations, and
medical staff, including relaying messages from providers to and from Primary Care Provider (PCP) or other
providers
2. Coordinate referrals for specialist consultations, diagnostic studies, community resources, and Coordinated Care
Organizations (CCO)
3. Assist in coordination of care with pharmacies, insurance companies and other providers in the community.
Ensure that information goes when and where it is needed
4. Track external and internal referrals as outlined in WCHC referral procedure. Ensure all reports are received in a
timely manner from referral, and distribute report to appropriate PCP
5. Send records to specialists as requested
6. Participate in team decisions regarding data requirements for pro-actively managing the team’s panel
7. Use and update the directory of resources in the community to meet basic health and human needs
8. Perform other duties as assigned by supervisor.

1. As needed, perform the duties of Scheduler, Medical Records, and Patient Service Representative
EDUCATION: High school diploma or GED
EXPERIENCE: Minimum three years’ experience in the medical field

 Excellent customer service skills
 Keyboarding skills at 45+ wpm
 Ability to report for work as scheduled

WORKING CONDITIONS:
 Exposed to – Cold/heat controls, close contact with sick people
 Hazardous Conditions - Exposure to blood borne pathogens, infectious diseases, potential exposure to physical
violence, exposure to potentially hazardous chemicals
 Multiple Duties: All duties can be expected to be performed concomitantly. Must be able to work under
conditions of constant interruption and be able to stay on task
 Physical Demands – Lifting, bending, reaching, pushing, carrying, sitting, vision corrected in normal range,
hearing corrected in normal range
 Physical Effort – Mobility to visit all work sites and attend training, meetings, etc. as required
 Multiple Duties: All duties can be expected to be performed concomitantly. Must be able to work under

 Working environment – In an office/clinical setting and occasionally out in the field; Occasional evening or
weekend work
 Periodic state and national travel; travel between clinic sites
HIPAA REQUIREMENTS: This position has access to Protected Health Information (PHI) in order to perform the
functions of the job and employee shall apply the minimum necessary standard of HIPAA at all times. You are only to
