GUIDELINES/PERMISSION FORM For ALL CHS Students AND Invited Guests
Dear Parent/Guardian and Student:
To attend the Homecoming Dance, we are requiring all parents/guardians and students to read, discuss, and sign the
guidelines below.
Students attending this function with a student from another school district or who does not attend school MUST print his
or her guest’s name on the assigned line (see below). We reserve the right to deny admission to guests who do not meet
our eligibility standards. The guest must provide a school ID or other form of identification for admittance. All guests
must be in ninth grade or older. The guests and CHS students must follow and adhere to the guidelines as written.
I understand that:
1. ALL students and guests attending this CHS function WILL BE subject to a passive breathalyzer test to be
given at the door. Students and guests may also be required to take additional test during the function.
2. Students and guests suspected of being under the influence of alcohol or drugs may be required to take an
additional breathalyzer test to determine their blood alcohol concentration (BAC). Students may be taken to the
hospital for additional testing depending on their physical condition. Non-students who are determined to be
under the influence will be refused admittance and will be turned over to the Cinnaminson Police Department.
3. If the test is positive, the student will be disciplined in accordance with school policy and referred to the
Cinnaminson Police Department.
4. Students must arrive at the Homecoming Dance before 8:00 pm and remain until the end of
the dance . Students arriving after 8:00 pm will not be admitted and no refunds will be issued. In addition,
students will not be permitted to leave the Homecoming Dance and then return.
5. Semi-Formal attire is required for admittance. No Jeans, hats, or sneakers are permitted. Students
and/or guests who do not meet dress code requirements will not be admitted and there will be NO refunds.
6. This form must be returned when tickets are purchased. Tickets will be on sale from September 21, 2015, to
October 2, 2015 for $25.00. All guest tickets must be purchased by Friday, October 2, 2015, at 2:15
pm. Tickets will be sold to CHS students only during the week of Monday, October 5, 2015 through Friday,
October 9, 2015, at 2:15 pm for $30.00. No tickets will be sold to an individual unless this form is completed and
turned in at the time of ticket purchase. No tickets will be sold at the door.
7. Students must be present in school on Friday, October 9, 2015, to attend the dance on Saturday, October 10th.

EACH CHS STUDENT AND GUEST MUST COMPLETE AND RETURN THIS FORM AT THE TIME TICKETS
ARE PURCHASED. NO EXCEPTIONS.
