Education Jordan University of Science and Technology Irbid-Jordan 2009-2011 Master's Degree Average 89.3 *** Thesis oriented: "GENETIC POLYMORPHISM OF RARA GENE AND ITS EFFECT ON THE CLINICAL OUTCOMES OF ISOTRETINOIN-TREATED ACNE PATIENTS" Oral isotretinoin is a vitamin A derived medication, used for the treatment of severe acne that is resistant to adequate courses of standard therapy with systemic antibacterial and topical therapy. Isotretinoin is prescribed with the full understanding of the risks of therapy and the monitoring requirements. The drug is teratogenic and its use is highly associated with significant side effects such as dyslipidemia, liver dysfunction and depression. The mechanism for the development of side effects associated with the isotretinoin use such as isotretinoin-induced elevation of liver enzymes remains elusive, and it is not known why some patients may develop this side effect. Genetic polymorphisms of the Retinoic Acid Receptor Alpha (RARA) gene, which is the main target receptor of isotretinoin, may be associated with the clinical outcomes of this medication. In this cross sectional study, we have determined the prevalence of three single nucleotide polymorphisms (SNPs) in the RARA gene (rs4890109, rs9303285 and rs2715554) in acne patients treated with oral isotretinoin. This is the first study to assess the association between these gene variants and changes in biochemical parameters and development of side effects during use of oral isotretinoin. We found that AST % change (the percentage change of AST after one month of treatment with oral isotretinoin) is associated with rs4890109 and rs2715554 polymorphisms. Also, we showed that increased levels of AST by treatment are associated with rs2715554 polymorphism. Allele T of rs9303285 was found to be protective against developing depression in patients treated with isotretinoin. Allele G of rs4890109 was found to have association with elevated levels of total cholesterol by treatment. In conclusion, our findings suggest an association between certain genetic polymorphisms of RARA gene, and some biochemical markers and psychiatric state of acne patients treated with oral isotretinoin.
