Celebrating 49 Years of Camp Superkids
Dear Campers & Parents/Guardians:
We are very excited you have chosen to apply for a fun filled week at Camp Superkids this summer. Camp will be on
Sunday June 28 – Friday July, 3 2015 at YMCA Camp Ihduhapi in Loretto, MN. Transportation will not be provided. You
will need to drop your child off on Sunday between 1:00 – 3:00 p.m. and pick up on Friday between 1:00 – 2:30 p.m.
All forms must be sent in to secure your spot in camp. We will accept 160 campers with some priority given to first-time
campers and campers on a daily controller medication. To secure your spot:
If your camper HAS HAD a physical or asthma appointment since July 1, 2014
1. Parent/Guardian fill out pages 1-6
2. Get pages 7-8 to your child’s physician to fill out
3. Return all forms to Cynthia:

If your camper HAS NOT had a physical or asthma appointment since July 1, 2014
1. Schedule a physical exam or an asthma appointment
2. Fill out pages 1-6 and write date of appointment on bottom of page 6.

4. Bring the Asthma History and Physical Examination pages (physician forms page 7-8) to the scheduled appointment.
5. Have the doctor’s office fax pages 7-8 to Cynthia at 651-227-5459
6. Please call Cynthia at 651-268-7587 to make sure the paperwork came through.
7. Cynthia will need to receive your paperwork within 3 days of your appointment or your camper may lose his/her
spot.
You will receive a letter confirming your camper’s spot in camp by June 5th.
We anticipate that we will have an extensive waiting list. If for any reason you are unable to attend camp, please
notify us as soon as possible. All payments for camp must be received by June 5th.
