The terms and conditions applicable to this post are those of all NHS Employees.

2. Superannuation

You have the option to join the NHS Superannuation Scheme, to participate in the State

Earnings Related Pension Scheme or to take out a Personal Pension.

Employee’s contributions to the NHS Scheme amount to 5% to 13.3% of salary

(depending on rate of Pensionable Pay) and the emplo yers’ contribution equates to

Job Title: Highly Specialist Clinical Pharmacist Orthopaedic Pre-Admission Clinic

& General Surgery & Ophthalmology

2. JOB PURPOSE

To provide highly specialist pharmaceutical care for orthopaedic patients, ensuring safe and cost effective use of medicines including compliance with medicines regulations. To contribute to the development of the pharmacy service to the Orthopaedic Pre-Admission

Clinic.

To lead and develop the pharmacy service to General Surgery & Ophthalmology

Contribute to efficient medicines management systems and clinical pharmacy services

Monitoring and reporting on drug expenditure to promote the safe and effective management of medicines within the Surgical Services Directorate.

3. ORGANISATIONAL POSITION

4. SCOPE AND RANGE

Along with the lead for Orthopaedics, responsible to the Chief Pharmacist for strategic planning and delivery of pharmacy services to the Orthopaedic Pre-Admission Clinic, and

General Surgery & Ophthalmology.

Develop and manage clinical pharmacy services to the Orthopaedic Pre-Admission

Clinic, and as part of the Enhanced Recovery multidisciplinary team for Orthopaedic inpatients

Develop and manage clinical pharmacy services to General Surgery & Ophthalmology .

Approx 60% clinical role in Pre-admission, and 40% clinical role for in-patients.

5. MAIN DUTIES/RESPONSIBILITIES

Management of Pharmacy Service to Orthopaedics

Implementation, development and update of policies, procedures, and SOPs relating to the independent prescribing role at Pre-Admission in line with professional code of ethics, GJNH and NHS Scotland policies and legislation, including: o Patient medication history taking o Promotion of the u se of patients’ own medicines at admission and during inpatient stay o Documentation of out-patient care issues including risk management issues. o Clinical pharmacy policy in conjunction with the Chief Pharmacist

Provision, analysis and reporting of management information on medicines usage including: o Workload o Ward drug expenditure

Co-ordination of the training of pharmacy staff in the Pre-Admission Clinic

Practice research and Audit of clinical pharmacy services o Carrying out regular audit of services o Analysis of results and the preparation and implementation of improvement plans o Use of research data to identify current best practice

Forward planning of pharmacy services to Orthopaedic Pre-Admission: o Manpower planning requirements for services o Future IT requirements o Development of clinical pharmacy policies in relation to the independent prescriber role at Pre-Admission

Act as pharmacy representative on: o Drug and Therapeutics Committee o Surgical Services Clinical Governance Group

Clinical Practice

To act as a role model for pharmacy staff working in orthopaedics

Apply highly specialist clinical knowledge to analyse complex patient information to ensure the optimal clinical outcome.

Apply highly specialist clinical knowledge and judgment to analyse information, at times where evidence may be lacking, in order to negotiate the optimal clinical outcome.

Apply highly specialist knowledge on the use of medicines to deliver pharmaceutical care to patients within a specified patient population or directorate/clinical speciality and improve patient care by:

interpreting individual patient drug charts, prescriptions and medical information to provide advice on medication treatments. This includes o checking dosing, drug/drug or drug/disease contraindications, side-effects, precautions, and inappropriate or omitted treatment. o ensuring medicines and doses prescribed during hospital stay are appropriate for the patient and their medical conditions

o considering various options and deciding the best risk benefit choice for the individual patient

documenting care and risk management issues including o Individualising pharmaceutical care plans and documenting / resolving pharmaceutical care issues o Clarifying ambiguous prescriptions to avoid medication errors o Patient allergy o Adverse drug reactions and interactions o Medication errors inc. prescribing and administration errors o Identification of other risks associated with the use of medicines

and implement strategies to reduce risks, where possible

recommending alternative medications or forms of medication where these are more clinically or cost effective

taking medication histories on admission and reviewing/verifying inpatient and discharge prescriptions-ensuring patients are prescribed the correct medicines on admission to and discharge from hospital.

provision of a therapeutic drug monitoring system

ensuring patients/carers understand complicated therapies e.g. warfarin

ensuring patients can use medical devices, e.g. inhalers, correctly and know when to use them

developing and implementing systems to facilitate the discharge process discussing medicines with patients / carers and persuading / motivating them to comply with them on discharge

assessing what aids would assist patients to take their medicines correctly at home

 liaising with the patient’s community pharmacy and/or GP where appropriate and developing systems to ensure continuity of pharmaceutical care across different healthcare settings.

integrating research evidence into clinical practice

Assist the multi-disciplinary team in the care of each patient by e.g.

 Suggesting alterations to patients’ complicated medication regimes to simplify and aid therapy compliance.

Participating in consultant ward rounds and multidisciplinary team meetings providing clinical and financial advice if appropriate.

Practising as a supplementary or independent prescriber, where appropriate

Promoting rational, safe and economic use of medicines

Advising doctors of problems with medicines

Advising on doses / course lengths of drugs / route of administration

 Advising other healthcare professionals on how a patient’s medication may affect their practice e.g. infection control personnel

Providing information and negotiating with clinical staff to ensure compliance with Scottish Medicines Consortium guidance, formulary, guidelines, drug and therapeutic committee decisions, and other pharmacy related strategies

Generation and interpretation of medicines utilisation reports from the

pharmacy stock control system

Provide advice to other specialist and clinical pharmacists within the defined clinical speciality

Dispensing services o Professionally checking prescriptions and liasing with clinical staff to confirm ambiguous prescriptions and correct where appropriate. o Dispensing of out-patient, TTO and clinical trial prescriptions as required o Checking of out-patient, TTO and clinical trial prescriptions as required o Contributing to the collection of dispensary statistics

Prescribing

To work as an independent non-medical prescriber within areas of work as required, taking responsibility for the clinical assessment of the patient, establishing a diagnosis and the clinical management required, as well as responsibility for prescribing where necessary and the appropriateness of any prescription issued. Independent prescribers will be responsible for prescribing a range of drugs within their competence in accordance with local protocols and British National Formulary.

To write prescriptions in accordance with local policy, taking full responsibility for thepatient as well as for the medicines prescribed

 Ensure the appropriate records are made in the patient’s case notes where appropriate which identify you as the independent prescriber.

To ensure that contemporaneous records which are unambiguous and legible are maintained and kept in accordance with local policy and professional codes of conduct

To communicate fully with those patients who are unable to communicate or give consent ensuring that the benefits of non-medical prescribing are documented fully, and to provide reassurance to both the patient and the carer

 Where it is recognised that a patient’s needs are outside your clinical and professional experience, to take responsibility make to make referrals to a specialist doctor, the multidisciplinary team or other agency.

Assist with the development of policies and inform development of non-medical prescribing across the organisation

Take the lead on policy, procedural development and patient information relating tononmedical prescribing in designated areas of practice.

To be responsible for ensuring the security and safe handling of inpatient, and outpatient prescription forms and to report any loss or theft accordingly.

In the event of adverse drug reaction, to be accountable for ensuring that the

Consultant is advised accordingly and that the local and national incident reporting process is followed.

Clinical Pharmacy Leadership

Implementation, development and update of clinical pharmacy services in relation to the General Surgery & Ophthalmology units

Support Lead Clinical Pharmacists in developing a pharmacy team of specialist clinical pharmacists and technicians in the development, implementation and monitoring of pharmaceutical care within general surgery & ophthalmology units, providing clinical supervision.

Regularly supervise (at least once per week) more junior clinical pharmacists, preregistration pharmacists, MSc students and pharmacy undergraduates in the provision of pharmaceutical care for patients.

Provide highly specialist advice to the multidisciplinary team in general surgery & ophthalmology on the legal, ethical, safe, clinical and cost-effective use of medicines, ensuring compliance with medicine legislation and guidelines.

Identify clinical pharmacy service needs within specialty and, working with the Chief

Pharmacist and other senior healthcare professionals, support the development of appropriate business cases.

Review adverse clinical incidents and use highly specialist clinical knowledge to advise the Chief Pharmacist general surgery & ophthalmology on strategies to minimise risks during the use of medicines.

Provide Medicines Governance leadership within the multidisciplinary team responsible for the care of general surgery & ophthalmology patients.

Interpret financial reports and use highly specialist clinical knowledge to advise general surgery & ophthalmology on the cost effective use of medicines.

Identify and prioritise patient groups for pharmaceutical care including supplementary/independent prescribing by pharmacists in line with national and local legislation and as agreed with the Lead Clinical Pharmacist.

CPD/Training commitment

Contribute to the development of strategies to meet the training needs of staff providing pharmaceutical care within a specified patient population or directorate/clinical speciality.

Develop, implement and evaluate training in pharmaceutical care for undergraduates, pre-registration and all grades of pharmacists including those studying for a

Diploma/MSc in Clinical Pharmacy

Plan, deliver and evaluate education and training to pharmacy and other staff, including medical and nursing staff, in relation to safe, clinical and cost-effective use of medicines.

Provide education and training for patients and their carers on the safe and effective use of their medicines

Organise and participate in the regular clinical training and supervision of junior grade pharmacists and pharmacy technicians.

9. PHYSICAL DEMANDS OF THE JOB

General keyboard skills

High degree of precision and accuracy for dispensing and final checking, often with strict time constraints.

Frequent requirement for high levels of concentration, often with interruptions when checking dispensing.

Clinical work requires a high level of concentration characterised by assessing different pieces of sometimes conflicting information and forming a clinical opinion. This work is often conducted under time pressure and subject to frequent interruptions.

Devising, developing and implementing the delivery of the pharmacy service to

Explaining highly complex medicine regimes to patients / carers where there may be barriers to understanding, whilst managing the patient’s expectations regarding treatment outcomes.
