Description of Services: CCT (Community Treatment Team): A mental health support program
which provides intensive case management, psychiatric treatment, and 24-hour crisis intervention to
individuals with “serious and persistent mental illness”. Treatment team consists of a psychiatrist, a
nurse, and master’s and bachelor’s level therapists. Individuals in need of CTT services and who
lack medical insurance coverage may be eligible for CTT services in a county-funded program
(programs which have contracts with Lehigh County Mental Health). Individuals in need of countyfunded CTT services must complete a “Financial Liability Determination” through the office of Lehigh
County Mental Health: Adult Unit, and meet income eligibility guidelines.

Description of Services: ICM (Intensive Case Management) a Case Manager assigned to an
individual identified as requiring intensive mental health treatment and support. The ICM assists the
individual in accessing mental health treatment programs and community resources. The ICM is
required to meet face-to-face with the individual a minimum of every two weeks and offers mental
health crisis intervention 24 hours per day/7days per week. Medical Assistance insurance is
required. CTT programs contracted with Lehigh County MH/MR/D&A for financially eligible
individuals lacking Medical Assistance and/or Private Medical Insurance Coverage. Treatment
Team consists a psychiatrist, 3 nurses, master’s level staff, and bachelor’s level staff. 24 hours
per day/7 days per week. Serves adults 18+ years of age and no longer in high school. Accepts
Medical Assistance insurance.

Description of Services: CRR (County-funded, apartment based program) (3-4 consumers per
apartment) 2 sites: More intensive program site has staff 24 hours a day and serves 10
consumers. Usually a 1 year long program and consumers can go to the moderate level.
Moderate intensive program has staff for 10 hours per day and serves 28 consumers. To qualify
for this program the consumer needs to have more independent living skills. This is a transitional
program to independent living. Rent based on income. Consumers need to participate in some
form of day programming at least 3 days per week, which includes: work, partial hospitalization,
clubhouse, etc.
