that refers to a grade asks that you record the student’s current grade in your class. The column that refers to
attendance is asking whether or not the student has any unexcused absences in your class during the past
week. In addition, you are welcome to provide additional comments.

To the Student:
The student must take the grade-monitoring sheet to each and every teacher Between Monday and
Wednesday. The completed form must be turned into Ms. Scales at the Thursday weekly meeting. If any of
your grades is below a C, you will be suspended from council till the grade is a C or above. Failure to turn in the
sheet will prohibit the student from participating the following week. These will need to turned in every other
week.
