Social Care
Services
Public Bodies (Joint Working) (Scotland) Bill
The purpose of the above Bill is to create statutory Health and Social
Care Partnerships in each local authority in cooperation with health
boards to replace existing Community Health Partnerships (CHP’s).
In Lanarkshire two Health and Social Care Partnerships have been
developed, one each in North and South Lanarkshire and these will
replace existing CHP’s. Discussions between NHS Lanarkshire and the
two Local Authorities are on-going in considering the range of services
that will be managed through the Health and Social Care Partnerships.
In the North a Shadow Board is being established with ongoing
recruitment taking place. In the South a Shadow Board is now in place.
A strategic plan will be developed for each Health and Social Care
Partnership including considering the range of services that will be
managed from April 2015. This service may in the future be included
within this range.
Management

The supporting operational and clinical management structures are
focused on enhancing patient safety, quality improvement and local
delivery at hospital level, with visibility of clinical leadership and support
for individual professional accountability. Our Medical Managers will

The Post
We are seeking to recruit a Consultant in Gastroenterology and General
Medicine based at Monklands Hospital. The appointee to the post will join Drs
A Prach, Dr M Gangi and Dr S Radhakrishnan in providing and developing
specialist Gastroenterology services based at Monklands Hospital.
The appointee will share responsibility for providing acute medical receiving
duties with the other Physicians (currently a 1:22 rota plus prospective cover).
The rota for acute receiving duties is based around a Physician of the Week
model (2.4 weeks/ year at present), supported by Acute Physician input and
also a Physician of the Day
The successful applicant will fully participate in delivering both the inpatient
and outpatient gastroenterology services along with general medical duties. It
is anticipated that the successful candidate will develop a specialty interest to
complement the existing services. An interest in liver disease would be
welcome, but equally the post would be entirely suitable for an appointee with
a strong interest in lumenal gastroenterology. Specific clinical interests and
an interest in research will be encouraged.
The precise details of the job plan will be dependent on the successful
candidate’s subspecialty training and interests. Appropriate office
accommodation, secretarial and IT support will be provided.
The job is offered as a full-time post on a 10 PA basis but applications will be
considered from those wishing to work less than full-time. If full-time up to 2
EPA’s may be available to undertake additional outpatient and/or procedure
sessions or management activity.
All new substantive Consultants are initially offered a minimum of 1 PA for
SPA duties but this will be reviewed within 6 months (or earlier if required) of
appointment and revised upwards if additional responsibilities have been
undertaken. Additional SPA time (up to 2.5 PA’s in total) can be incorporated
into the job plan depending on the time required to support the successful
candidate’s professional activities and the needs of the service.

POST INFORMATION
Department of Lanarkshire has a population of around 550,000 and is served by three
Medicine
acute Hospitals: Wishaw Hospital (Wishaw), Hairmyres Hospital (East
Kilbride) and Monklands Hospital (Airdrie). The Department of Medicine
at Monklands Hospital encompasses the specialties of Acute Medicine,
Cardiology, Dermatology, Diabetes & Endocrinology, Gastroenterology,
Infectious Diseases, Nephrology and Respiratory Medicine. It forms part
of the Emergency and Medical Directorate, which also includes
Emergency Medicine and Care of the Elderly.
Consultant Physicians’ inpatient responsibilities are arranged in specialty
groupings and each medical specialty has a defined allocation of beds.
This has allowed the development of specialist care in the wards.
Dermatology, Infectious Diseases and Nephrology are concentrated
services with all their inpatient beds at the Monklands site, while the other
medical specialties have beds at all three sites. Haematology (part of a
separate Directorate) inpatient provision is also concentrated at the
Monklands site, with close links maintained between Haematology and
the Department of Medicine.
There are visiting consultants in Neurology, Rheumatology and Oncology.
A Primary Coronary Intervention (PCI) service for Lanarkshire was
established in October 2008, which means that patients with acute STelevation MI are transferred directly to the cardiac catheterization
laboratory at Hairmyres Hospital rather than being admitted to CCU at
Monklands.
The Medical Directorate is involved with undergraduate teaching in all
years of the new curriculum at Glasgow University. The hospital is
recognized for pre-registration and postgraduate training. There is an
active postgraduate teaching programme including x-ray meeting, Journal
Club and Hospital Grand Round Meetings.
The hospital has a
postgraduate lecture theatre, a number of seminar rooms and an
excellent library.
General and Acute Medicine
Monklands hospital admits just under 14,000 acute medical admissions
per annum, with around half of these coming from Emergency Medicine
and the other half constituting GP referrals.

The rota arrangements for Acute/ General Medicine at Monklands have
recently been revised, moving to a new ‘Physician of the Week’ model
that is more closely aligned to the Standards that have been developed
by the Society for Acute Medicine (SAM). The current Monday-Friday
arrangements are that the ‘Physician of the Week’ (8AM – 4PM) provides
enhanced front-door consultant continuity. S/he is supported in the
assessment of new admissions by Acute Physician sessions (8AM –
8PM) and a ‘Physician of the Day’ on-call General Physician (2PM to
10PM). Care of the Elderly consultants also provide support for ERU.
We aim to discharge a large proportion of our referrals directly from the
Acute Assessment area following senior decision maker review, or within
48 hours of admission to the ERU ward. Patients requiring a longer stay
in hospital are allocated a hospital inpatient bed on a specialty ward that
is determined, as far as possible, by the patient’s clinical needs.
NHS Lanarkshire Board has approved funding for a substantial additional
investment in Acute and/or General physicians, amounting to 6 new
Consultant posts (phased recruitment, with 3 posts initially). It is
anticipated that these appointments will allow us to further improve our
current ‘front door’ model.
Medical HDU and Critical Care
There is excellent 5-bedded Intensive Care Unit at Monklands Hospital,
run by Intensivists/ Anaesthetists, but with ongoing consultant physician
input into the care of relevant patients.
A medical High Dependency Unit (HDU) has also been established,
currently comprising four Level 2 Critical Care beds. This is co-located
with the existing Monklands Coronary Care Unit (CCU) on Ward 18. The
care of patients on medical HDU is primarily overseen by sessional input
from Acute Medicine, with plans to develop this service further.
Department of Gastroenterology
There are 4 Consultant Gastroenterologists at Monklands Hospital
(including this post). The existing Gastroenterologists all cover lumenal
gastroenterology and hepatology with inpatient and outpatient
commitments.

Gastroenterology at Monklands Hospital has its inpatient base on Ward
19 (a 24-bedded ward). Acute gastroenterology patients admitted via
ERU are, as far as possible, transferred to Ward 19 for ongoing specialist
care. Strong links are maintained with the other medical and surgical
specialties at Monklands Hospital, as well as with the Liver Transplant
Unit, Royal Infirmary of Edinburgh and GI services at the Glasgow Royal
Infirmary.
Outpatient clinics primarily take place within the main Outpatient
Department, although with some smaller nurse-led clinics (IBD and
venesection) are undertaken within the endoscopy department.
An IBD nurse specialist is in post. Nurse specialists also support the
venesection programme as well as coeliac disease follow-up.
Endoscopy Unit
Upper and lower GI endoscopy takes place within a dedicated Endoscopy
Unit at Monklands Hospital which has the full range of therapeutic
facilities. The consultant-provided endoscopy lists are supported by GI
nurse-endoscopist sessions. Electronic reporting software (Unisoft) is
used. The gastroenterologists plus a general surgeon provide emergency
endoscopy on a dedicated morning slot five days per week.
ERCP is not provided on site, but is performed in NHS Lanarkshire at
Hairmyres Hospital. For candidates with a particular interest in ERCP, it
would be feasible to arrange for a sessional ERCP commitment at
Hairmyres Hospital.
A Lanarkshire-wide review of endoscopy services has recently taken
place. The aim is for the 3 site-based endoscopy departments to link
closely and provide an effective and comprehensive endoscopy service
for the whole of NHS Lanarkshire. It is anticipated that a 24/7 acute
bleeding rota for NHS Lanarkshire will be developed in due course.
Investigational facilities
Monklands Hospital has an excellent Radiology Department, with close
links to the Department of Gastroenterology. Access to CT scanning is
available 24/7 for GI emergencies. There is an on-site MRI scanner, with
MRCP and MRI enteroclysis modalities available. There is also a strong
Nuclear Medicine service based at Monklands Hospital.
The Oesophageal Physiology lab is based at Hairmyres Hospital.
Capsule Endoscopy is available in Glasgow.

DUTIES AND RESPONSIBILITIES
Main Duties
This will include programmed activities for outpatient clinics in
Gastroenterology, ward cover (Ward 19) and Emergency Receiving Unit
(ERU) duties when on call for General Medicine.

As required under Section 3 of the Terms and Conditions of Service, the
duties and responsibilities are supported by a job plan and work
programme detailed as follows:
Job Planning/Programmed Activities
The job plan will be dependent on the successful candidate’s subspecialty
training and interests. As per standard arrangements for new consultant
posts in NHS Scotland, the initial job plan will be based around a DCC to
SPA sessional split of 9:1.

b)
This appointment is superannuable under the NHS Superannuation Scheme, which is
contracted out of the State Earnings Related Pension Scheme (SERPS). All new
eligible employees commencing employment from 1 March 2013 onwards will
automatically become a member of the pension scheme and your remuneration will be
subject to deduction of superannuation contributions, current personal contributions
range from 5% for the lowest earners to 14.5% for the highest. Levels of contributions
and scheme regulations are subject to change, any changes will be communicated to
you or can be found on the SPPA website. Contributions are taken from your pay
before tax, so you get tax relief on any amount you pay. This can reduce the actual
amount that you pay between approximately 3.5% and 5.1% depending on your
contribution rate, earnings level and personal rate of tax. In addition to personal
contributions NHS Lanarkshire will contribute 13.5%.
In accordance with recent legislative changes, new eligible employees commencing
from 1 March 2013 onwards will automatically be enrolled in the pension scheme.
After considering the benefits and personal circumstances, you can if you wish opt out
of the scheme. To opt-out, you must complete an opt-out form which is available on
the Scottish Public Pensions Agency website and return this to the Payroll
Department, Kirklands Cottage, Kirklands Hospital. Should you choose to opt-out in
accordance with these changes you will be enrolled into the scheme again every 3
years (June 2016, 2019 etc).
Under the NHS Superannuation Scheme Regulations pension records for short term
appointments, such as Bank Staff and the equivalent kind of contracts for other staff
groups, may remain open for short breaks in pensionable employment up to but not
more than 3 months from the date on which they last paid contributions. During this
period the member will continue membership in qualifying but not pensionable terms.
Where the period of break exceeds 3 months, membership of the scheme will be
closed and leaver information will be submitted to the Scottish Public Pensions
Agency.

From 1 April 2015 the NHS Pension Scheme is changing in line with other public
pension schemes. All those joining the scheme for the first time from 1 April 2015 will
join the new scheme. Existing members who are not protected will transfer to the new
scheme, with benefits in previous scheme(s) preserved. The new scheme will be
based on a proportion (1/54th) of the member’s pensionable earnings each year. The
pension will be the sum of each year’s value built up over the length of the member’s
career and revalued each year in line with the rate of inflation. This is called a Career
Average Related Earnings scheme. Normal pension age will be linked to the
member’s individual state pension age.

on the SPPA website.

The employment is subject to 3 months’ notice on either side subject always to the

Reimbursement of certain telephone charges may be payable on application to NHS

The successful candidate will be required to complete a disclosure Scotland PVG
(Protecting Vulnerable Groups Scheme) form. No approach will be made without
written permission of the successful applicant who will be asked to sign and complete a
Disclosure PVG application, giving authorization for the check to be undertaken.

NHS Lanarkshire does not negotiate salary placements. On commencement the salary
will be in line with paragraph 5.1 of the terms and conditions of the new consultant
contract. Appointees start on the scale minimum except in the circumstances of
paragraphs 5.1.2 – 5.1.7 of the terms and conditions of service.
h)
From 1st April 2014 the starting salary for the post is £76,001 per annum (based on 10
Programmed Activities per week). The successful candidate’s total salary will be
dependent on his/her previous service history. Remuneration for any extra
programmed activities will be dependent upon the job plan agreed at the time of
appointment.

Dependent upon present place of residence, NHS Lanarkshire may require the
successful candidate to undertake to remove home to a distance acceptable to NHS
Lanarkshire, normally within 10 miles of the base Hospital, in which case removal
expenses will be payable in accordance with NHS Lanarkshire Policy. In some cases,
however, a residence more than 10 miles distant from the base hospital will be
acceptable and in this case removal expenses will be subject to the prior approval of

For the purpose of immigration status, overseas doctors and dentists are
those who, regardless of where they obtain their primary medical/dental
qualification, are not nationals of the European Economic Area (EEA). A
list of EEA countries is attached.
On 7th March 2006 the Department of Health announced changes to the
immigration rules for postgraduate doctors and dentists. The changes
came into effect on 3rd April 2006 and mean that all doctors and dentists
who wish to work in the UK from outside the EEA will need to meet the
requirements of an employment category such as the work permit
provisions. New applications for permit-free training visas will no longer be
granted for doctors undertaking training posts
Non-EEA doctors and dentists will still be able to come and train in the UK
and are still eligible to apply, in open competition for training posts
provided all the other eligibility criteria is met. However, training posts will
now be considered employment for immigration purposes. Work permits
will only be issued where there is a specific post for the overseas doctors
that cannot be filled by someone from the EEA.
The changes in immigration requirements for trainee doctors and dentists
are a result of a Scottish Executive Health Department announcement
and immigration issues are the remit of the Home Office. Clarification and
