You will be joining our existing team at an exciting time in the development of the Ayrshire
Cardiology service.
This post, based at University Hospital Ayr, will bring our
establishment to six posts, three based each at University Hospital Ayr and University
Hospital Crosshouse.
This post offers the opportunity to play a major role in the future development of cardiac
services within Ayrshire. The candidate will have a sub-speciality interest in Imaging
within Cardiology, complement our existing full range of cardiac services including
diagnostic coronary angiography, permanent pacemaker and ICD service, transthoracic
and transoesophageal echocardiography and nuclear cardiology. There is a Rapid Access
Chest pain Service and excellent relationships with local GPs, an award-winning cardiac
rehabilitation and heart failure service, comprising 2 dedicated rehabilitation nurses, 4
heart failure nurses and a well established Managed Clinical Network. One consultant
colleague also participates in interventional cardiology services performed in Hairmyres
Hospital,
There are close links with Glasgow University, and there is an active undergraduate and
postgraduate teaching programme.
Our modern District General Hospitals are situated in one of the most scenic parts of the
UK. With easy access to Glasgow and Edinburgh, excellent housing, and first class
educational and recreational facilities, such as internationally famous golf courses and
sailing, which all contribute to a first class quality of life. The Terms and Conditions of
Service are those determined by the new Consultant Contract (Scotland) 2004 and are
offered on a 10 programmed activity basis. Applications from individuals who may wish to
work on a part-time or job share basis will be considered. Opportunities may exist for
Extra Programmed Activities subject to service requirements.
Accredited in Cardiology with a sub-specialty interest in imaging, you will have the
necessary training and expertise to provide a complete range of cardiology services for
patients in Ayrshire. NHS Ayrshire and Arran aspires to develop a full non-invasive
imaging service based around CT, MRI and stress echo. Dual accreditation in General
Medicine is desirable but not essential. Applicants will be appropriately experienced with
Full GMC Registration, including a current Licence to Practice. Applicants must also be on
the Specialist Register of the GMC or be within 6 months of the anticipated award of a
CCT or CESR at the time of interview. Evidence of eligibility of inclusion on the Specialist
Register will require to be provided at time of application.

JOB DESCRIPTION
CONSULTANT IN CARDIOLOGY
1.
Introduction
This new post will join the existing 5 Consultant Cardiologists in NHS Ayrshire & Arran,
with 2 Cardiologists at the University Hospital Ayr and 3 in University Hospital
Crosshouse.
The post is based in the Medical Unit at The University Hospital, Ayr, but there is close
cross-site cooperation between the cardiology team, and some of the applicant’s duties
will be performed at University Hospital Crosshouse. The Medical Unit is part of the
Medical Directorate of NHS Ayrshire and Arran ICES, which is responsible for acute
services across Ayrshire & Arran, serving a population of approximately 368,000.
2.
Facilities, Resources and Activity
The University Hospital Ayr (348 beds) provides acute and out-patient medical services
to the population (approx. 180,000) of South Ayrshire. The University Hospital Ayr
Medical Directorate has 11,000 in-patient discharges per year and treats over 2,100 day
cases, 6,000 new out-patients and 15,000 review out-patients annually.
There are 144 Medical beds including an 18 bed acute medical receiving ward, a six
bed Medical High Care and a Coronary Care Unit of six beds. There has recently been
established a cross-site dedicated cardiology rota, shared between five cardiologists
and one Speciality Doctor. There is funding available for a second cardiology speciality
doctor post. The cardiology rota provides selected out-of-hours cardiology input for
Ayrshire. At the weekends the cardiologists perform ward rounds in both Acute Cardiac
Care Units.
The cardiology team works in close association with colleagues in General Medicine.
The candidate will not contribute to the General Medical rota, but will continue to
provide ongoing medical care for patients in their ward area. Priority is given to allocate
cardiology patients preferentially.
There is access to a 5 bedded Intensive Therapy Unit and a 4 bed High Dependency
Unit where clinical responsibility for medical patients is shared with Consultant
Anaesthetic staff.
Medical services are supported by the Directorates of General Surgery, Vascular
Surgery, Urology, Orthopaedics, Ophthalmology and Accident and Emergency
Medicine. Radiological support is provided by a staff of 6 Consultants, providing a full
interventional and diagnostic service including interventional radiology, CT and MR
imaging as well as Nuclear Medicine including Nuclear Cardiology.
Laboratory (haematology and biochemistry) services are provided on-site up to midnight
and on Saturday and Sunday mornings at weekends. An area laboratory service,
based at University Hospital Crosshouse, provides non-routine and ‘out of hours’
services and the Area Pathology Service is also based there.
Allied Health Professional support, including physiotherapy, occupational therapy,
dietetics, speech & language therapy and podiatry is also based at The University
Hospital Ayr.
There are visiting Consultants in Radiotherapy, Neurology and Renal Medicine and
close working relationships exist with Regional Neurosurgical and Cardiothoracic
colleagues in Glasgow.
Cardiology Services
 In-patient Cardiology services at The University Hospital Ayr are centred on a sixbedded Acute Cardiac Care Unit and a 24 bedded cardiology ward. Full monitoring
facilities are available for all CCU beds with telemetry for patients in the cardiology
ward. There are facilities for bedside echo and temporary pacing.
 Extensive non-invasive investigative techniques are well established and include colour
flow and Doppler transthoracic echocardiography, Holter and Transtelephonic
monitoring, transoesophageal echocardiography and Tilt Table Testing.
The
department is accredited with the British Society for Echocardiography. We have four
accredited echo cardiac physiologists, one of whom is accredited as an independent
operator for TOE.
 Out-patient exercise tests are supervised by Senior Physiological Measurement
Technicians and there is an open access service for ECG, exercise tests and
echocardiography. There is a Rapid Access Chest Pain clinic run by a GP with Special
Interest. Radionuclide perfusion scintigraphy and ventriculography are available within
the Nuclear Medicine Department.
 A permanent pacemaker implantation programme was established in 1993 and
approximately 120 implants are carried out each year. There are two Cardiac
Physiologists accredited by NASPE in pacing, supported by other senior Cardiac
Physiologists. This is an area service and is run with the close co-operation of the
Cardiologists from University Hospital Crosshouse.
 A regional ICD service was established in 2008, initially with follow-up service and a full
implantation programme since 2011 (approx 40 ICDs per annum).
 The Cardiac Service is supported by a team of Cardiac Physiologists (12.98 WTE).
 There is a long established nurse led cardiac rehabilitation service and a heart failure
service.
 There is a well established Resuscitation Training Department (4 WTE) in Ayrshire and
Arran, with frequent UKRC ALS, ATLS and APLS courses.
 There is well established Managed Clinical Network in Cardiology.
 Four of the Ayrshire Cardiologists have one session weekly for diagnostic
catheterisation at the University Hospital Ayr.
 A national network for the management of adult congenital heart disease is recently
established.
The successful candidate will be encouraged to develop a non-invasive imaging service
for Ayrshire. Both sites have state-of-the-art MRI and CT scanners, with some appropriate
software for cardiac imaging. There is considerable expertise in both Ayr and Crosshouse
in echocardiography, and the opportunity exists to develop a stress echo service.
Education:
The Medical Unit holds twice weekly CPD sessions, and is fortunate to have the use of a
large modern Education Centre located in the centre of the hospital. There is a well
stocked library with good e- learning facilities, a large lecture theatre with state-of-the art
equipment and meeting rooms.
The Directorate is fully involved in the teaching of Glasgow University undergraduates and
the postholder will be expected to contribute to both undergraduate and postgraduate
teaching programmes.

Future Development of Cardiology Services in Ayrshire
In line with the West of Scotland Regional Planning Group Cardiac Imaging strategy, it is
planned that there will in future be a shift from invasive coronary arteriography towards
non-invasive assessment based on cardiac CT and MRI.

Proposed Weekly Programme
The proposed weekly programme is shown at Section 4. Activities with current fixed time
commitments will be carried out as detailed in the work programme e.g. clinics. Other DCC and SPA
activities are shown with indicative timings within the weekly programme and will be discussed with the
appointee. This timetable will be reviewed three months following appointment and should therefore
be regarded as an interim programme. Opportunities may exist for Extra Programmed Activities to be
undertaken subject to service requirements and in accordance with national terms and conditions of
service.
4.2
Notes on the Programme
Patient Administration: This activity covers the management of individual patients including Out
Patient administration, results reporting, letters/phone calls to patients, carers, GP’S and members of
the wider multidisciplinary team involved in the patients care.
Ward Rounds: the time allocated is indicative and will be discussed with the appointee. Ward work
will include teaching ward rounds as required.
Travel: Any travel allocation will be included within the Total Programmed Activities and will be
determined by location at which Direct Clinical Care and Supporting Professional activities are carried
out.
On call arrangements: The applicant will contribute to a 1:5 Ayrshire Cardiology Rota, supported by
the Speciality Doctor. This rota provides out of hours specialist cardiology advice and procedures. It
is not expected to provide routine cardiology care for all patients which remains the responsibility of
the General Medical team. At weekends, a member of the cardiology team will perform ward rounds
on the Acute Cardiac Care Units in both Ayr and Crosshouse. This takes approximately five hours
including travel, and on-call availability remains thereafter from home.
Level 1 availability supplement is 8%. Predictable and unpredictable work associated with the
provision of on call duties including weekend receiving ward rounds equivalent to 1 PA per week.
Details thereof can be discussed on an individual basis.
Supporting Professional Activities: A minimum of 1 SPA is included in the indicative job plan,
amounting to 168 hours per annum which shall normally be sufficient to reflect activities such as
revalidation, appraisal, personal audit, and professional development (occurring outwith the 30 days
of study leave entitlement in any three year period). Time permitting, it may also cover minimal
teaching, training and non-clinical administration. Any additional SPA allocation will require to be
evidenced as mutually beneficial and required by the department. Adjustment to the programme to
incorporate additional SPA will require other activities to be reviewed to accommodation any increase
as necessary. It will be requested that SPAs are delivered at the normal place of work, unless there
are mutual advantages to it being performed elsewhere. The exact timing and location of SPAs, and
flexibility around these, will be agreed during the 1:1 meeting with the Clinical Director/Associate
Medical Director and included in the prospective job plan
Research: Research is encouraged and supported by an active Research and Development
Committee. The appointee will be encouraged to develop research interests associated with their
specialist interest.
Teaching: The postholder will be responsible for the training and supervision of post-graduates and
under-graduates and will be expected as part of their SPA allocation to devote time to this activity on
a regular basis. In addition he/she will be expected to ensure that Junior Staff and medical students
receive adequate support and advice and may act as a contact as the person responsible for
overseeing their training and as an initial source of advice.
Private Practice: If the postholder wishes to undertake any private practice, they are obliged to
inform their employer at the time of appointment of their intentions to do so. This should be submitted
in writing to the Medical Director.
The postholder shall be free to undertake private practice without approval provided such work is
undertaken outside the time agreed in the job plan for programmed activities. (Refer Section 6 of the
New Consultant Contract).
5.
The Post : Consultant in Cardiology
The postholder will be accountable to the Clinical Director who will agree the Job Plan.
He/she will be expected to work with local managers and professional colleagues in the efficient
running of services and will share with Consultant colleagues in the medical contribution to
management. Subject to the provisions of the Terms and Conditions of Service, he/she is expected to
observe the NHS Ayrshire and Arran’s agreed policies and procedures, drawn up in consultation with
the profession on clinical matters, and to follow the standing orders and financial instructions of NHS
Ayrshire & Arran.
In particular, where he/she formally manages employees of NHS Ayrshire and Arran, the postholder
will be expected to follow the Local and National Employment and Personnel Policies and Procedures.
He/she will be expected to make sure that there are adequate arrangements for hospital staff involved
in the care of patients to be able to make contact with the postholder when necessary.
The postholder is required to comply with the NHS Ayrshire and Arran Health and Safety Policies.
He/she will be responsible for the training and supervision of Junior Medical Staff who work with the
postholder and will be expected to devote time to this activity on a regular basis. In addition, he/she
will be expected to ensure that Junior Staff have access to advice and counselling. If appropriate, the
postholder will be named in the Contracts of Doctors in training grades as the person responsible for
overseeing their training, and as the initial source of advice to such Doctors on their career.
5.1
Resources
The staff resources of the Directorate are listed elsewhere. The postholder will have access to such
general administrative support as is required for the discharge of his/her duties and responsibilities.
This will include the provision of adequate secretarial and clerical support and the availability of
accommodation, equipment etc.
The postholder will receive support from such other professional staff as are employed within the
Division and are deployed to his/her area of patient care.
5.2
Duties and Responsibilities
The main duties and responsibilities of the post include:




Provision with Consultant colleagues of a service in Cardiology with responsibility for the
prevention, diagnosis and treatment of illness and the proper function of the clinical service.
Development of a non-invasive cardiac imaging service in conjunction with Cardiology
colleagues in Ayrshire and Arran and the Regional Planning Group for Cardiology.
Ongoing care for a minority of General Medical in-patients.
Provision of cover for Consultant Cardiologist colleagues during periods of leave.






5.3
Professional supervision and management of Junior Medical Staff.
Responsibilities for carrying out teaching, accreditation and examination duties as required, and
for contributing to undergraduate and postgraduate medical education. The postholder will be
expected to comply with College recommendations on Continuing Medical Education.
The postholder will be required to comply with NHS Ayrshire and Arran Policies on Clinical
Governance.
The successful applicant will be encouraged to participate in research and to develop a relevant
subspecialty interest, subject to resources and local priorities.
Requirements to participate in medical audit and in continuing medical education.
Managerial, including budgetary, responsibilities (where appropriate).
Annual Appraisal & Job Planning
The postholder will also be required to participate in annual appraisal. Job planning is linked closely
with, but is separate from, the agreed appraisal scheme for consultants. The job plan review will take
into account the outcome of the appraisal discussion and reflect the agreed personal development
plan.

Grade and Salary ~ New Consultant Contract
The conditions of service are those laid down and amended from time to time by
the appropriate negotiating body and the General Whitley Council.
1st Time Consultant Appointment = Placed on the minimum of the Consultant
payscale.
Consultants who have held previous Consultant posts ~ subject to verification, all
previous regular service in the consultant grade in the NHS, including any
absence on authorised leave, will be counted in full in determining the starting
salary, seniority point and seniority date. Service under an honorary NHS
consultant contract and any service on a part-time basis will be regarded as
regular service in the consultant grade. Documentary evidence of all such
previous relevant service must be presented for verification. All new consultants
appointed shall be placed on the minimum point of the scale until previous
service has been confirmed. The seniority point/seniority date shall be recalculated as appropriate, to the commencement date.
Salary will be paid directly into your bank account on the last Thursday of each
month.
3.
Hours of Duty
Full time hours of duty for pay purposes will be 40 hours per week (expressed as
10 programmed activities (PA), which shall normally comprise of 9 PA for direct
clinical care, and 1 PA for supporting professional activities SPA). Any agreed
extra programmed activities (EPA) shall be paid at 1/10th of the basic salary
(inclusive of discretionary points). These EPAs are reviewed annually and may
be terminated at any time by either the consultant or the employer giving the other
a min of 3 months’ notice.
4.
Superannuation
You will be enrolled automatically into membership of the NHS Pension Scheme
(Scotland) if you are over 16 and under 75 years of age. Your level of
contribution will be dependant on your whole time equivalent superannuable
salary. Details of the scheme are given in the scheme guide which is available
from the Scottish Public Pensions Agency

Private Practice
With effect from 1/4/04, any Private Practice work will be governed by the Code of
Conduct in relation to private practice activities as contained with the Terms and
Conditions of Service. You will be required to declare any private practice
interests on appointment (refer to Private Practice Protocol attached).
6.
Car Leasing
A car lease may be available in accordance with NHS Ayrshire and Arran’s Car
Leasing Policy.
7.
Location
For employment purposes, the base will be either Crosshouse Hospital, Ayr
Hospital, Ayrshire Central Hospital, or Ailsa Hospital. The post may require you
to travel to and work at various locations in Ayrshire and Arran and occasionally
further afield. Home to work expenses will be met by yourself. Travel expenses
during the course of work will be met by NHS Ayrshire and Arran.
8.
Travel Expenses Incurred When Attending for Interview
Reasonable travel expenses incurred when attending for interview will be
reimbursed. The travel expenses for the successful candidate will be paid when
they take up post and will be included in their first monthly salary. Please note,
however, reimbursement of expenses shall NOT be made to individuals who
refuse an offer of employment on the grounds which, in the opinion of NHS
Ayrshire and Arran, are inadequate. Travel expenses will normally be
reimbursed from the point of entry to the UK.
9.
Relocation Expenses
Removal expenses, associated with necessary relocation to the area, may be
payable to the successful candidate. It will be confirmed at interview whether or
not removal expenses are applicable. A maximum of £12,000 may be reimbursed
for reasonable removal expenses.
10.
Annual Leave
The annual leave entitlement appropriate to this grade is 6 weeks. Public
holidays, as determined for NHS Ayrshire and Arran staff, are granted in addition
to the annual leave entitlement.
11.
Smoking, Alcohol, Drugs and Health at Work
NHS Ayrshire and Arran operates designated policies on *smoking, alcohol and
drugs. All staff are required to observe these policies.

General Statement Re Disclosure
If a consultant wishes to undertake any private practice they are obliged to inform their employer
at the time of appointment of their intentions to do so. This should be submitted in writing to the
Medical Director. (Section 4 para 4.4.8) (For consultants in post prior to 1 April 2004, similar
notification should have been submitted to the Medical Director).
Consultants will be free to undertake private practice without approval provided such work is
undertaken outside the time agreed in the job plan for programmed activities. (Section 6 para
6.1.1)
Code of Conduct: Standards of Practice
The Code of Conduct for Private Practice contained at Appendix 8 of the Terms and Conditions
of service sets out recommended standards of best practice for consultants about their conduct
in relation to private practice.
Consultants undertaking private practice are required to adhere to the Code. (Section 6 para
6.1.2)
The Code is based on 4 Key Principles:
NHS Consultants and NHS employing organisations will work on a partnership basis to prevent
any conflict of interest between private practice and NHS work. It is also important that NHS
Consultants and NHS organisation minimise the risk of any perceived conflicts of interest;
although no consultant should suffer any penalty (under the code) simply because of perception;
The provision of services for private patients should not prejudice the interest of NHS patients or
disrupt NHS Services; with the exception of the need to provide emergency care, agreed NHS
commitment should take precedence over private work; and NHS facilities, staff and services
may only be used for private practice with the prior agreement of the NHS employer.
The code will be used at the annual Job Plan review as the basis for reviewing the relationship
between NHS duties and any private practice.
Job Planning and Disclosure relating to actual or perceived conflict of interest
Consultants should declare any private practice, which may give rise to any actual or perceived
conflict of interest, or which is otherwise relevant to the practitioner’s proper performance of
his/her contractual duties. (Code of Conduct Part II)
As part of the annual Job Planning process, consultants should disclose details of regular
private practice commitments, including the timing, location and broad type of activity, to
facilitate effective planning of NHS work and out-of-hours cover.

4.1
Managing Private Patients in NHS Facilities
Consultants may only see patients privately within NHS facilities with the explicit agreement of
the Medical Director and where approval is granted will be required to adhere to organisational
policy and procedure in relation to private practice.
5.
5.1
General
Consultants participating in Private Practice must familiarise themselves with the Code of
Conduct and determine what action is required of them in ensuring they fully comply.
Clarification can be sought from the Medical Director’s office of Human Resources where
required.
Consultants will be required to demonstrate adherence to the Code of Conduct, and sections
4.4.6 – 4.4.10 of the National Terms and Conditions of Service in order to secure pay
progression.
