The hospital will bleep the on-call midwife and ask her to contact you. The midwife is the main point of contact for expectant mothers and guides them through the pregnancy, birth and postnatal period working in partnership with other health and social care services to meet individual mothers’ needs. Patient Participation Groups (PPG) were developed to give patients the opportunity to express their views, comments, suggestions and concerns regarding health services provided by their General Practitioner and Warwickshire PCT. As well as allowing patients to voice their opinions, the PPG also aims to increase patient understanding of services provided by and available throughout the NHS. Patient satisfaction is important to us, therefore, by involving patients we hear about the issues that matter to you – the patients. These issues may include areas such as the comfort and warmth of the surgery, appointment systems and prescription services or they may be about hospital admissions/discharges or transport for the elderly or district nursing. How does the PPG work? The PPG meet bi-monthly for approximately 1 hour at the practice. Members of the PPG will have an informal chat about issues raised. Improvement suggestions are noted and where possible these are implemented. The Practice Manager and GP are in attendance. Who can join the PPG? Any patient registered at the surgery and aged 18+ is eligible to join the PPG. We aim to involve patients from many areas of the practice population. How do I become a member of the PPG? Appointments 48-Hour Access – A Doctor or Health Professional may be seen within 48 hours of a request being made. A patient may see the Doctor of their choice within 7 working days. Counsellor – Weekly sessions are held in-house for patients referred by a doctor. Family Planning – Family planning services are provided at the surgery by prior appointment with the Practice Nurse. If the HGV Medicals – Available by request with Dr Street. A fee is applicable. Home Visits – Please make every effort to attend the surgery for your appointment. However, if you are unable to attend, where possible please try to request a home visit before 10am. Holiday Vaccinations – An assessment form can be obtained from reception then an appointment is required with the Practice Nurse. A fee may apply and a full list of charges is available from reception. Please remember to book well in advance of your departure as some vaccinations take a while to be affective. Emergencies – All emergencies are handled as they occur. surgery is closed you will be directed to the out of hours service. Minor Surgery – Minor surgical procedures are carried out in the surgery by appointment only. Routine Appointments – Appointment times are given on the previous page and can be booked up to four weeks in advance. Services Antenatal – Clinics are held every fortnight on Fridays with the midwife. Patients may also be seen for routine antenatal checks by the doctor during normal surgery hours. Asthma – Clinics available by appointment with the Practice Nurse. Cervical Smear Tests – It is advisable that all women between the ages of 25-50 years have a cervical smear test every three years, whilst those between 51-65 have a cervical smear test every five years. An appointment is required with the Practice Nurse for this screening test. Medical Insurance Notices – A fee is applicable. New Patients – If you wish to register with the Practice please speak to one of the Receptionists. As a new patient you will be asked to attend a New Patient Medical check with the Practice Nurse. Physiotherapy – Clinics are held at the Practice on Monday afternoons for patients referred by a doctor. Patient Checks - All patients aged 16-74 years may request a 3year health check. Patients over the age of 75 may request a 12month health check if they have not been seen by another health professional in the last 12 months.
