be used toward satisfaction of your UCD degree requirements. Fill out one copy of this form for each class you
have taken at another institution that you would like credit for. Attach the course’s catalog description and
syllabus to this form. If no English-language version is available, please translate the catalog description for
us and attach the foreign-language material you have. Particularly if requesting bypass credit, it is also desirable
to attach samples of your work done for the class.
