JOB SPECIFICATION
1.
Summary of the broad purpose of the position in relation to the
organisation's goals (its expected outcome and how it is achieved).
The Aboriginal Aged Care Worker is accountable to the Director of the Murray Mallee
Community Health Service. The Aboriginal Aged Care Worker provides a range of
services which meet the community service needs of the frail, aged and younger
disabled members of the Aboriginal community in the Murray Mallee Region
2.
Reporting/Working relationships (to whom the person reports, staff for
whom the person is responsible and other significant connections and
working relationships within the organisation).


The Aboriginal Aged Care Worker is accountable to the Director
(MMCHS), reports to and is supervised by the Regional Coordinator
Aboriginal Aged Care and is a member of a multi-disciplinary team.
Special conditions (such as non-metropolitan
requirements, frequent overtime etc.).







4. Statement of Key Outcomes and Associated Activities (group into major
areas of responsibility/activity and list in descending order of

4.1 Promote the health and well-being of the Aboriginal frail, aged and young
disabled family members by:



Providing a timely and comprehensive range of services including
shopping, and social support
Providing referrals for Respite Services
Assisting with the transport needs of clients
Promoting the use of locally based mainstream community health services
4.2 Contribute to effective
clients and staff by:

assisting in client advocacy and assessment and informing the Aboriginal
Health Workers at the MMCHS of the additional needs of Aboriginal
families
ensuring client privacy, security of client records and confidentiality of
client related information
assisting families to access mainstream health services delivered by
MMCHC and Murray Bridge Hospital staff
Under the direction of the Regional Coordinator provide day to day
support and supervision as required to other staff employed in the
program (eg aged care trainees, volunteers, contract staff)
4.3 Maintain accountability and contribute to the efficient and effective
management of programs and services to frail, aged and younger disabled
community members by:


maintaining accurate and legible client records and statistical information
on activities, and by providing data and reports as required
participating in program planning, decision making, implementation,
evaluation and review processes
being responsible and accountable for adequately managing the official
records created and received according to relevant legislation, policies
and procedures

Experience (including community experience)








Demonstrated active participation in the Aboriginal community
Demonstrated experience in the coordination of services
Experience working alongside Aboriginal community members, children and
families in the provision of respectful and culturally relevant services
Experience in working in partnership with the Aboriginal community using a
community development approach, including case management
Experience in organising groups, developing health programs, and the provision
of appropriate referrals for Aboriginal people
Experience in dealing with and negotiating with people in government and nongovernment and community organisations
Sound community development and group work experience
Working within multi-disciplinary teams
Knowledge

Current knowledge of Aboriginal Community Care services and issues including
Aged Care, Community Care and other programs relevant to the support of frail,
aged and younger disabled Aboriginal community members

LEVEL CHARACTERISTICS
This position is classified at the Operational Services Level 2 by virtue of the
requirement to:

POSITION SUMMARY
The Aboriginal Aged Care Worker is accountable to the Regional Coordinator
Aboriginal Aged Care Program and will be required to contribute to the development
of strategies and programs to meet the Community Service needs of the frail, aged
and younger disabled Aboriginal community in the Southern Fleurieu region.
This is achieved through identifying the frail, aged and younger disabled Aboriginal
community and their families, providing an assessment of the needs of these clients
and responding by facilitating and coordinating the delivery of services, information
and other programs which respond to identified needs. In the process, the focus of
Aboriginal Aged Care Program will be sharpened within local and regional health
service planning and activities.

POSITION CHARACTERISTICS
The Aboriginal Aged Care Worker is responsible for providing appropriate delivery of
Community Services in the region. Day to day management supervision of the
incumbent will be provided by the Regional Coordinator, Aboriginal Aged Care
Program
The Murray Mallee Community Health Service and the Southern Fleurieu Health
Service are integrated health care units which provide a wide range of Domiciliary
care and community health services to the local communities of the district councils
of Murray Bridge, Alexandrina, Victor Harbor and Coorong including an Aboriginal
population estimated between 1 000 and 1 100 persons.
The Aboriginal Aged Care Worker will have working relationships with other
MMCHS/SFHS staff, local and statewide agencies and individuals with responsibility
for the health and well-being of Aboriginal families. The Aboriginal Aged Care
Worker will liaise closely with other Aboriginal Health Workers and service providers
within the Hills Mallee Southern region.
The incumbent works autonomously on a day to day basis in planning, implementing
and coordinating of services at the local level. The incumbent will have a broad
range of knowledge and experience and have well developed communication,
networking and organisational skills.
