CINCINNATI STATE TECHNICAL & COMMUNITY COLLEGE OTA 4660 and 4661 Level II Fieldwork Policy for Successful Completion of Level II Fieldwork In order to pass OTA 4660 and OTA 4661, the student must: 1. Receive a passing score on each of the two rotation evaluations using the AOTA "Fieldwork Performance Evaluation for the Occupational Therapy Assistant Student". 2. The two rotations shall consist of either one pediatric or psychiatry/psychosocial and one physical dysfunction/related area. 3. If the student successfully completes the two rotations, they may take an optional, third specialty rotation. 4. If a student receives a failing score on one rotation, they will be given one opportunity to repeat and pass that type of rotation (i.e., psychiatry, psychosocial, physical dysfunction, school system, etc.) 5. If a student receives a failing score on an evaluation, the make-up rotation place and dates will be scheduled according to site availability and at the convenience of the school. A Clinical Practice Hearing will be held for students who fail fieldwork or who have problems at the fieldwork site which cannot be resolved. 6. If a student receives a failing score on two consecutive field work rotations OR receives a failing score on two rotations of the same type, they will be dismissed from the program. 7. If a student receives a passing score on the field work evaluation but receives a rating indicating they are "not ready for clinical practice", the student will be given a Clinical Practice Hearing. If this occurs during their first rotation, it will be up to the discretion of the faculty as to whether the student begins their second rotation prior to the hearing. If this occurs on the second rotation, it will be up to the discretion of the faculty as to whether or not the student is considered having successfully completed the field work requirements or whether the student has a Clinical Practice Hearing before being considered to have successfully passed the fieldwork. 8. The American Occupational Therapy Association requires that Level II fieldwork be completed within 18 months of the students completion of the OTA Program didactic course work. CINCINNATI STATE TECHNICAL & COMMUNITY COLLEGE OTA 4660 and 4661 Level II Fieldwork Attendance Policy The attendance policy outlined in the Health Division Student Handbook is effective for OTA 4660 and OTA 4661. Any days missed (regardless of the reason) must be made up. Any time missed due to tardiness is to be made up according to the discretion of the fieldwork supervisor. The student is to follow facility policies regarding holidays. The Cincinnati State calendar is not in effect for OTA 4660 and 4661. If the supervising department is closed, then the student shall have the day off and not be required to make up the time. If the faculty has an official holiday or snow day (i.e., school system whereas the school is closed) the student is not required to make up the time. If Cincinnati State has an official day off (i.e., Martin Luther King's Birthday), the student is required to report to the facility unless the facility is also closed in recognition of a holiday. Grading Policy The student will be given a pass/fail grade each for OTA 4660 and for OTA 4661. The grade will be assigned by the Cincinnati State Academic Fieldwork Coordinator. The grade will be based on the final score of the fieldwork evaluations. The score will be based on the Fieldwork Performance Evaluation for the Occupational Therapy Assistant Student.
