Pediatric Award of Excellence Purpose and Eligibility Award Purpose: To recognize an outstanding Canadian who exemplifies personal and/or professional commitment and achievements in the field of pediatric hospice palliative care To publicly communicate and highlight exemplary efforts in the advancement of pediatric hospice palliative care. Award Spirit: Accomplished Dedicated Committed to excellence Champion and friend Compassionate and knowledgeable Award Eligibility: An exemplary individual whose excellence is devoted to pediatric hospice palliative care in Canada in either a paid or volunteer capacity. The nominee’s achievements have made a difference in the quality of life for children facing a life threatening illness and their families and or have positively impacted the pediatric hospice palliative care movement in Canada. Emphasis will be placed on individuals who have made an impact at the national level, those who champion the cause broadly. The recipient of the Pediatric Award of Excellence must meet all of the selection criteria therefore, it is possible that the award may not be granted in every year. The award will be given to only one individual in a given year. Nominations: Submitted nominations must include: the completed Pediatric Award of Excellence Nomination Form signed by the nominee two letters of support describing the nominee’s accomplishments, competencies and talents related to pediatric hospice palliative care one copy of the current CV of the nominee electronic copy of a recent photo of the nominee CHPCA Pediatric Award of Excellence – Purpose and Eligibility April 2015 Award Selection Committee: The award selection committee will be a subcommittee of the Canadian Network of Palliative Care for Children (CNPCC). Selection Committee Role: To review all nominations within a designated time frame To objectively evaluate and score each nomination according to the agreed upon evaluation criteria. To discuss, debate and build consensus around the annual Award recipient via scheduled meetings/teleconference discussions of the Canadian Network of Palliative Care for Children (CNPCC) To inform the CHPCA Executive Committee of decision To develop and implement communications materials to leverage the Award Recipient Announcement which will coincide with the CHPCA AGM To ensure the Award is prepared and presented at the AGM To leverage and communicate the Award Announcement with key audiences including local media, CHPCA members and stakeholders.
