

Known CAD (circle all that apply, history of coronary stenosis greater than or equal to
50% = 1 point)
o Coronary stenosis greater than 50%
o Coronary artery stent
o CABG
o Angina

Date of last aspirin use:__________(ASA use in the past 7 days = 1 point)

Date, time and description of chest pain (chest pain in the past 24 hours = 1 point)

Elevated Cardiac Markers = 1 point

Describe any ST segmentation deviation by lead location and in millimeters (ST deviation
> 0.5mm = 1 point)
Risk at 14 days of: all-cause mortality, new or recurrent MI, or severe recurrent ischemia requiring
