Bridges to Independence 2.2 Sample checklists of skills In the table below there are five sample checklists. These have been created from a selection taken from the main listings of skills using the skills, attributes and knowledge which are relevant to five areas of work. Double click on one of the yellow Bridges Project logos to open up a checklist. To create a checklist which suits your agency’s remit for your staff to work from you can copy and paste rows from the listings of skills into a new word document, this way staff do not need to look through the whole listing each time they need to choose another skill. You may wish to show your agency’s priorities by highlighting some of the skills. Double click on a Bridges Project logo in the table below to open up a form which can then be saved as a copy onto your computer for use with a young person. The Action Plan template has been provided to assist workers who do not have a standard assessment recording template for client information. It should be adjusted to suit the worker’s or the agency’s needs. The template assumes a continuing assessment process where the baseline assessment has provided sufficient information to shape up an initial action plan with the client. The Baseline Assessment Framework is for recording the young person’s details such as address, reason for referral and consent for sharing of information. The Workbook to be worked on. Coversheet is a template for entering the skills The Skills Shortlist form is a template for the worker to use to enter a small selection of skills to discuss with the young person.
