2011 Tommy Cooper Award This is your nomination form for the 2011 Tommy Cooper Award, which recognizes an individual for his or her outstanding contribution to local agriculture. The Tommy Cooper Award has been presented since 1959 by Bayshore Broadcasting Corporation and the Owen Sound Sun Times. Tommy Cooper died in 1981, but his legacy lives on. He is well remembered for his 39 years as a provincial government farm extension worker in Grey County. Cooper is credited for helping area farmers adapt to new scientific and mechanical innovations in the first half of the 20th Century. He was also instrumental in the founding of the Grey-Bruce Livestock Co-operative, and was active in many commodity organizations. In the nomination forms, be certain to include any activity of the nominee you believe has contributed to the betterment of agriculture and rural living in Grey and Bruce Counties. Special, though not exclusive attention will be paid to those accomplishments made in the last five years. If the nominee is proposed for his/her accomplishments made through employment, please ensure those accomplishments are significantly above and beyond usual job duties. If you wish, you may include newspaper clippings, photographs, etc., for documentation. Complete and accurate information is essential. The deadline for entries is 12:00 p.m. (NOON) , Wednesday March 28th, 2012 at which time completed applications will be distributed to the judging panel. There will be no exceptions to the deadline. This year, the Award Ceremony will be held Friday, April 13th, 2012 starting at 6:30 p.m., at the Elmwood Community Centre. Achievements (20 points) Please explain what goals were met in the position(s) listed above. 3 Main Area of Contributions as a Volunteer (10 points) Please describe the one greatest contribution the nominee made for the betterment of agriculture and rural life. This could be through representation to senior governments, an important development for a major farm organization or other group, an outstanding job, or representing the Grey Bruce area or its interest. Please include any contributions to rural living in Grey Bruce, or outstanding service in the community, provincial, national, or international level. Youth Participation (10 points) Please list any associations or work within Junior Farmers or Junior Institute, 4-H, Scholastic Achievement, and any extra curricular activities. If the nominee is/was not a participating member in the above listed organizations at this time, points will be awarded for being a mentor within these groups. 4 Community Service (20 points) Please list any work of the nominee in local governments, boards, committees, etc. Please do not re-list previously noted achievements. 5 Special Achievements The Judges Panel has an additional 10 points to be used at their discretion to suitably weigh the contribution of each nominee using the following criteria. Should any of these criteria apply to your nominee, be sure to tell the judges how. Age. The nominee has contributed much, despite his/her youth, or continues to make a contribution in senior years. Roles. Nominations should reflect volunteer work within the industry. However, if the nominee’s work reflects upon the volunteer efforts (i.e. a nominee is a teacher, who is involved with the Agri-Food Education Council), then certainly cite the professional accomplishments. Significance. The nominee’s contribution benefits more than a narrow segment of agriculture. 6 Relevance. The nominee’s efforts and contributions to agriculture have been especially noteworthy in the recent past. World and National Service. If service extends beyond the boundaries of Grey and Bruce Counties, with effects still felt at home. Various exchange programs, national boards, etc. fit this category.
